The extraction of maxillary lateral incisors for the treatment of a Class II crowded malocclusion: a case report.

BACKGROUND: The extraction of an upper lateral incisor for orthodontic purposes is rare and must be adequately justified. AIM: The present case report describes the management of a skeletal Class II crowded malocclusion that was facilitated by the extraction of upper lateral incisors and lower first premolars. METHODS: A 14-year-old male patient presented with a skeletal Class II crowded malocclusion with associated speech and chewing difficulties. Phase I of treatment involved the extraction of the upper lateral incisors and functional appliance therapy. Phase II included the extraction of

Early nutritional support in non-metastatic stage IV oral cavity cancer patients undergoing adjuvant concurrent chemoradiotherapy: analysis of treatment tolerance and outcome in an area endemic for betel quid chewing.

PURPOSE: Non-metastatic stage IV oral cavity cancer patients undergoing concurrent chemoradiotherapy (CCRT) are at risk of malnutrition because of postoperative eating problems and CCRT-related complications. A high percentage of betel quid use, which is associated with metabolic disorders, is found in oral cavity cancer patients in Taiwan. The aim of this study is to evaluate the effect of an early and intensive nutritional support program, comprising individualized counseling, nasogastric tube feeding, and mandatory hospitalization, throughout the CCRT period for such cases in an area where

Nerve growth factor-evoked masseter muscle sensitization and perturbation of jaw motor function in healthy women.

AIM: To replicate and extend previous findings of nerve growth factor (NGF)-induced mechanical sensitization in healthy young men to women and test for associations between mechanical sensitization and oral motor function. Combined these data would indicate if injection of NGF into the masseter muscle is a valid model of muscle pain related to temporomandibular disorders (TMD). METHODS: A double-blind, placebo-controlled study was conducted on 14 healthy women. Each subject received an injection of NGF (5 microg in 0.2 mL) into 1 masseter muscle and buffered isotonic saline (control, 0.2 mL)

Post-traumatic TMJ internal derangement: impact on facial growth (findings in a pediatric age group).

Many clinical studies have shown how jaw injuries sustained during impact trauma to the face or mandible are the single most important cause of TMJ subsequent internal derangement. Proper function of the masticatory system is certainly the most influential variable in the TMJ remodelling; once a TMJ is internally deranged, adaptative or degenerative osteocartilagineous processes take place in the mandible, temporal bones and muscles. To evaluate relationships between consequences of post-traumatic TMJ internal derangement and disturbed facial skeleton growth in children, 25 patients (16 boys 9

Central administration of the neurotensin receptor antagonist SR48692 attenuates vacuous chewing movements in a rodent model of tardive dyskinesia.

Tardive dyskinesia is a movement disorder that develops in 20-30% of patients treated with chronic neuroleptics. Whilst the pathogenesis of tardive dyskinesia remains unclear, altered expression of neuropeptides in the basal ganglia has been implicated in its emergence. The peptide neurotensin is expressed in both dopamine D1 receptor-bearing neurons of the direct striatonigral pathway and dopamine D2 receptor-bearing neurons of the indirect striatopallidal pathway. Increased levels of striatal neurotensin messenger RNA (mRNA) are reported following chronic neuroleptic therapy. Chronic treatme

TMJ fractures in children: importance of functional activation of muscles in preventing mandibular asymmetries and facial maldevelopment.

Normal development of the mandible as well as some portions of the upper jaw and face are related to good function of the masticatory apparatus: the integrity and interaction of bony and soft-tissue structures may be highly disturbed by injury of the TM joints and result in facial and occlusal disharmonies. When the neuromuscular system is in harmony, the mandibular muscles collectively exert their effect on both position and movement of the jaw and the loading of forces on the TM joints is optimal and balanced. Unfortunately, TMJ fractures may alter completely this balance with loss of the su

[A trans-parotid approach to the osteosynthesis of fractures of the mandibular condyle. A study apropos of 30 cases].

The authors report a homogeneous series of 30 cases with fractures of the mandibular condyles treated by internal fixation with a mini-plaque via the transparotidial route. The patient were treated between 1987 and 1992, with a minimum follow-up of one year. The transparotidial route has the reputation of being dangerous because of the proximity of the facial nerve and the poor vascular supply to the condyle. It is described in detail. Indications are exclusively extra-articular fractures. Complications were observed although no definitive facial nerve paralysis was seen. The authors conclude

Minireview. Stress induced eating.

The relationship of oral behaviors to stress has long been recognized both in humans and in wild animals. In the last decade numerous advances have been made in our understanding of stress-induced feeding predominately because of the development of the simple tail-pinch model of stress induced feeding in rats. Present evidence strongly implicates monoamines and the endogenous opioid peptides as well as other neuropeptides as playing a role in the central regulation of stress-induced eating.

Oral dysfunctions and cognitive impairment/dementia.

With global increases in the aging population, the number of patients with dementia is greatly increasing, which has become a big social problem. Many studies have shown strong associations between oral disorders and systemic disorders, such as diabetes, arthritis, sepsis, aspiration pneumonia, arteriosclerosis, bacterial endocarditis, and other cardiovascular diseases. Similarly, numerous cross-sectional studies showed that patients with dementia usually have poor oral conditions and tooth loss. These have long been considered as a result of difficulty with oral care due to impaired cognitive

Effects on jaw function shortly after whiplash trauma.

Normal jaw function involves muscles and joints of both jaw and neck. A whiplash trauma may disturb the integrated jaw-neck sensory-motor function and thereby impair chewing ability; however, it is not known if such impairment is present shortly after a neck trauma or develops over time. The aim was to evaluate jaw function after a recent whiplash trauma. Eighty cases (47 women) were examined within 1 month after a whiplash trauma and compared to 80 controls (47 women) without neck trauma. Participants completed the Jaw disability checklist (JDC) and Neck Disability Index (NDI) questionnaires

[Interdisciplinary orthodontic surgical treatment of children with cleft lip and palate from 9 to 20 years of age].

Cleft lip and palate is a common congenital malformation with a prevalence of 1:600 newborns. Children with orofacial clefts are treated by an interdisciplinary team of specialists while parents and child play a key role in their own care process. The orthodontic and facial orthopedic treatment of a child with a cleft takes many years. Children often get bored of the long treatment and this can cause problems with compliance and oral hygiene. Therefore it is advisable to distinguish 5 well-defined stages in the orthodontic treatment and to attempt to have some 'orthodontics free' time in betwe

Discectomy without replacement for the treatment of painful reciprocal clicking or catching and chronic closed lock of the temporomandibular joint: a clinical follow-up audit.

Our main aim was to find out if the outcome after temporomandibular joint (TMJ) discectomy differed between patients diagnosed with chronic closed lock and those with reciprocal clicking. A further aim was to identify factors that could have an adverse influence on the outcome of discectomy. The study was designed as a prospective, clinical, follow-up audit. A total of 51 patients were included, 24 diagnosed with closed lock and 27 with reciprocal clicking. All patients had a TMJ discectomy and were followed up for 6 months. The treatment was regarded as successful when the maximal interincisa

Genetics of adverse reactions to haloperidol in a mouse diallel: a drug-placebo experiment and Bayesian causal analysis.

Haloperidol is an efficacious antipsychotic drug that has serious, unpredictable motor side effects that limit its utility and cause noncompliance in many patients. Using a drug-placebo diallel of the eight founder strains of the Collaborative Cross and their F1 hybrids, we characterized aggregate effects of genetics, sex, parent of origin, and their combinations on haloperidol response. Treating matched pairs of both sexes with drug or placebo, we measured changes in the following: open field activity, inclined screen rigidity, orofacial movements, prepulse inhibition of the acoustic startle
