A procedure for denoising dual-axis swallowing accelerometry signals.

Dual-axis swallowing accelerometry is an emerging tool for the assessment of dysphagia (swallowing difficulties). These signals however can be very noisy as a result of physiological and motion artifacts. In this note, we propose a novel scheme for denoising those signals, i.e. a computationally efficient search for the optimal denoising threshold within a reduced wavelet subspace. To determine a viable subspace, the algorithm relies on the minimum value of the estimated upper bound for the reconstruction error. A numerical analysis of the proposed scheme using synthetic test signals demonstra

Validity of accelerometry in ambulatory children and adolescents with cerebral palsy.

To evaluate the validity of the ActiGraph accelerometer for the measurement of physical activity intensity in children and adolescents with cerebral palsy (CP) using oxygen uptake (VO(2)) as the criterion measure. Thirty children and adolescents with CP (mean age 12.6 ± 2.0 years) wore an ActiGraph 7164 and a Cosmed K4b(2) portable indirect calorimeter during four activities; quiet sitting, comfortable paced walking, brisk paced walking and fast paced walking. VO(2) was converted to METs and activity energy expenditure and classified as sedentary, light or moderate-to-vigorous intensity accord

Advancing Wrist-Worn Accelerometry for Measurement of Infant Motor Behavior.

Wearable sensor technology, particularly accelerometry, is an emerging tool to measure infant motor behavior. We evaluated (1) differences and relationships between accelerometry-derived variables in structured versus unstructured contexts and (2) the convergent validity of accelerometer-derived variables with coded motor behavior during structured motor tasks and with caregiver reports of infant motor behavior. Typically developing infants (n = 176, 6 months adjusted age) were evaluated via bilateral wrist-worn accelerometers during structured, laboratory toy play tasks and at home for 2 days

Mechanical loading prediction through accelerometry data during walking and running.

Currently, there is no way to assess mechanical loading variables such as peak ground reaction forces (pGRF) and peak loading rate (pLR) in clinical settings. The purpose of this study was to develop accelerometry-based equations to predict both pGRF and pLR during walking and running. One hundred and thirty one subjects (79 females; 76.9 ± 19.6 kg) walked and ran at different speeds (2-14 km·h-1) on a force plate-instrumented treadmill while wearing accelerometers at their ankle, lower back and hip. Regression equations were developed to predict pGRF and pLR from accelerometry data. Leave-one

Classification of occupational activity categories using accelerometry: NHANES 2003-2004.

BACKGROUND: An individual's occupational activity (OA) may contribute significantly to daily physical activity (PA) and sedentary behavior (SB). However, there is little consensus about which occupational categories involve high OA or low OA, and the majority of categories are unclassifiable with current methods. The purpose of this study was to present population estimates of accelerometer-derived PA and SB variables for adults (n = 1112, 20-60 years) working the 40 occupational categories collected during the 2003-2004 National Health and Nutrition Examination Survey (NHANES). Unsupervised clustering of free-living human activities using ambulatory accelerometry.

This paper investigates unsupervised pattern recognition approaches to segment raw accelerometer signals into a sequence of events, for activity monitoring in a free-living environment. Ambulatory devices, such as accelerometers, have made it possible to classify movement, measure long-term trends in activity, and detect unexpected events such as falls. While such technologies are gaining acceptance for use in controlled clinical settings, their use in varying and unrestricted environments is still problematic. This is principally due to the difficulty of obtaining sufficient annotated data to

Continuous observation of Parkinsonian symptoms using symptom diaries & wearable accelerometry.

Treatment adjustments in Parkinson's Disease (PD) are often based on clinical evaluations at single time points which are insufficient to adequately assess real-life motor fluctuations. Patient-written symptom diaries on the other hand are highly subjective and require well-educated and adherent patients to provide reliable results. Wearable accelerometry might provide a reliable, objective, and continuous diagnostic method to assess PD motor symptoms & fluctuations. However, large datasets of simultaneous sensor data and symptom diaries are needed for such method development and validation. W

Assessing Motor Function in Congenital Muscular Dystrophy Patients Using Accelerometry.

BACKGROUND: When tested in a controlled clinic environment, individuals with neuromuscular-related symptoms may complete motor tasks within normal predicted ranges. However, measuring activity at home may better reflect typical motor performance. The accuracy of accelerometry measurements in individuals with congenital muscular dystrophy (CMD) is unknown. We aimed to compare accelerometry and manual step counts and assess free-living physical activity intensity in individuals with CMD using accelerometry. METHODS: Ambulatory pediatric CMD participants (n = 9) performed the 6-minute walk test

Classifying prosthetic use via accelerometry in persons with transtibial amputations.

Knowledge of how persons with amputation use their prostheses and how this use changes over time may facilitate effective rehabilitation practices and enhance understanding of prosthesis functionality. Perpetual monitoring and classification of prosthesis use may also increase the health and quality of life for prosthetic users. Existing monitoring and classification systems are often limited in that they require the subject to manipulate the sensor (e.g., attach, remove, or reset a sensor), record data over relatively short time periods, and/or classify a limited number of activities and body

Accelerometry-defined physical activity and quality of life in hypertrophic cardiomyopathy.

BACKGROUND: Patients with hypertrophic cardiomyopathy (HCM) often reduce their physical activity due to concerns about sudden cardiac death. However, objective data on activity patterns in HCM, particularly in relation to clinical phenotype and quality of life (QoL), remain limited. METHODS: We assessed physical activity using 7-day accelerometry in 203 patients with HCM and 37 genotype-positive, phenotype-negative (G+/P-) individuals. Outcomes included daily step counts, time spent in moderate-to-vigorous physical activity (MVPA) and sedentariness. QoL was measured using the Kansas City Card

Leveraging Accelerometry as a Prognostic Indicator for Increase in Opioid Withdrawal Symptoms.

Treating opioid use disorder (OUD) is a significant healthcare challenge in the United States. Remaining abstinent from opioids is challenging for individuals with OUD due to withdrawal symptoms that include restlessness. However, to our knowledge, studies of acute withdrawal have not quantified restlessness using involuntary movements. We hypothesized that wearable accelerometry placed mid-sternum could be used to detect withdrawal-related restlessness in patients with OUD. To study this, 23 patients with OUD undergoing active withdrawal participated in a protocol involving wearable accelerom

Evaluation of dynamic instability of the dysplastic hip with use of triaxial accelerometry.

BACKGROUND: Dysfunction of the hip secondary to dysplasia is a complex problem that includes excessive stresses on the articular cartilage, dynamic hip instability, and muscular fatigue, eventually leading to degenerative arthritis if left uncorrected. Mechanical stress on the dysplastic hip has been widely described, but dynamic instability requires further evaluation. The purpose of this study was to investigate dynamic instability of the dysplastic hip with use of triaxial accelerometry. METHODS: We evaluated forty-eight hips of twenty-four patients with unilateral hip dysplasia (a center-

Feasibility of Accelerometry in Adults With Intellectual Disabilities in Health Promotion Research.

BACKGROUND AND PURPOSE: Despite its health benefits, physical activity levels remain low in adults with intellectual disabilities. Efforts to promote engagement in activity are limited in part by a lack of appropriate assessment tools for evaluating activity levels in this population. This study aims to assess the feasibility of applying standard accelerometer wear protocols among adults with intellectual disabilities. METHODS: This retrospective study included 28 adults with intellectual disabilities. Demographic and accelerometer wear and activity data were analysed via descriptive statisti

An accelerometry and gyroscopy-based system for detecting swallowing and coughing events.

Measuring spontaneous swallowing frequencies (SSF), coughing frequencies (CF), and the temporal relationships between swallowing and coughing in patients could provide valuable clinical insights into swallowing function, dysphagia, and the risk of pneumonia development. Medical technology with these capabilities has potential applications in hospital settings. In the management of intensive care unit (ICU) patients, monitoring SSF and CF could contribute to predictive models for successful weaning from ventilatory support, extubation, or tracheal decannulation. Furthermore, the early predictio

Physical activity limitation as measured by accelerometry in pulmonary arterial hypertension.

BACKGROUND: The 6-min walk test, commonly used to assess exercise capacity and response to therapy in pulmonary arterial hypertension (PAH), has many well-described limitations. Sedentary time is associated with adverse cardiovascular outcomes and reduced quality of life, and measuring sedentary time and physical activity using accelerometry is another potential way to quantify exercise capacity in PAH. Whether sedentary time is different in patients with PAH vs control subjects is unknown. METHODS: Physical activity was measured in 20 patients with PAH and 30 matched healthy control subjects

Time-frequency analysis of accelerometry data for detection of myoclonic seizures.

Four time-frequency and time-scale methods are studied for their ability of detecting myoclonic seizures from accelerometric data. Methods that are used are: the short-time Fourier transform (STFT), the Wigner distribution (WD), the continuous wavelet transform (CWT) using a Daubechies wavelet, and a newly introduced model-based matched wavelet transform (MOD). Real patient data are analyzed using these four time-frequency and time-scale methods. To obtain quantitative results, all four methods are evaluated in a linear classification setup. Data from 15 patients are used for training and data

Device-embedded accelerometry complements neural signals for tracking parkinsonian motor states.

Adaptive deep brain stimulation (aDBS) relies on physiological biomarkers to infer motor state and guide therapeutic stimulation in Parkinson's disease. However, neural biomarkers may themselves be altered by stimulation, potentially limiting their utility for closed-loop control. We address this limitation by testing whether DBS device-embedded accelerometers can accurately track Parkinsonian motor state across stimulation conditions. We analysed over 1,900 hours of chronic recordings of subthalamic nucleus (STN), sensorimotor cortical and device-embedded accelerometry signals acquired before

Comparability of accelerometry outcomes across popular metrics and widespread sensor positions.

Accelerometry is a state-of-the-art procedure to capture physical activity. However, the large variety of accelerometry metrics and wearing positions hamper the comparability of outcomes. Since this is a well-known challenge, we investigated how outcomes can be translated across four metrics and six sensor positions. Twenty healthy adults conducted 32 structured and semi-structured activities while wearing accelerometers at the hip, chest, thigh, wrist, ankle, and upper arm. The raw data was converted into four common metrics: Movement Acceleration Intensity (MAI), Euclidean Norm Minus One (EN

Detecting Parkinson's Disease from Wrist-Worn Accelerometry in the U.K. Biobank.

Parkinson's disease (PD) is a chronic movement disorder that produces a variety of characteristic movement abnormalities. The ubiquity of wrist-worn accelerometry suggests a possible sensor modality for early detection of PD symptoms and subsequent tracking of PD symptom severity. As an initial proof of concept for this technological approach, we analyzed the U.K. Biobank data set, consisting of one week of wrist-worn accelerometry from a population with a PD primary diagnosis and an age-matched healthy control population. Measures of movement dispersion were extracted from automatically segme
