Long-term results of delayed primary anastomosis for pure oesophageal atresia: a 27-year follow up.

The management of a newborn with pure oesophageal atresia continues to be challenging. We started treating babies with pure oesophageal atresia by delayed primary anastomosis in 1977. The purpose of this study was to review the long-term outcome in infants with pure oesophageal atresia (EA) treated by delayed primary anastomosis with special emphasis on gastroesophageal reflux (GOR) related morbidity. The medical charts of all patients treated by delayed primary anastomosis between 1977 and 2004 were retrospectively reviewed. All survivors were followed up with completion of a questionnaire an

Balloon catheter dilatation in children with congenital and acquired esophageal anomalies.

BACKGROUND/PURPOSE: Esophageal stricture requiring dilatation often is the sequelae in children with a history of congenital or acquired esophageal anomalies. Balloon catheter dilatation (BCD) for esophageal stricture is well established in adults, yet its role in children is less well described. The authors report their initial experience using BCD in children with esophageal stricture, assessing outcome. METHODS: Between January 1994, and December 1998, 20 children (age range, 17 days to 12 years) underwent BCD for esophageal strictures and were divided into 2 etiology groups (A and B). (A,

Intestinal obstruction after lung transplantation in children with cystic fibrosis.

BACKGROUND/PURPOSE: Distal intestinal obstruction syndrome (DIOS) occurs in 15% of patients with cystic fibrosis (CF). The authors reviewed their experience to determine the incidence, risk factors, and natural history of adhesive intestinal obstruction and DIOS after lung transplantation. METHODS: Eighty-three bilateral transplants were performed in 70 CF patients between January 1990 and September 1998. All were on pancreatic enzymes preoperatively, and none had preoperative bowel preparation. Fifty-six patients (80%) had prior gastrostomy (n = 54) or jejunostomy (n = 2). Eighteen patients

Hypertensive lower esophageal sphincter pressures and gastroesophageal reflux: an apparent paradox that is not unusual.

UNLABELLED: The association of a hypertensive lower esophageal sphincter pressure (LESP) with significant gastroesophageal reflux (GER) has always seemed paradoxical because of the original association of a hypotensive sphincter with reflux. OBJECTIVES AND METHODS: In this study, the manometric and ambulatory pH studies of a group of consecutive patients evaluated over a 3-yr period were reviewed to identify and characterize patients with a hypertensive LESP and abnormal GER as defined by increased acid exposure in the distal esophagus as measured on ambulatory pH monitoring. RESULTS: Eighte

Functional Outcomes and Anti-Reflux Performance of Reconstruction Methods Following Proximal Gastrectomy: A Systematic Review.

Proximal gastrectomy preserves gastric function; however, reconstruction choice influences reflux control and quality of life (QOL), and the comparative efficacy of available techniques remains unclear. A PRISMA-guided search of PubMed and Web of Science Core Collection (January 2000-April 2025) identified studies reporting reflux or QOL outcomes after proximal gastrectomy. Eligible studies included adult gastric cancer cohorts with original clinical data. Two reviewers independently screened articles, extracted predefined variables, and performed a narrative synthesis. The protocol was archiv

Perioperative morbidity and midterm outcomes following antireflux surgery in overweight and obese populations.

BACKGROUND: As the worldwide prevalence of obesity increases, more patients with obesity are referred for antireflux surgery (ARS). Although bariatric surgery is typically recommended, at times straightforward ARS is offered. We aimed to compare surgical and patient-reported outcomes among overweight and obese patients undergoing minimally invasive ARS. METHODS: This retrospective cohort study included consecutive patients with BMI ≥ 25 kg/m2 who underwent primary elective ARS by an experienced foregut surgeon (September 2016-June 2025). Patients were stratified by BMI: overweight 25-30, clas

Predictive value of preoperative esophagram in postoperative outcomes after antireflux surgery.

BACKGROUND: Gastroesophageal reflux disease (GERD) significantly impairs quality of life. Antireflux surgery (ARS) improves symptoms when medical therapy fails, but postoperative recurrence and dysphagia remain concerns, emphasizing the need for improved preoperative risk stratification. This study evaluates whether esophageal transit time (ETT) measured on standard preoperative esophagram can predict postoperative outcomes after ARS and serve as an accessible method to characterize esophageal motility on esophagram and assist in preoperative decision-making. METHODS: In this retrospective co

Comparison on the reflux and nutritional status of different reconstruction methods after laparoscopic proximal gastrectomy: a systematic review and network meta-analysis.

BACKGROUND: The rising prevalence of gastric cancer in the upper third of the stomach has generated considerable interest in laparoscopic proximal gastrectomy (LPG). Traditional esophagogastric anastomosis after LPG has been associated with postoperative reflux issues. Despite the availability of various improved reconstruction techniques, there is still ongoing debate on the optimal approach. This network meta-analysis seeks to assess the reflux and nutritional outcomes associated with various reconstruction techniques subsequent to LPG. METHODS: A comprehensive literature search was perform

Gastroesophageal reflux disease and the phantom of Barrett's esophagus after most-often-used bariatric procedures: are future investigations necessary?

BACKGROUND: Studies have investigated the incidence of gastroesophageal reflux disease (GERD) and Barrett's esophagus (BE) after common bariatric surgeries. However, many of these studies have bias or limitations. Therefore, it is crucial to determine the true incidence of GERD in long-term follow-ups (FUs) post-surgery. AIMS: The aim of this study was to review and summarize long-term data regarding the incidence of post-surgical GERD and BE after various bariatric procedures, discuss the characteristics of current information available, and establish the need for future studies to determine

Case Report: The importance of early intervention for gastroesophageal reflex disease caused by hiatal hernia.

BACKGROUND: Gastroesophageal reflux (GER) disease (GERD) is a condition wherein GER causes troublesome symptoms that can affect daily functioning and/or clinical complications within the esophagus or other systems. To avoid this, patients with GERD often require treatment; hence, it is important to distinguish GER from GERD. Patients with GERD exhibiting alarm signs should be examined early to differentiate it from GER and treated accordingly. Herein, we present a case of GERD caused by a hiatal hernia that required surgical intervention for esophagial cicatrical stenosis despite oral treatmen

Early postoperative telehealth visit protocol implementation reduces emergency department utilization following benign foregut procedures.

INTRODUCTION: Emergency department (ED) visits and readmissions following benign foregut surgery (BFS) represent a burden on patients and the health care system. The objective of this study was to identify differences in ED visits and readmissions before and after implementation of an early postoperative telehealth visit protocol for BFS. We hypothesized that utilization of telehealth visits would be associated with reduced post-operative ED and hospital utilization. METHODS: An early postoperative telehealth protocol was initiated in 2020 at an academic medical center to provide a video conf

Long-term outcomes of laparoscopic Heller's myotomy with angle of His accentuation in patients of achalasia cardia.

BACKGROUND: Laparoscopic Heller's myotomy (LHM) is an established treatment for achalasia cardia. Anti-reflux procedures (ARP) are recommended with LHM to reduce the post-operative reflux though the optimal anti-reflux procedure is still debatable. This study reports on the long-term outcomes of LHM with Angle-of-His accentuation (AOH) in patients of achalasia cardia. METHODS: One hundred thirty-six patients of achalasia cardia undergoing LHM with AOH between January 2010 to October 2021 with a minimum follow-up of one year were evaluated for symptomatic outcomes using Eckardt score (ES), DeM

Comparing anterior gastropexy to no anterior gastropexy for paraesophageal hernia repair: a study protocol for a randomized control trial.

BACKGROUND: More than half of patients undergoing paraesophageal hernia repair (PEHR) will have radiographic hernia recurrence at 5 years after surgery. Gastropexy is a relatively low-risk intervention that may decrease recurrence rates, but it has not been studied in a prospective manner. Our study aims to evaluate the effect of anterior gastropexy on recurrence rates after PEHR, compared to no anterior gastropexy. METHODS: This is a two-armed, single-blinded, registry-based, randomized controlled trial comparing anterior gastropexy to no anterior gastropexy in PEHR. Adult patients (≥18 year
