Early postoperative telehealth visit protocol implementation reduces emergency department utilization following benign foregut procedures.

INTRODUCTION: Emergency department (ED) visits and readmissions following benign foregut surgery (BFS) represent a burden on patients and the health care system. The objective of this study was to identify differences in ED visits and readmissions before and after implementation of an early postoperative telehealth visit protocol for BFS. We hypothesized that utilization of telehealth visits would be associated with reduced post-operative ED and hospital utilization. METHODS: An early postoperative telehealth protocol was initiated in 2020 at an academic medical center to provide a video conf

Long-term outcomes of laparoscopic Heller's myotomy with angle of His accentuation in patients of achalasia cardia.

BACKGROUND: Laparoscopic Heller's myotomy (LHM) is an established treatment for achalasia cardia. Anti-reflux procedures (ARP) are recommended with LHM to reduce the post-operative reflux though the optimal anti-reflux procedure is still debatable. This study reports on the long-term outcomes of LHM with Angle-of-His accentuation (AOH) in patients of achalasia cardia. METHODS: One hundred thirty-six patients of achalasia cardia undergoing LHM with AOH between January 2010 to October 2021 with a minimum follow-up of one year were evaluated for symptomatic outcomes using Eckardt score (ES), DeM

Comparing anterior gastropexy to no anterior gastropexy for paraesophageal hernia repair: a study protocol for a randomized control trial.

BACKGROUND: More than half of patients undergoing paraesophageal hernia repair (PEHR) will have radiographic hernia recurrence at 5 years after surgery. Gastropexy is a relatively low-risk intervention that may decrease recurrence rates, but it has not been studied in a prospective manner. Our study aims to evaluate the effect of anterior gastropexy on recurrence rates after PEHR, compared to no anterior gastropexy. METHODS: This is a two-armed, single-blinded, registry-based, randomized controlled trial comparing anterior gastropexy to no anterior gastropexy in PEHR. Adult patients (≥18 year

Post-POEM reflux: who's at risk?

INTRODUCTION: Per-Oral Endoscopic Myotomy (POEM) is a less invasive alternative to laparoscopic Heller myotomy for patients with achalasia. While a partial fundoplication is often performed concurrently with laparoscopic myotomy, an endoscopic approach does not offer this and leaves patients prone to post-operative reflux. The objectives of this study were to (1) identify patients with post-POEM reflux using BRAVO pH and endoscopic evaluations, and (2) investigate risk factors associated with post-POEM reflux and esophagitis to optimize patient selection for POEM and identify those who will be

Successful outcome following pneumonectomy in a teenage boy with cystic fibrosis: a case report.

BACKGROUND: Cystic fibrosis lung disease is generally a diffuse process however rarely one lung may become particularly damaged through chronic collapse and consolidation resulting in end-stage bronchiectasis with relative sparing of the contralateral lung. This clinical situation is sometimes referred to as "destroyed lung". Lung resection surgery is seldom indicated in cystic fibrosis and the associated medical literature is relatively sparse. CASE PRESENTATION: A 14 year old boy was referred to our centre for lung transplantation assessment. He had a chronic history of complete collapse an

Rapid progressive long esophageal stricture caused by gastroesophageal reflux disease after pylorus-preserving pancreatoduodenectomy.

BACKGROUND: Delayed gastric emptying (DGE) is a major postoperative complication after pylorus-preserving pancreatoduodenectomy (PpPD) and sometimes causes reflux esophagitis. In most cases, this morbidity is controllable by proton-pump inhibitor (PPI) and very rarely results in esophageal stricture. Balloon dilation is usually performed for benign esophageal stricture, and esophagectomy was rarely elected. In the present case, there were two important problems of surgical procedure; how to perform esophageal reconstruction after PpPD and whether to preserve the stomach or not. CASE PRESENTAT

Evaluating the impact of a minimally invasive pediatric surgeon on hospital practice: comparison of two children's hospitals.

PURPOSE: Widespread adoption of minimally invasive surgery (MIS) techniques in pediatric surgery has progressed slowly, and the shift in practice patterns has been variable among surgeons. We hypothesized that a pediatric surgeon committed to MIS could effectively change surgical practice by creating an emphasis on MIS. METHODS: Annual case volumes from 2000 to 2009 at two tertiary care pediatric hospitals, one with a dedicated minimally invasive pediatric surgeon, were evaluated for trends in MIS for ten different operations. Univariate analyses of the differences between hospitals in the us

The efficacy of peroral endoscopic myotomy (POEM) for type III achalasia: a systematic review and meta-analysis.

BACKGROUND: Peroral endoscopic myotomy (POEM), a newly invented treatment for patients with achalasia, has achieved great success; however, for the rarest manometric subtype (type III), its efficacy remains unknown. We aimed to evaluate the efficacy of peroral endoscopic myotomy (POEM) for type III achalasia. METHODS: A search was conducted in three databases (PubMed, Embase, and Cochrane Library) from inception to July 2025. The primary outcome was the pooled efficacy (defined by an Eckardt score ≤ 3 after the operation) at the longest available follow-up time. Secondary outcomes included th

Surgical management of gastroesophageal reflux in complex and medically fragile children: single-centre retrospective study and future perspectives.

INTRODUCTION: Gastroesophageal reflux disease (GERD) is a common pediatric condition that can significantly impair quality of life and lead to severe complications in medically fragile patients. Although conservative and pharmacological therapies are effective in most cases, some children require surgical intervention, which remains associated with non-negligible postoperative morbidity and the need for reoperation. This study aimed to evaluate the surgical management of pediatric gastrointestinal reflux in a tertiary referral center, focusing on surgical approaches, operative outcomes, postop

Definitive, urgent repair of acutely incarcerated paraesophageal hernias is comparable to an elective repair.

BACKGROUND: Emergent repair of paraesophageal hernias (PEHs) is rare. This study aimed to compare outcomes of elective and urgent PEH repairs in patients with type II-IV PEHs. METHODS: This was a single-center retrospective review of type II-IV PEHs from February 2014 to April 2024. The operations were classified as elective or urgent. Indications for urgent repair included acute incarceration and/or gastric outlet obstruction, which required immediate NGT decompression and/or EGD followed by definitive repair during the same hospital admission. Outcomes were 90-day hernia recurrence, mortali

Robotic esophageal reconstruction with a diagonal stapling technique: technical description and early outcomes from a single-center series.

BACKGROUND: Esophageal reconstruction after total or proximal gastrectomy is technically demanding. Robotic platforms provide enhanced dexterity, yet anastomotic protocols vary widely. We developed a unified, geometry-driven robotic technique-applicable to both total and proximal gastrectomy-that creates a wide V-shaped anastomosis by extending and rotating a right-anterior esophageal flap to maximize perfusion and minimize axial tension. METHODS: Between March 2021 and May 2025, 41 patients underwent robotic gastrectomy using our "diagonal V-geometry" reconstruction: 18 proximal gastrectomie

Ubiquitin-proteasome pathway activation in the diaphragm of humans with reflux esophagitis.

Some forms of gastroesophageal reflux disease (GERD) are associated with crural diaphragm (CD) dysfunction, suggesting that GERD may be influenced by skeletal muscle deficiencies. Skeletal muscle atrophy has been strongly linked to alterations in the ubiquitin-proteasome system, the primary pathway for protein degradation. This study aimed to assess the expression of muscle atrophy-related proteins in the CD of patients with reflux esophagitis compared with those without esophagitis. In addition, we examined the correlation between these proteins, esophagitis severity, and esophageal acid expo

Patient-tailored algorithm for laparoscopic cruroplasty standardization: comparison with hiatal surface area and medium-term outcomes.

BACKGROUND: Different methods have been described for laparoscopic hiatoplasty and hiatus hernia (HH) repair. All techniques are not standardized and the choice to reinforce or not the hiatus with a mesh is left to the operating surgeon's preference. Hiatal surface area (HSA) has been described as an attempt at standardization; in case the area is > 4 cm2, a mesh is used to reinforce the repair. OBJECTIVE: The aim of this study was to describe a new patient-tailored algorithm (PTA), compare its performance in predicting crura mesh buttressing to HSA, and analyze outcomes. METHODS: Retrospect
