Use of abciximab during infrainguinal peripheral vascular interventions: initial experience.

PURPOSE: To investigate the use of abciximab as an adjunct during infrainguinal angioplasty. MATERIALS AND METHODS: Abciximab was used in conjunction with recanalization techniques and angioplasty to treat stenoses or occlusions in 16 patients: 10 patients had high-grade stenoses or occlusions longer than 5 cm in the superficial femoral artery and popliteal artery, and six patients had stenoses or occlusions below the popliteal artery. All patients received a bolus of heparin after arterial access was achieved. Abciximab was administered by means of a weight-base nomogram that consisted of an

Abciximab and percutaneous coronary intervention: new indications. Medium-term benefit.

(1) Abciximab is now indicated for the prevention of ischaemic complications in patients undergoing percutaneous coronary intervention, and for the prevention of myocardial infarction in patients with unstable angina undergoing percutaneous coronary intervention. (2) The clinical file on abciximab now includes three placebo-controlled trials. (3) The EPIC trial involved patients at high risk of thrombosis, while the EPILOG trial included patients regardless of their risk of thrombosis. In neither trial did abciximab reduce overall mortality. In high-risk patients abciximab reduced the risk of

Abciximab (Reopro): a clinically effective glycoprotein IIb/IIIa receptor blocker.

Acute coronary syndromes are responsible for the deaths of tens of thousands of patients every year. Rupture of coronary atheromatous plaques with resultant luminal thrombosis is the cause in most cases. Although great steps forward have been taken in the management of acute myocardial infarction (MI) and unstable angina (UA), new therapeutic strategies are required to reduce further the incidence and risk of these events. At present, aspirin, nitrates and heparin are the conventional treatments for unstable angina. Aspirin, in combination with a thrombolytic agent or with percutaneous translu

Prophylactic abciximab in elective coronary stenting: results of a randomized trial.

BACKGROUND: The use of abciximab (c7E3 Fab; ReoPro , Eli Lilly & Company, Indianapolis, Indiana) during percutaneous coronary intervention (PCI) decreases the incidence of early (30-day) and late (6-month to 1 year) adverse cardiac ischemic events. In a high-risk population, abciximab also reduced the need for target lesion revascularization. PCI of lesions with complex morphology, particularly long lesions, is associated with more complicated outcomes. The use of multiple and/or long intracoronary stents to cover long coronary lesions may lower the incidence of acute or subacute occlusion, bu

Abciximab therapy in percutaneous intervention: economic issues in the United States.

Whether abciximab therapy should be the standard of care during percutaneous intervention in the United States depends on its efficacy, safety, and economics. In view of the EPIC, CAPTURE, and EPILOG data, few question the superior efficacy and relative safety of abciximab compared with conventional high-dose heparin therapy during percutaneous intervention. Economic considerations have been the major issue limiting its use. Review of the economic data demonstrates that the incremental direct medical care cost of abciximab therapy is $290 to $600 per patient treated in the EPIC and EPILOG popu

Delayed thrombocytopenia following administration of abciximab: Pharmacovigilance survey and literature review.

BACKGROUND: Abciximab (ABX) is used for acute coronary syndrome and unstable angina. Thrombocytopenia is a frequent adverse effect described as occurring in the first 24hours. The aim of this study was to evaluate, in a context of pharmacovigilance survey, the occurrence of delayed thrombocytopenia following ABX infusion in pharmacovigilance database reports and in the literature. METHODS: Individual case safety reports (ICSRs) of delayed thrombocytopenia-between 3 and 30 days - with ABX presented as a single suspect were selected in VigiBase®, the WHO global database of ICSRs. The French cas

[Does abciximab improve the prognosis of diabetics after percutaneous coronary intervention?].

INTRODUCTION AND OBJECTIVES: It is known that the outcome of percutaneous coronary intervention is worse in diabetics than in non-diabetics. The aim of our study was to determine whether abciximab therapy could improve clinical outcome in an unselected diabetic population that underwent percutaneous coronary interventions. MATERIAL AND METHODS: We analyzed retrospectively 198 diabetic patients who underwent PTCA from January 1997 to January 2000. Seventy-three patients (36.7%) were treated with abciximab and the remaining 125 patients (63.3%) did not receive abciximab. The mean follow-up was

Intracarotid abciximab injection to abort impending ischemic stroke during carotid angioplasty.

BACKGROUND: Abciximab, a platelet glycoprotein IIb/IIIa receptor antagonist, prevents ischemic complications during percutaneous transluminal coronary angioplasty and was recently shown to open occluded vessels in patients with acute myocardial infarction when used alone or in combination with other thrombolytic agents. In an animal model of arterial thrombosis, abciximab was found to be safe and effective for the prevention of carotid artery thrombosis. However, the safety and efficacy of abciximab in the treatment of acute ischemic cerebrovascular events is unknown at present. CASE DESCRIPT

Acute profound abciximab induced thrombocytopenia: a correct management of a methodological error.

Thrombocytopenia is a rare complication of glycoprotein IIb/IIIa treatment. We report a case of an acute profound abciximab induced thrombocytopenia and its successful management. The patient, presenting with unstable angina, underwent percutaneous coronary intervention with implantation of three drug eluting stents without receiving a clopidogrel loading dose according to guidelines. The rapid drop in the platelet count after abciximab elastomeric pump infusion was treated with drug discontinuation and platelet transfusion. The high risk of stent thrombosis was avoided by a timely readministr

Abciximab administration and clinical outcomes after percutaneous intervention for in-stent restenosis.

Abciximab therapy improves clinical outcomes after percutaneous interventions for de novo coronary artery disease. We sought to determine whether clinical outcomes after percutaneous intervention for in-stent restenosis are affected by abciximab administration. Between January 1996 and July 1999, 322 consecutive patients underwent percutaneous intervention for in-stent restenosis; 157 patients received abciximab and 165 patients were treated without abciximab based on operator discretion. Baseline clinical and angiographic variables and type of percutaneous intervention were recorded. Follow-u

[New therapy with monoclonal antibodies--abciximab, chimeric anti-GP IIb/IIIa antibody].

Platelet aggregation is now believed to be mediated by fibrinogen binding to activated GP IIb/IIIa. Antigen binding site of mouse anti-human GP IIb/IIIa monoclonal antibody 7E3, which can inhibit fibrinogen binding to activated human platelet GP IIb/IIIa, was combined with constant region of human IgG to reduce immunogenicity. Monovalent Fab of this mouse-human chimeric antibody named abciximab effectively reduced the onset of cardiovascular accidents when administered in combination with heparin and aspirin in patients undergoing coronary interventions. Although several anti-GP IIb/IIIa other

Free wall rupture following rescue angioplasty and intravenous abciximab for failed thrombolysis.

Early thrombolytic therapy reduces the risk of cardiac rupture but delayed thrombolysis may increase this risk, despite improving overall survival. The mechanism appears to be related to both unsuccessful early reperfusion and haemorrhagic transformation following delayed reperfusion. The effect of antiplatelet therapy with glycoprotein IIb-IIIa receptor blockers (abciximab) on cardiac rupture is unknown. It is possible that they may contribute to cardiac rupture by promoting haemorrhagic transformation of the infarcted area. In this report we describe a 57 year old man who underwent emergency

Abciximab as a bridging strategy to overcome morphine-prasugrel interaction in STEMI patients.

OBJECTIVE: The present study investigated whether the glycoprotein (GP)IIb/IIIa receptor blocker abciximab might be a successful bridging strategy to achieve adequate levels of platelet inhibition rapidly in cases where prasugrel is used in morphine-pretreated ST-elevation myocardial infarction (STEMI) patients. METHODS: In a prospective observational cohort study, 32 patients presenting with STEMI were given prasugrel at a loading dose of 60 mg. Patients were stratified into four groups, according to morphine and/or abciximab use. Adenosine diphosphate (ADP)-induced platelet aggregation was

Effect of fibrinogen concentration and platelet count on the inhibitory effect of abciximab and tirofiban.

Glycoprotein (Gp) IIb/IIIa blockers are powerful antithrombotic agents, but display a wide variability of their effect using weight adjusted dosing. We investigated whether some hemostatic variables affected in vitro platelet inhibition exerted by the antibody abciximab and the peptidomimetic tirofiban. High fibrinogen concentrations reduced platelet inhibition by tirofiban, assessed in whole blood (r = -0.85, n = 10, p <0.01) and in platelet rich plasma (r = -0.89, n = 10, p <0.01) through PFA 100 and Born's aggregometry assay, respectively. Both drugs were unaffected by von Willebrand factor

Abciximab as an adjunct to high-risk carotid or vertebrobasilar angioplasty: preliminary experience.

OBJECTIVE: Abciximab, a platelet glycoprotein IIb/IIIa receptor inhibitor, has been shown to reduce the risk of ischemic events associated with coronary intervention. However, its role in neurointerventional procedures needs to be defined. We prospectively evaluated our initial experience with the use of abciximab in a series of high-risk patients undergoing carotid, basilar, or vertebral artery angioplasty. METHODS: Patients were given an intravenous abciximab bolus (0.25 mg/kg), followed by infusion (10 microg/min) for a period ranging from 12 to 24 hours, as an adjunct to angioplasty in 20
