Hypothermia-induced accelerated idioventricular rhythm after cardiac surgery; a case report.

BACKGROUND: Accelerated idioventricular rhythm (AIVR) is a slow ventricular arrhythmia, commonly due to myocardial ischemia in coronary artery disease. It is a transitory rhythm that rarely causes hemodynamic instability or necessitates any specific therapy. Besides, the common predisposing factors for ventricular arrhythmias after open-heart surgery are hemodynamic instability, electrolyte imbalances, hypoxia, hypovolemia, myocardial ischemia and infarction, acute graft closure, reperfusion injury, and administration of inotropes and antiarrhythmic drugs. Here we report a case of AIVR after c

Accelerated Idioventricular Rhythm Following Intraoral Local Anesthetic Injection During General Anesthesia.

Some anesthetic agents or adjunct medications administered during general anesthesia can cause an accelerated idioventricular rhythm (AIVR), which is associated with higher vagal tone and lower sympathetic activity. We encountered AIVR induced by vagal response to injection-related pain following local anesthetic infiltration into the oral mucosa during general anesthesia. A 48-year-old woman underwent extraction of a residual tooth root from the left maxillary sinus under general anesthesia. Routine preoperative electrocardiogram (ECG) was otherwise normal. Eight milliliters of 1% lidocaine (

Is accelerated idioventricular rhythm a good marker for reperfusion after streptokinase?

BACKGROUND: Accelerated idioventricular rhythm (AIVR) is a common arrhythmia observed in patients with ST segment elevation myocardial infarction (MI). It is not clear how much value AIVR has in predicting successful reperfusion, since there have been conflicting data regarding this in the past. Streptokinase (STK) even today is the commonest thrombolytic agent used in the public health care set-up in India.(1) Most data for the use of STK are from the 1990s, which had showed that at best it is effective in only 50% of patients in restoring adequate flow.(2) It is probable that with the curren

Multiform accelerated idioventricular rhythm (AIVR) in a child with acute myocarditis.

Acute myocarditis is complicated by various types of conducting disturbances and heart failure. We describe the case of a school boy with acute myocarditis, who developed various types of conduction disturbances, including multiform accelerated idioventricular rhythm (AIVR), during convalescence from severe pump failure. Multiform AIVR has been reported mostly in adults but, to the best of our knowledge, not in children. The arrhythmia noted in our patient persisted for only 2 days and disappeared with recovery.

Cilostazol-Induced Digoxin Toxicity Presenting as Accelerated Idioventricular Rhythm and Recurrent Syncope.

BACKGROUND: Digoxin has a narrow therapeutic index, and toxicity may occur even with apparently normal serum levels, particularly when drug interactions alter its pharmacokinetics or pharmacodynamics. CASE SUMMARY: A 77-year-old man with atrial fibrillation and heart failure developed accelerated idioventricular rhythm (AIVR) after cilostazol was initiated for peripheral arterial disease. Serum digoxin appeared therapeutic, but the sample was drawn >30 hours postdose, outside the appropriate sampling window. Digoxin was discontinued and AIVR resolved completely. DISCUSSION: Cilostazol potent

Accelerated idioventricular rhythm degenerating into bidirectional ventricular tachycardia following acute myocardial infarction.

Bidirectional ventricular tachycardia (BVT) is a rare ventricular tachyarrhythmia. It is usually regular, demonstrating a beat-to-beat alternation in the QRS frontal axis that varies between -20° to -30° and +110°. The tachycardia rate is typically between 140 and 180 beats/min and the QRS is relatively narrow, with a duration of 120 to 150 ms. The etiology of published BVT cases is most commonly digitalis toxicity and, rarely, herbal aconitine poisoning, hypokalemic periodic paralysis, catecholaminergic polymorphic ventricular tachycardia (CPVT), myocarditis, and Andersen-Tawil syndrome. Autonomic patterns preceding and following accelerated idioventricular rhythm in acute myocardial infarction.

INTRODUCTION: We have investigated the potential relationship between cardiac autonomic activity and accelerated idioventricular rhythm (AIVR) in response to reperfusion in the setting of an acute myocardial infarction (AMI) through spectral analysis of heart rate variability (HRV). METHODS AND RESULTS: We studied 16 patients with AMI who developed spontaneous sustained AIVR after initiation of intravenous thrombolysis. Sympathovagal interactions were evaluated by analysis of the low- (LF) and high-frequency (HF) spectral components of HRV for each 5-min interval over the 30-min periods prece

Accelerated idioventricular rhythm as a manifestation of chronic renocardiac syndrome: A case report.

In this case report, we describe a patient who presented with chronic symptoms and signs of uremia and persistent accelerated idioventricular rhythm (AIVR) on electrocardiogram. Findings from blood tests, echocardiography, renal ultrasound, and renal scan were suggestive of heart failure with reduced ejection fraction and chronic kidney disease, and attendance of daily hemodialysis sessions led to the restoration of sinus rhythm. Typically, AIVR has a favorable prognosis and, if necessary, medical intervention focuses on addressing the underlying responsible causes. Accumulation of uremic toxi

Accelerated Idioventricular Rhythm: A Rare Case of Wide-Complex Dysrhythmia in a Teenager.

BACKGROUND: Accelerated idioventricular rhythm (AIVR) is an uncommon and typically benign dysrhythmia with similarities to more malignant forms of ventricular tachycardia (VT). It is often seen in adults after myocardial infarctions, although it also arises in the newborn period, as well as in children with and without congenital heart disease. CASE REPORT: We describe a presentation of AIVR in an otherwise healthy 13-year-old girl, discovered on arrival to the pediatric emergency department in the setting of post-tonsillectomy bleeding. The case reviews the diagnostic criteria of AIVR, assoc

Transient post-reperfusion left bundle branch block and accelerated idioventricular rhythm with paradoxical QRS narrowing.

Accelerated idioventricular rhythm (AIVR) commonly follows coronary reperfusion and has been called a "reperfusion arrhythmia". Transient left bundle branch block (LBBB) is only rarely seen after interventional reperfusion and is usually considered a procedural complication. We report herein electrocardiograms (ECGs) in a case of acute lateral myocardial infarction which demonstrate both post-perfusion AIVR and a simultaneous transient LBBB with fusion complexes causing paradoxical QRS narrowing.

18-Month-Old with Lethargy and Accelerated Idioventricular Rhythm in Prehospital Setting: A Case Report.

INTRODUCTION: We report a case of accelerated idioventricular rhythm (AIVR) identified by Emergency Medical Services (EMS) monitoring of an infant presenting with lethargy and respiratory distress. Accelerated idioventricular rhythms are rare ventricular rhythms originating from the His-Purkinje system or ventricular myocytes, consisting of >3 monomorphic beats with gradual onset and termination.1 An AIVR is usually well-tolerated and does not require treatment, though sustained arrythmia may induce syncope, and the rhythm has been seen in newborn infants with congenital heart diseases.1 Monit

Intraoperative management of accelerated idioventricular rhythm triggered by tourniquet deflation: a case report and mechanistic insights.

BACKGROUND: Accelerated idioventricular rhythm (AIVR) is a common reperfusion arrhythmia, but reports of AIVR triggered by tourniquet deflation in orthopedic trauma surgery are rare. CASE PRESENTATION: A 59-year-old male with left tibiofibular open fracture underwent emergency open reduction and internal fixation. Recurrent AIVR and severe hemodynamic instability occurred twice immediately after tourniquet deflation, with wide QRS complexes, atrioventricular dissociation, and transient ST-segment depression, which was initially misdiagnosed as acute myocardial ischemia. The arrhythmia resolve

Frequent accelerated idioventricular rhythm in an otherwise healthy child: a case report and review of literature.

BACKGROUND: Accelerated idioventricular rhythm (AIVR) is a wide QRS complex dysrhythmia that, as far as pediatric population is concerned, occurs mostly in children with underlying systemic or heart disease. Its clinical course is thought to be typically benign in otherwise healthy children and treatment to be completely needless. Existing guidelines/recommendations are based entirely on cases that had low daily burden of AIVR, and those referring to treatment itself are very unspecific. Pharmacologic therapy has been mostly unsuccessful and catheter ablation as a way of treatment has been onl

Accelerated idioventricular rhythm in newborns: a worrisome but benign entity with or without congenital heart disease.

Broad QRS rhythms (fast and slow) are worrisome findings in newborns. We present five cases with varied clinical presentations found to have broad QRS tachycardias, consistent with idioventricular rhythms. Each patient had an excellent prognosis because the tachycardias resolved, and eventually the patients were in sinus rhythm. None were symptomatic from their arrhythmia. It is important to establish the diagnosis when it occurs to differentiate this benign phenomenon from dangerous ventricular tachycardia.

Intracardiac echocardiography-guided catheter ablation of highly symptomatic accelerated idioventricular rhythm originating from the right ventricular apical diverticulum.

Ventricular diverticula are saccule-like structures formed by the protrusion of the ventricular myocardium from the endocardial surface towards the free wall. Most diverticula are muscular structures, and patients usually have no obvious clinical symptoms. However, diverticula may contribute to arrhythmogenesis due to localized myocardial structural disturbances. Right ventricular apical diverticulum (RVAD) is very rare, and we report a case of highly symptomatic accelerated idioventricular rhythm (AIVR) originating from the RVAD that underwent intracardiac echocardiography (ICE)-guided cathet
