Relation of Deep Paraspinal Muscles' Cross-Sectional Area of the Cervical Spine and Bone Union in Anterior Cervical Decompression and Fusion: A Retrospective Study.

OBJECTIVE: In the cervical spine, deep paraspinal muscles (DPMs) function to preserve spinal stability and mobility. However, the relationship between DPMs and bone union after cervical fusion surgery has not yet been investigated. The objective of this study is to evaluate the relationship between the cross-sectional area (CSA) of DPMs and bone union after cage alone anterior cervical decompression and fusion (ACDF). METHODS: A total of 243 patients who underwent cage alone ACDF at the C5-6 or C6-7 segments were enrolled. The CSAs of the longus colli (LCo), longus capitis (LCa), semispinalis

Preoperative CT planning of screw length in arthroscopic Latarjet.

PURPOSE: The Latarjet procedure has shown its efficiency for the treatment of anterior shoulder dislocation. The success of this technique depends on the correct positioning and fusion of the bone block. The length of the screws that fix the bone block can be a problem. They can increase the risk of non-union if too short or be the cause of nerve lesion or soft tissue discomfort if too long. Suprascapular nerve injuries have been reported during shoulder stabilisation surgery up to 6 % of the case. Bone block non-union depending on the series is found around 20 % of the cases. The purpose of t

Does intrawound application of vancomycin influence bone healing in spinal surgery?

PURPOSE: Surgical site infections represent a major complication of spinal surgery. The application of lyophilised vancomycin into the wound is reported to significantly decrease infection rates. As concentrations applied locally can exceed the minimal bacterial inhibitory concentration for more than a 1000-fold, toxic side effects on local tissue may be possible. METHODS: Primary osteoblast cell cultures were generated from bone tissue samples of 10 patients. Samples were incubated in absence or presence of either 3, 6 or 12 mg/cm(2) vancomycin according to a planned phase I clinical trial p

Vascular bone transfer options in the foot and ankle: a retrospective review and update on strategies.

BACKGROUND: The foot and ankle area has a tenuous blood supply that can easily be damaged with trauma or open exposures. The navicular and talus are susceptible to avascular necrosis, making arthrodesis difficult. In addition, in this region, large bone gaps occur as a result of avascular necrosis, trauma, or infection. Often, vascularized bone flaps are required for reconstruction or salvage. METHODS: A retrospective review of all vascularized bone flaps to the foot and ankle performed by the senior surgeon (L.S.L) from July of 2006 to July of 2012 was performed. Twelve cases were identified

Sandwich allografts for long-bone nonunions in patients with osteogenesis imperfecta: a retrospective study.

BACKGROUND: Patients with osteogenesis imperfecta often develop nonunions, as internal fixation has limited applicability in this condition. We report the outcomes of a modified "sandwich technique" in the treatment of long-bone nonunions in patients with osteogenesis imperfecta; this technique brings circumferential stabilization and normal collagen to the nonunion site. METHODS: From May 2003 through February 2012, twelve patients (eight females, four males; median age, 39.0 years; range, eleven to seventy-eight years) who had osteogenesis imperfecta (Sillence type I [three], type III [eigh

May smokers and overweight patients be treated with a medial open-wedge HTO? Risk factors for non-union.

PURPOSE: The purpose of this retrospective study was to investigate the rate of non-union after medial open-wedge high tibial osteotomy (HTO) with the Tomofix(®) plate. In addition, risk factors with a possible influence on the development of a non-union were analysed. METHODS: Between 2006 and 2008, a total of 186 medial open-wedge HTOs were performed in 182 patients. RESULTS: Ten cases of non-union (5.4%) were recorded. Risk factors with a statistically significant influence on the development of a non-union included smoking, body mass index and fracture of the lateral cortical hinge. No i

Vitamin D and Metabolic Supplementation in Orthopedic Trauma.

Active assessment and management of hypovitaminosis D among orthopedic patients is low-risk and low-cost while retaining significant potential to improve patient care. Vitamin D has an established role in musculoskeletal development and calcium homeostasis, and vitamin D deficiency is pervasive in orthopedic trauma populations. Clinical guidelines for screening and supplementation for hypovitaminosis D are lacking. Literature on the effects of vitamin K on bone health is limited. Anabolic hormone analogues may have a future role in delayed union or nonunion treatment. Vitamin D deficiency and

[Clinical experience with bone morphogenetic protein 7 (BMP 7) in nonunions of long bones].

BACKGROUND: Nonunions of long bone fractures are considerable therapeutic and economic problems with increasing tendency. Basic surgical options are autogenous cancellous bone grafting, rod dynamization, reamed nailing, plate fixation with compression, and bone transport techniques. If these methods fail to work, alternative treatment options are needed. MATERIAL AND METHODS: Since May 2001 treatment with recombinant human (rh) bone morphogenic protein 7 [BMP-7 or osteogenic protein 1 (OP-1)] in combination with a type one collagen carrier has gained interest. BMP 7 induces the formation of n

Scaphoid nonunions treated with vascularised bone grafts: MRI assessment.

PURPOSE: To assess the value of MR imaging (MRI) with regard to union, graft viability and proximal pole bone marrow status, after use of vascularized bone grafts for treating scaphoid nonunions. MATERIALS AND METHODS: Vascularized bone grafts from the distal radius were used to treat 47 scaphoid nonunions resulting from fractures or enchondromas. Clinical and imaging evaluation was used for the pre- and postoperative assessment of all patients. Apart of conventional radiographs obtained in all cases, 15 patients were also assessed postoperatively with MRI at 3 months. From these 15 patients,

Epidemiology of Fracture Nonunion in 18 Human Bones.

IMPORTANCE: Failure of bone fracture healing occurs in 5% to 10% of all patients. Nonunion risk is associated with the severity of injury and with the surgical treatment technique, yet progression to nonunion is not fully explained by these risk factors. OBJECTIVE: To test a hypothesis that fracture characteristics and patient-related risk factors assessable by the clinician at patient presentation can indicate the probability of fracture nonunion. DESIGN, SETTING, AND PARTICIPANTS: An inception cohort study in a large payer database of patients with fracture in the United States was conduct

NSM00158 Specifically Disrupts the CtBP2-p300 Interaction to Reverse CtBP2-Mediated Transrepression and Prevent the Occurrence of Nonunion.

Carboxyl-terminal binding proteins (CtBPs) are transcription regulators that control gene expression in multiple cellular processes. Our recent findings indicated that overexpression of CtBP2 caused the repression of multiple bone development and differentiation genes, resulting in atrophic nonunion. Therefore, disrupting the CtBP2-associated transcriptional complex with small molecules may be an effective strategy to prevent nonunion. In the present study, we developed an in vitro screening system in yeast cells to identify small molecules capable of disrupting the CtBP2-p300 interaction. Her

Functional results after proximal row carpectomy to salvage a wrist.

Proximal row carpectomy is a movement-preserving procedure in the treatment of arthrosis of the wrist. We have retrospectively assessed the objective and subjective functional results after proximal row carpectomy. Assessment of outcome included measurement of range of movement (ROM), grip strength and self-assessment of pain relief with a visual analogue scale (VAS) and the Disabilities of arm, shoulder, and hand (DASH) questionnaire. Results were graded using the Mayo and Krimmer wrist scores. Fourty-five patients (mean age 48 (30-67) years) were evaluated with a follow-up of 32 (8-115) mont

Health status after Ilizarov reconstruction of post-traumatic lower-limb deformity.

Previous studies of the Ilizarov procedure have concentrated on musculoskeletal assessments rather than the opinions of patients. In a prospective trial of 25 consecutive patients, we evaluated the effect of Ilizarov reconstruction of post-traumatic deformity on general health status using the SF36 and Nottingham Health Profile (NHP). The patients had very low preoperative scores, which remained low during treatment and correction, but increased postoperatively. The mean overall SF36 score improved from 36 +/- 3 to 58 +/- 7 (p = 0.031) and the NHP score from 39 +/- 11 to 67 +/- 10 (p = 0.002).

[Development of length of stay and reimbursement in elective hand surgery after the introduction of DRGs in Germany].

BACKGROUND: Since its introduction in Germany, the DRG (Diagnosis-Related Groups) system is often fraught with negative connotations. Frequent points of criticism are a deterioration of patient care by decreasing length of stay (LOS) in hospital and a decline in reimbursement. The following investigation analyzes and compares the development of length of stay and reimbursement in hand surgery based on the 3 most common elective procedures. MATERIAL AND METHODS: The main diagnoses scaphoid nonunion (PSA), Dupuytren's contracture (DK) and rhizarthrosis (RIA) were evaluated for number of cases,

[Tibial valgus osteotomy using a tricalcium phosphate medial wedge: a minimally invasive technique].

PURPOSE OF THE STUDY: We present a minimally invasive technique for tibial valgus osteotomy using a medial wedge composed of tricalcium phosphate. MATERIAL AND METHODS: The bone substitute is composed of slowly resorbable tricalcium phosphate material shaped to the desired form and having mechanical properties allowing stable osteotomy via a short incision and staple fixation. Intraoperative fluoroscopy enables a reliable and reproducible technique. A lateral fixation staple is required because there is a risk the lateral hinge could break. This technique was used for 58 knees in 55 patients

Screening and validation of autophagy-related mRNAs and lncRNAs in bone nonunion based on transcriptome sequencing analysis.

This study aims to screen for differentially expressed mRNAs (DEmRNAs) and lncRNAs (DELncRNAs) related to autophagy in bone nonunion through transcriptome sequencing, and explore their molecular mechanisms at the cellular level. In this study, 5 pairs of bone nonunion and bone healing tissue samples were collected for RNA sequencing to obtain DEmRNAs and DElncRNAs. The protein-protein interaction (PPI) network of autophagy-related differential genes was constructed. In addition, an lncRNAs-mRNAs interaction network was constructed. qRT-PCR was used to verify key autophagy-related genes. shBNIP
