Treatment failure in patients with obesity with venous thromboembolism receiving truncated vs. recommended lead-in times with apixaban at an academic medical center.

Optimal lead-in duration of apixaban following a period of therapeutic parenteral anticoagulation for venous thromboembolism (VTE) has become controversial, and truncated lead-in periods accounting for parenteral therapy have been proposed in recent studies. Results from previous studies cannot be generalized to many subpopulations of interest, including patients with obesity. This study characterized recurrent VTE in patients with obesity within 6 months of apixaban initiation based on full versus truncated lead-in times following parenteral anticoagulation. This single-center, multi-site, re

Evaluation of Real-World Studies on Management of Relapsed Multiple Myeloma After BCMA-Directed Therapy Failure from U.S. Academic Centers and USMIRC.

B-cell maturation antigen (BCMA)-directed therapies (BDTs) have transformed relapsed/refractory multiple myeloma treatment, but optimal post-failure sequencing remains undefined. We evaluated real-world outcomes from three retrospective, multicenter analyses. Study 1 compared BCMA CAR-T and BCMA T-cell engagers (TCEs) in BDT-exposed patients (n = 95). Study 2 evaluated teclistamab in BDT-exposed versus BDT-naïve patients (n = 164). Study 3 examined talquetamab (GPRC5D-targeting TCE) in heavily pretreated patients (n = 68). CAR-T therapy achieved superior outcomes versus TCE (overall response r

Cultivating Academic Buoyancy in a High-Pressure Confucian Context: Testing Adaptive Beliefs About Failure as a Longitudinal Mediator of Self-Compassion Among Chinese Adolescents.

While self-compassion buffers stress, the longitudinal cognitive mechanisms supporting academic buoyancy under high-pressure educational systems remain unclear. A three-wave design tracked 856 Chinese adolescents (mean age = 15.43 years, SD = 0.62, 51.9% female) from nine Hong Kong secondary schools. A random-intercept cross-lagged panel model partitioned traits from within-person fluctuations. At the between-person level, stable individual differences in self-compassion, adaptive beliefs about failure, and academic buoyancy were positively correlated. At the within-person level, temporary pos

Corneal Transplant Failure Is Associated With a High Percentage of Epithelial Downgrowth: A 24-Year Retrospective Clinical and Histological Study From a Single Academic Institute.

PURPOSE: Epithelial downgrowth (EDG) is a relatively infrequent complication of intraocular surgery or eye trauma. The literature is limited on this condition, and it remains a diagnostic challenge, both histologically and clinically. We have conducted a 24-year single-institution clinical and pathological retrospective review of EDG cases. METHODS: Cases of histologically suspected EDG were identified through the electronic record system at Casey Eye Institute Pathology Laboratory. Keywords entered in the search included "epithelial ingrowth" or "epithelial downgrowth" spanning the dates Jan

Effects of Neighborhood-level Data on Performance and Algorithmic Equity of a Model That Predicts 30-day Heart Failure Readmissions at an Urban Academic Medical Center.

BACKGROUND: Socioeconomic data may improve predictions of clinical events. However, owing to structural racism, algorithms may not perform equitably across racial subgroups. Therefore, we sought to compare the predictive performance overall, and by racial subgroup, of commonly used predictor variables for heart failure readmission with and without the area deprivation index (ADI), a neighborhood-level socioeconomic measure. METHODS AND RESULTS: We conducted a retrospective cohort study of 1316 Philadelphia residents discharged with a primary diagnosis of congestive heart failure from the Univ

Recommendations on pre-hospital & early hospital management of acute heart failure: a consensus paper from the Heart Failure Association of the European Society of Cardiology, the European Society of Emergency Medicine and the Society of Academic Emergency Medicine.

Acute heart failure is a fatal syndrome. Emergency physicians, cardiologists, intensivists, nurses and other health care providers have to cooperate to provide optimal benefit. However, many treatment decisions are opinion-based and few are evidenced-based. This consensus paper provides guidance to practicing physicians and nurses to manage acute heart failure in the pre-hospital and hospital setting. Criteria of hospitalization and of discharge are described. Gaps in knowledge and perspectives in the management of acute heart failure are also detailed. This consensus paper on acute heart fail

Sex- and gender-specific research priorities for the emergency management of heart failure and acute arrhythmia: proceedings from the 2014 Academic Emergency Medicine Consensus Conference Cardiovascular Research Workgroup.

The emergency department (ED) is the point of first contact for patients with acute heart failure and arrhythmias, with 1 million annual ED visits in the United States. Although the total numbers of men and women living with heart failure are similar, female patients are underrepresented in clinical studies, with current knowledge predominantly based on data from male patients. This has led to an underappreciation of the sex-specific differences in clinical characteristics and pathophysiology-based management of heart failure. Similar disparities have been found in management of acute arrhythm

Implications of failure on students who have repeated a class in a faculty of pharmacy.

INTRODUCTION: Academic failure is a major contributor to attrition in pharmacy education, and this attrition is also associated with other effects that are secondary fallouts of academic failure. This study sought to assess the emotional implications and overall psychological impact of academic failure among students who repeated at least one class in the pharmacy program at Obafemi Awolowo University (OAU), Ile-Ife, Nigeria. METHODS: The study involved 177 students from the 200 level to 500 level at the School of Pharmacy, OAU who had repeated at least one class. A pretested questionnaire wa

Removing the academic framing in student evaluations improves achievement in children with dyslexia: The mediating role of self-judgement of competence.

Self-judgement is known to play a crucial role in academic achievement, and as such, may be expected to have an impact on students with dyslexia. Their self-judgements may reflect the negative stereotype of low competence that targets people with disabilities. Their repeated academic failures may lead to a negative association between "school" and "failure". The aim of the present study was to investigate how such factors contribute to academic failure in students with dyslexia. Participants were 183 French middle school students. We assessed students' self-judgement and manipulated the framin

Parasympathetic Nervous System Reactivity Moderates Associations Between Children's Executive Functioning and Social and Academic Competence.

This study examined whether children with poor executive functioning (EF) evidenced less social and academic impairments, compared to other children, if they demonstrated adaptive parasympathetic nervous system (PNS) regulation during experiences of failure. Participants with and without clinical elevations in ADHD symptoms (N = 61; 9-13 years; 48% male; 85% Caucasian) were administered a battery of EF tests and completed manipulated social and cognitive failure tasks. While participants completed failure tasks, respiratory sinus arrhythmia reactivity (RSA-R) was measured as an indicator of PN

Impact of teaching hospital status on postoperative outcomes following posterior cervical fusion/instrumentation.

BACKGROUND: Despite the high volume of posterior cervical fusions performed annually, the impact of academic versus non-academic hospital status on postoperative complications and readmissions requires clarification. METHODS: A retrospective analysis was conducted to identify patients who underwent posterior cervical instrumentation from 2010 to 2022. Patients were grouped by academic or non-academic treatment setting. Propensity score matching controlled for age, gender, race, and comorbidities. Outcomes assessed included surgical and medical complications at 30 and 90 days, pseudoarthrosis

Patient debt management and student academic progress.

A survey of the patient debt management policies of all U.S.-accredited dental and dental hygiene educational programs was taken to assess institutional patient debt management procedures and their relationship to student academic progress. The policies were evaluated to determine the level of compliance with existing standards and to analyze them in light of their legal implications relative to student rights. The results illustrated a vast breadth of policies for both dental and dental hygiene programs, ranging from no relationship between debt management and student progress, to an unspecif

Symptom-level associations between childhood trauma and academic defeat: A network analysis of 11,031 Chinese adolescents.

BACKGROUND: Academic defeat represents a critical construct associated with mental health outcomes in East Asian societies with high-stakes competition. However, conventional models obscure symptom-level mechanisms linking childhood trauma to this sense of failure. This study investigated the topological architecture connecting childhood trauma to academic defeat and identified specific bridge pathways. METHODS: This cross-sectional study included 11,031 adolescents recruited via multi-stage stratified cluster sampling in Shandong Province, China. The Childhood Trauma Questionnaire-Short Form
