The practice of radiobiology in the radiologic sciences.

To effectively practice radiation protection, radiographers must understand the potential biologic effects of radiation. This article reviews the importance of a knowledge of radiobiology to the work of radiologic technologists. The author describes four specific biologic effects from radiation exposure--leukemia, thyroid cancer, breast cancer and fetal effects--and presents clinical examples that relate to current radiation protection practices and concerns.

1992 assessment of radiation risk from dental radiography.

Recent studies suggest that the lifetime cancer risks from exposure to low levels of ionizing radiation may be greater than previously estimated. This review first summarizes the findings of these studies as they pertain to dental radiology, then uses their concepts in combination with dosimetry from the dental literature to estimate the radiation risk from dental radiology. Estimation of risk from groups of exposed individuals requires use of mathematical models that fit the epidemiological data. The ICRP estimates that a single brief whole-body exposure of 1 Gy to 10,000 people results in ab

Medical imaging radiation safety for the female patient: rationale and implementation.

For the modern practitioner of women's imaging, achieving a balance between the positive diagnostic benefits available from current medical imaging on the one hand, and the potentially deleterious effects of ionizing radiation exposure on the other, has become a central issue. Increased public and professional awareness of the side effects of radiation now require a comprehensive understanding of the facts involved, the various risks to which patients are exposed, and the measures that can be implemented to minimize these risks. The major challenges posed by pregnancy, radiosensitive breast ti

Clinical, genetic and regulatory consequences of exposure to mutagens.

Not a single agent is known to cause an increase of genetic disorders in humans. Even studies on large numbers of children born to parents after exposure to ionizing radiation or DNA alkylating agents failed to detect significant genetic consequences. This is in contrast to effects observed in human somatic cells or germ cells, and to various investigations with laboratory animals. A definite explanation of these discrepancies does not exist. Nevertheless, on the basis of the currently available human data it cannot be justified to advise a previously exposed person against having children, if

Real and perceived risks of medical radiation exposure.

There is almost no subject that elicits greater concern and less objective understanding than that of exposure of patients to radiation accompanying diagnostic radiologic procedures. One or more of the health effects (cancer, developmental abnormalties and genetic changes) attributed to low-level exposure to radiation are frequently misinterpreted as likely consequences rather than statistical risks following radiation exposure. Even less well understood is the small magnitude of the risks accompanying radiation exposure compared with those related to activities encountered during the daily ro

Radiation risks for patients having X rays.

In addition to radiation from naturally occurring radioactive materials and cosmic rays, individuals in developed countries receive radiation doses to bone marrow and gonads from the medical diagnostic use of X rays. A brief discussion of radiation epidemiology shows that deleterious effects are low even when doses are high. The concept of "acceptable risk" is introduced to help evaluate the small, but still existent, risks of radiation dose. Examples of bone marrow and gonadal doses for representative X-ray examinations are presented along with the current best estimates, per unit of X-ray do

Utilization of developmental basic science principles in the evaluation of reproductive risks from pre- and postconception environmental radiation exposures.

The subject of the reproductive toxicity of various forms of radiation can be anxiety provoking to the public on two accounts, since reproductive failure engenders an unusual level of guilt and anger in the affected families, and radiation effects are misunderstood and feared by the public. Reproductive problems include an array of genetic and acquired diseases affecting parents and their offspring. Many of these problems are associated with the risk of being induced by preconception and/or postconception exposures to environmental agents. For the various forms of radiation, namely, ionizing r

How significant are the risks from occupational exposure to ionizing radiation?

Radiologists and other physicians practicing in specialties where their principal operational procedures involve the use of ionizing radiation have health risks associated with their occupational exposures. Based on two years of film badge records from this institution, it was estimated that the general radiologists received an annual occupational dose equivalent of 350 mrems. The corresponding doses for therapeutic radiologists and those practicing special procedures and cardiac catheterization were 110 mrems and 550 mrems. The estimated excess cancer deaths for general radiologists would be

Complications of treatment encountered in lymphoma-leukemia long-term survivors.

Potential posttreatment complications for patients with the lymphomas and leukemia include: 1) impairment of growth and development in children, 2) CNS disturbances encompassing psychologic, intellectual and neurologic expressions, 3) gonadal effects--endocrine, reproductive, teratogenic and genetic, 4) dysfunction of other organs and structures, such as the thyroid, lung and heart and 5) oncogenesis. Many of these adversities are occasioned by radiation therapy, but chemotherapy also can be responsible for some long-term deleterious consequences. The results of combined chemo- and radiaton th

Industrial risk perceptions.

The risks of occupational exposure to radiation need fuller and more explicit characterization. They also need a more developed quantitative comparison with more familiar occupational hazards. To achieve this, some criterion is needed for establishing the amount of detriment one should attribute to different harmful effects, e.g., from accidents at work which cause death, temporary or permanent disability; from fatal and nonfatal cancers; from developmental abnormalities and any likely nonstochastic effects; and from a range of genetic defects. No such criterion for comparing incommensurable k

A systematic review of human evidence for the intergenerational effects of exposure to ionizing radiation.

PURPOSE: To provide a synthesis of the published evidence pertaining to the intergenerational health effects of parental preconceptional exposure to ionizing radiation in humans. METHODS: The study populations are the descendants of those who were exposed to ionizing radiation prior to conception. A Boolean search identified publications for review in accordance with Office of Health Assessment and Translation guidelines. Initially, a risk of bias assessment was conducted for each published study and relevant data extracted. Information was organized into adverse health outcome groups and exp

Imaging in pregnant patients: examination appropriateness.

A recurring source of contention between clinicians and radiologists continues to be examination appropriateness when imaging pregnant patients. With the multitude of references on potential radiation risks to the fetus, radiologists tend to be cautious and hesitant about exposing the fetus to radiation. This tendency is often interpreted by referring physicians as intrusion into and delay in the care of their patients. The risk burden of radiation exposure to the fetus has to be carefully weighed against the benefits of obtaining a critical diagnosis quickly and using a single tailored imagin

Epidemiological and experimental evidence for radiation-induced health effects in the progeny after exposure in utero.

PURPOSE: It has been known for many decades that radiation exposure of the developing embryo or fetus may cause two fundamentally different types of severe health effects: on the one hand, radiation may interfere with the normal intrauterine development, on the other hand, radiation may induce leukemia and cancer which become manifest in childhood. A large amount of epidemiological and experimental data has recently been presented which might be used to improve our understanding of underlying mechanisms and setting radiation protection standards. Yet, ecological studies in the populations expo

Organ-specific dosimetry in spinal radiography: an analysis of genetic and somatic effects.

Radiation doses absorbed by the uterus, ovary, testicle and active bone marrow are computed for cervical, thoracic, lumbar, full spine and chest series performed under typical office conditions. Assuming a nonthreshold, linear relationship between dose and radiogenic effect, the computed tissue-specific doses are used to estimate the probability that each X-ray series might enhance the statistical probability of occurrence of an adult leukemia fatality of the irradiated patient; a childhood leukemia, mental retardation or cancer fatality as a result of fetal irradiation; or a variety of sex ce

Combined effects of ultrasound and ionizing radiation on lymphocyte chromosomes.

The effects of ionizing radiation and ultrasound upon the induction of chromosomal-type aberrations and sister chromatid exchanges were investigated. No statistically significant increase in the frequencies of dicentric and ring chromosomes or sister chromatid exchanges was discovered after ultrasound exposure alone at the diagnostic level. Nor could elevated frequencies of these phenomena be found following exposure to ultrasound before or after ionizing radiation, compared with those resulting from an equivalent dose of ionizing radiation alone. However, simultaneous exposure to ultrasound a
