An investigation into the manganese superoxide dismutase (MnSOD Val-9Ala) gene polymorphisms employing high-resolution melting in patients with gastric cancer: A preliminary study.

BACKGROUND: Gastric cancer with complex carcinogenesis and a multi-factorial immunopathophysiology is well-known as the third life-threatening type of cancer in Asia. In this regard, it has been demonstrated that the role of Reactive Oxygen Species (ROS) in these processes should not be underestimated. Besides, mitochondrial Manganese Superoxide Dismutase (MnSOD) with antioxidant properties show protective effects against ROS. On the other hand, MnSOD catalyzes the dismutation of superoxide radicals to H2O2 and oxygen reactions. A replacement of T with C at nucleotide 47 (Val-9Ala) leads to a

2025 ADS/ANZCA/GESA/NACOS clinical practice recommendations on the peri-procedural use of GLP-1/GIP receptor agonists.

Glucagon-like peptide-1 receptor agonists (GLP-1RAs) are widely used for the treatment of type 2 diabetes and/or obesity. The physiological actions of endogenous GLP-1, and synthetic GLP-1RAs include inhibition of gastric emptying. This has peri-procedural implications due to the potential increased risk of retained gastric contents which may result in pulmonary aspiration. There is a need for local evidence-based guidelines to best manage patients on GLP-1RAs and dual GLP-1 and glucose-dependent insulinotropic polypeptide receptor co-agonists (GLP-1/GIPRAs) presenting for surgical and medical

Assessment of Gastroesophageal Reflux Disease in Patients Exhibiting Otolaryngology Symptoms at a Tertiary Care Hospital.

Background Gastroesophageal reflux disease (GERD) is characterized by repeated episodes of heartburn sensation or regurgitation; it can also present as extraesophageal symptoms, which include dysphagia and hoarseness alongside throat clearing. Patients frequently present with atypical GERD symptoms, which trigger admission to otolaryngology (ENT) practices; however, this often results in patients missing the opportunity for proper diagnosis and treatment by gastroenterologists. Objectives This study aims to analyze the frequency of ENT complaints among GERD patients referred to gastroenterolog

Comparative management practices of Wilson disease in Californian and Italian providers.

BACKGROUND: There is a scarcity of randomized and high-quality studies to aid clinicians in management and treatment of Wilson disease (WD). Even amongst society practice guidelines in North America and Europe, diagnosis and management of WD varies. The aim of this study is to elucidate WD diagnosis and treatment patterns by conducting a survey of clinicians in California and comparing the results to clinicians in Italy as a representation of European practices. METHODS: We developed a 51-item survey assessing WD diagnostics, therapeutics, and disease monitoring. The survey was distributed thr

Interobserver agreement of current and new proposed endoscopic scores for postoperative recurrence in Crohn's disease.

BACKGROUND AND AIMS: The modified Rutgeerts score (mRS) is widely used for the assessment of endoscopic postoperative recurrence (ePOR) in Crohn's disease (CD) after ileocolic resection to guide therapeutic decisions. To improve the validity and prognostic value of this endoscopic assessment, 2 new scores have been proposed. This study assessed the interobserver agreement of the current score (mRS) and the new endoscopic score for ePOR in CD. METHODS: Sixteen Dutch academic and nonacademic inflammatory bowel disease specialists assessed endoscopic videos (n = 71) of postoperative CD patients

Effective palliation of refractory ascites in cirrhosis is challenging.

BACKGROUND: Palliative care is often suboptimal for patients with end-stage liver disease (ESLD). Ascites remains the most common complication in ESLD. Though long-term abdominal drains (LTAD) are commonly used in refractory malignant ascites, the standard care for ESLD is hospital drainage (large volume paracentesis (LVP)). There is an ongoing National Institute for Health and Care Research (NIHR) funded trial (REDUCe 2 Study) (ISRCTN269936824) comparing palliative LTAD to LVP in ESLD. This 35-site trial is being conducted in England, Scotland and Wales. AIM: To understand the views and expe

Genitourinary manifestations of Lynch syndrome in the urological practice.

OBJECTIVE: Lynch syndrome (LS) is an autosomal dominant hereditary disorder resulting from germline mutation in at least one of the four mismatch repair genes or in EPCAM gene. From a clinical perspective, LS patients exhibit an increased predisposition to multiple primary malignancies and early age of onset compared to general population. We aimed to provide a comprehensive overview of all the genitourinary manifestations of LS, focusing on incidence, diagnosis, clinical features, therapeutic strategies, and screening protocols. METHODS: Previous literature was assessed through Medline, Scop

Clinical impact of gastrointestinal endoscopy on the early detection of pharyngeal squamous cell carcinoma: A retrospective cohort study.

BACKGROUND: In recent years, with the growing availability of image-enhanced gastrointestinal endoscopy, gastroenterologists have contributed to the early detection of pharyngeal squamous cell carcinomas (SCC). AIM: To clarify the clinical characteristics of pharyngeal SCCs detected by gastrointestinal endoscopy. METHODS: This is a retrospective cohort study conducted in a single-center, a university hospital in Japan. We retrospectively assessed the clinical records of 522 consecutive patients with oropharyngeal or hypopharyngeal SCC who were examined in our hospital between 2011 and 2018.

Targeted Biopsies Identify Larger Proportions of Patients With Colonic Neoplasia Undergoing High-Definition Colonoscopy, Dye Chromoendoscopy, or Electronic Virtual Chromoendoscopy.

BACKGROUND & AIMS: It is unclear what are the best and most appropriate endoscopic procedures for detecting colonic neoplasia in patients with long-term colonic inflammatory bowel disease (IBD). Dye chromoendoscopy (DCE) is the standard used in IBD surveillance colonoscopies. However, studies are needed to determine the optimal endoscopic technique for detecting dysplastic lesions. We investigated current practices used in surveillance colonoscopies by IBD gastroenterologists at a single tertiary center. We also determined the rate of neoplasia detection among different surveillance endoscopic

Abdominal migraine: an under-diagnosed cause of recurrent abdominal pain in children.

OBJECTIVE: Our objective was to demonstrate that, despite recognition by both the gastroenterology and headache communities, abdominal migraine (AM) is an under-diagnosed cause of chronic, recurrent, abdominal pain in childhood in the USA. BACKGROUND: Chronic, recurrent abdominal pain occurs in 9-15% of all children and adolescents. After exclusion of anatomic, infectious, inflammatory, or other metabolic causes, "functional abdominal pain" is the most common diagnosis of chronic, idiopathic, abdominal pain in childhood. Functional abdominal pain is typically categorized into one, or a combin

Patients and nonconsulters with irritable bowel syndrome reporting a parental history of bowel problems have more impaired psychological distress.

Little is known about the prevalence and risk factors for development of irritable bowel syndrome (IBS) in Japan. In the United States, it is reported that heredity and social learning contribute to the development of IBS. Our aims were (1) to estimate the prevalence of IBS, (2) to confirm that subjects with IBS are more likely to have parents with a history of bowel problems, (3) to confirm that gastroenteritis is a risk factor for IBS, and (4) to determine whether these two risk factors interact with psychological distress. Prevalence was estimated from a sample of 417 young adults seen for

A study of discrimination within the medical community as viewed by obese patients.

BACKGROUND: Although prejudice may not be verbal in nature, the lack of response from professional and non-professional medical personnel regarding the obese patients' needs (medical equipment, comfortable surroundings, properly fitting attire, etc.), leads one to assume that obese patients continue to be a target of unfavorable opinion. METHODS: 200 patients from four busy east coast bariatric practices received detailed surveys requesting information regarding experiences incurred during the entire peri-operative period. The responses were blinded as to patient or site of surgery. Patients

Bridging the Gap Between Endoscopy and Histology Indices: A Multi-Index Comparison in 146 Ulcerative Colitis Patients.

BackgroundUlcerative Colitis (UC) is a chronic inflammatory bowel disease affecting the colonic mucosa, with a relapsing-remitting course. Disease activity is assessed using clinical, serologic, endoscopic, and histological parameters, among which the latter two remain the cornerstone for assessing disease severity and therapeutic response. However, limited studies have evaluated the correlation between endoscopic and histological activity. This study evaluates the correlation between the Mayo endoscopic score (MES) and validated histological indices, namely the Geboes score (GS), Nancy index
