BACKGROUND AND STUDY AIMS: Placing small stents in the pancreatic duct at endoscopic retrograde cholangiopancreatography reduces the risk of pancreatitis. However, this practice means that a second procedure might be required to remove the stent, and stents can also damage the duct. The aims of this study were to determine the frequency of spontaneous dislodgment and to assess the incidence of stent-induced ductal irregularities.
PATIENTS AND METHODS: We performed a retrospective analysis of consecutive patients seen over a 3-year period (2001 - 2004) who had undergone placement of a 3-Fr pan

UNLABELLED: Bile leaks and bile duct injury has been the major postoperative complications described after laparoscopic cholecystectomy. In this study, we evaluated the role of hepatobiliary scintigraphy (HBS) in patients who underwent laparoscopic cholecystectomy, and there was a clinical suspicion of bile leak in postoperative period.
METHOD: Twenty-five patients (M/F=11:14, mean age 39+/-8 years; range 24-58 years) with suspected bile leak postlaparoscopic cholecystectomy underwent sequential HBS.
RESULTS: Thirteen patients had normal hepatobiliary scintigraphic studies. Five patients had

OBJECTIVES: Diagnostic pancreatoscopy was performed in 56 patients using the newly developed peroral electronic pancreatoscope (PEPS). The diagnostic value of the PEPS for pancreatic diseases was evaluated.
METHODS: The PEPS (external diameter = 2.1 mm, bidirectional tip deflection) was developed with a minute 50,000-pixel interline charge-coupled device. Pancreatoscopy was performed by means of an endoscopic retrograde approach without sphincterotomy. The cases examined were normal (three), chronic pancreatitis (32), pancreatic cancer (eight), and intraductal papillary mucinous tumors (13).

Follow-up results were analyzed to evaluate the surgical managements of pancreatic complications such as pancreatitis and protein plug formation in patients with choledochal cysts. Sixty-two patients with choledochal cysts treated between 1976 and 1999 were reviewed. Twenty-four were children and 38 were adults. Fifty-four patients showed primary cases. Cyst excision and hepaticoenterostomy were finally performed in 56 patients. Surgical sphincteroplasty or endoscopic sphincterotomy was performed to prevent recurrent protein plugs in six patients. The follow-up period was 8.1 +/- 6.1 years. Ac

OBJECTIVE: Out-patient endoscopic retrograde cholangiopancreatography (ERCP) is widely practised but the safety of this approach has only been addressed in a limited number of prospective series mainly from specialist North American centres. Our objective was to determine prospectively the safety and admission rates of out-patient ERCPs.
STUDY DESIGN AND PARTICIPANTS: Patients were selected for out-patient ERCP if in relatively good health, without major risk factors for complications following ERCP and with adequate social support. Our series consisted of 136 consecutive cases of which 82 we

OBJECTIVES: Bladder management in tetraplegic patients traditionally has been intermittent catheterization by a caretaker, placement of indwelling suprapubic or urethral catheters, sphincterotomy and external catheter drainage, or supravesical urinary diversion with an ileal conduit. The aim of this study was to examine the ileovesicostomy as an alternative form of bladder management in such patients.
METHODS: We report our experience with ileovesicostomy as an incontinent cutaneous urinary diversion not requiring ureteral reimplantation. Six tetraplegic patients who had experienced significa

The records of 56 patients operated upon for ampullary tumors were reviewed in order to determine the best way to diagnose and treat these tumors. Thirty six males and 20 females, 30 to 89 years old, were operated upon between October 1970 and October 1985. Eighty six p. 100 had jaundice and 21 p. 100, anemia. In recent years, ultrasonography, duodenoscopy and ERCP appeared to be the most useful tests for diagnosis. Mean size of the tumors was 2.3 cm (0.5 to 5 cm). Five tumors were benign (8.9 p. 100) and 51 were malignant. According to the classification of Martin, 5 were grade I: 10, grade I

The aim of this study was to specify the signs and course of patients with a dilated common bile duct without obstruction. We included patients with a dilated common bile duct of more than 12 mm on endoscopic retrograde cholangiography, and we excluded patients with stones, tumor or other visible obstruction. Two hundred and seven patients (8.4 p. 100 of endoscopic retrograde cholangiography) were included. One hundred and nineteen (57.5 p. 100) had undergone cholecystectomy. Sixty-five p. 100 of patients had signs suggesting biliary tract disease, and 78 p. 100 had biological signs of cholest

Pancreaticobiliary maljunction (PBM) is a rare congenital anomaly in which the pancreatic and bile ducts unite outside the duodenal wall, forming an abnormally long common channel that permits reciprocal reflux of pancreatic juice and bile. This leads to a high risk of recurrent pancreatitis, cholangitis, biliary stone formation, and biliary tract malignancy. Type D PBM, particularly when associated with complete pancreas divisum and aberrant bile duct drainage, represents an exceptionally rare and complex variant. We report the case of a 20-year-old female with a long-standing history of recu

Cholecystocolonic fistula (CCF) is a rare type of cholecystoenteric fistula, which are sometimes diagnosed incidentally in patients undergoing cholecystectomy. They are usually a complication of chronic cholecystitis, often secondary to gallstones, and they are difficult to diagnose due to their rarity and vague clinical presentation. If untreated, they can lead to complications such as acute cholangitis, biliary peritonitis, and biliary cirrhosis. This highlights the importance of early recognition of CCF and timely treatment. While the generally accepted management is surgical, there are no

Background and Study Aims A papilla located on the left wall of a periampullary diverticulum creates a mechanically unfavourable axis for biliary cannulation that is not readily corrected from the conventional endoscopist position. We describe the Extreme Anticlockwise Bileduct Cannulation Diverticulum (ABCD) technique, where "Extreme" qualifies the degree of anticlockwise rotation applied-an endoscopist-repositioning manoeuvre designed to address this constraint without additional instrumentation. Patients and Methods Following observations in two index cases, the technique was adopted pre-em

Recurrent embolic events are not unusual in patients with non-valvular atrial fibrillation, and management of anticoagulation in these patients can be particularly challenging. We report a case of a patient who experienced multiple embolic events in the lower extremities and cerebral circulation despite ongoing anticoagulant therapy for atrial fibrillation. The patient was an 88-year-old woman with chronic kidney disease Stage G3b who was taking oral steroids for rheumatoid arthritis. She was also taking a direct Xa inhibitor (edoxaban) for chronic atrial fibrillation, with a CHADS2 score of 2

BACKGROUND: Of endoscopic procedures, in which the skin is not breached, endoscopic retrograde cholangiopancreatography (ERCP) has by far the highest complication rate. The aim of this study was to determine if ERCP-related morbidity is even higher if performed out of working hours.
METHODS: Contemporaneously collected data of 3163 patients undergoing ERCP (213 out of hours and 2929 in hours).
RESULTS: Post-ERCP pancreatitis (PEP) occurred in 7 of 213 (3.3%) patients undergoing out of hours ERCP and 119 of 2929 (4.1%) patients undergoing in hours ERCP (p = 0.759). Average in hours procedure
