Is exercise necessary with repetitive nerve stimulation in evaluating patients with suspected myasthenia gravis?

We retrospectively evaluated the effect of exercise on the degree of decrement in ulnar, spinal accessory, and facial repetitive nerve stimulation (RNS) in 179 patients with myasthenia gravis (MG) to assess whether exercise increases the diagnostic yield of identifying significant decrements. The mean worsening of decrement following exercise was 1.9% (ulnar nerve), 1.9% (spinal accessory nerve), and 1.3% (facial nerve). Abnormal (> or =10%) decrement solely following exercise occurred in the ulnar nerve in 7% of patients, accessory nerve in 5%, and facial nerve in 7%. When analyzed according

Clinical signs of accessory nerve palsy.

BACKGROUND: Injury to the spinal accessory nerve causes paralysis of the trapezius muscle, which is a painful and disabling condition. Many injuries are iatrogenic. Diagnosis is often made after a long delay, suggesting that current clinical signs are inadequate. METHODS: Accessory nerve palsy is known to be a cause of winging of the scapula. Observation of six patients with accessory nerve palsy has shown that winging of the scapula is most prominent when the patient actively externally rotates the shoulder against resistance. RESULTS: This is in contrast to the other causes of winging of t

Spinal accessory neuropathy after deep brain stimulation for Parkinson's disease.

We report a man with Parkinson's disease who developed right spinal accessory neuropathy after right subthalamic nucleus deep brain stimulator and infraclavicular pulse generator implantation. He complained of right shoulder pain and weakness in the post-operative period. He was subsequently diagnosed with a right spinal accessory nerve injury, confirmed by neuromuscular electrodiagnostic studies - electromyography (EMG) and nerve conduction (NC) -, possibly caused by a stretch injury to the nerve at the time of creation of the subcutaneous tunnel for placement of the extension lead of the dee

Diagnostic yield of a practical electrodiagnostic protocol discriminating between different congenital myasthenic syndromes.

Congenital myasthenic syndromes (CMS) are a group of heterogeneous diseases of the neuromuscular junction. We report electrodiagnostic testing (EDX) and genetic findings in a series of 120 CMS patients tested with a simple non-invasive EDX workup with surface recording of CMAPs and 3Hz repetitive nerve stimulation of accessory, radial and deep fibular nerves. Five ENMG phenotypes were retrieved based on the presence or not of R-CMAPs and the distribution pattern of decremental CMAP responses which significantly correlated with genetic findings (p <0.00001). R-CMAPs were found in all COLQ-mutat

Accessory Nerve Disruption Following Core-Needle Biopsy: A Review of Iatrogenic Accessory Nerve Injuries and an Illustrative Case.

Iatrogenic spinal accessory neve (SAN) injury is a rare complication. It most commonly occurs after open lymph node biopsies. Needle biopsy is thought to present lower risk to the SAN. However, we describe a case of a 66-year-old man who underwent core needle biopsy for lymphadenopathy and subsequently experienced ipsilateral shoulder weakness and pain. Physical examination revealed shoulder depression, lateral scapular winging, and decreased range of motion. Subsequent studies and magnetic resonance imaging demonstrated complete paralysis and denervation of SAN muscles. The nonfunctional SAN

[The modifed Eden-Lange procedure for paralysis of the trapezius muscle].

Trapezius muscle paralysis results from injury to the spinal accessory nerve. Impairment in the trapezius muscle function may destabilize the muscle resulting in winged scapula. A 25-year-old university student who was active in sports had complaints of shoulder drop and pain on abduction. He had a three-year history of fall resulting in a scapular fracture for which he received conservative treatment. Physical examination showed asymmetry and drop of the right shoulder. Lateral scapular winging was apparent particularly above 90 degrees of abduction. Electromyography revealed isolated paralys

Spinal accessory nerve injury.

Injury to the spinal accessory nerve can lead to dysfunction of the trapezius. The trapezius is a major scapular stabilizer and is composed of three functional components. It contributes to scapulothoracic rhythm by elevating, rotating, and retracting the scapula. The superficial course of the spinal accessory nerve in the posterior cervical triangle makes it susceptible to injury. Iatrogenic injury to the nerve after a surgical procedure is one of the most common causes of trapezius palsy. Dysfunction of the trapezius can be a painful and disabling condition. The shoulder droops as the scapul

Resolution of Iatrogenic Spinal Accessory Neuropathy Following Delayed Surgical Intervention.

We present an interesting case of a 74-year-old female who presented with spinal accessory neuropathy following an ipsilateral right-sided carotid endarterectomy 6 months after the initial injury. Subsequent surgical exploration revealed a surgical clip that had been placed directly across the spinal accessory nerve. Clip removal and neurolysis was subsequently performed, leaving the nerve intact. After surgical intervention, she immediately demonstrated increased bilateral shoulder shrug motion and continues to improve her function with physical therapy. This case demonstrates a fascinating c

[Evaluation of iatrogenic accessory nerve injury in forensic medical practice].

The authors give a survey of the clinical and medical-legal characteristics of the accessory nerve injury. In the past two decades the conception of the successfulness of the surgical treatment of the accessory nerve injury became prevailing. About the medical-legal aspects of the iatrogenic injury of the nerve reported in connection of the reconstructive surgery chiefly also departments of neurosurgery, orthopedics and traumatology. In the case of the authors a 70 year old patient suffered 10 years ago a iatrogenic accessory nerve injury. The mild trapezius palsy recovered spontaneously pract

Atrophy of the Vagus Nerve in Parkinson's Disease Revealed by High-Resolution Ultrasonography.

Background: The vagus nerve has been suggested to represent one major route of disease progression in Parkinson's disease (PD). Here, we examined whether patients with idiopathic PD exhibit an atrophy of the vagus nerve in comparison to age-matched controls. Methods: In this cross-sectional study, performed between July 2017 and January 2018, we measured the caliber (cross-sectional area) of the mid-cervical vagus, accessory and phrenic nerves in 20 patients with PD (disease duration: 10.1 ± 7.4 years) and 61 (including 20 age-matched) controls using high-resolution ultrasonography. Iatrogenic accessory nerve injury.

Accessory nerve injury produces considerable disability. The nerve is most frequently damaged as a complication of radical neck dissection, cervical lymph node biopsy and other surgical procedures. The problem is frequently compounded by a failure to recognise the error immediately after surgery when surgical repair has the greatest chance of success. We present cases which outline the risk of accessory nerve injury, the spectrum of clinical presentations and the problems produced by a failure to recognise the deficit. Regional anatomy, consequences of nerve damage and management options are d

Accessory nerve injury: conservative or surgical treatment?

In order to clarify the functional prognosis of accessory nerve injury after nerve repair and nonsurgical treatment, 27 of our cases with accessory nerve injury were studied. 20 cases were followed up for more than 8 months. In ten cases treated conservatively, the dull feeling and hypaesthesia did not improve. However, pain and dysfunction of the shoulder improved in half of these cases. In ten cases treated surgically, nerve suture was performed in two cases, nerve graft in five cases and neurolysis in three cases. In the surgically treated group, subjective complaints disappeared in all cas

The X-pointer: A forgotten anatomical relationship of spinal accessory nerve and great auricular nerve.

INTRODUCTION: The preservation of the spinal accessory nerve cannot be overlooked in neck dissection. Injury to the nerve results in shoulder dysfunction and other related morbidities. In this article, we describe a unique constant relationship between spinal accessory nerve and great auricular nerve, at the junction of the anterior and posterior triangles of the neck, eponymously labelled the X- pointer. METHODOLOGY: This was an observational study conducted at a tertiary care cancer centre that runs a comprehensive surgical training program. A 100 cases of modified radical neck dissection p

[Management of iatrogenic lesions of the spinal accessory nerve].

Lesions of the spinal accessory nerve are usually iatrogenic, especially after lymph node extirpation on the neck. Between 1994 and 2003, 31 patients were operated on in the Neurosurgical Department of the University of Ulm for iatrogenic lesions of the XIth cranial nerve. Of 31 patients, 22 had undergone a previous lymph node extirpation, 2 had been injured during a selective peripheral denervation for spasmodic torticollis, and the other 7 patients by different causes. The neurosurgical intervention was performed 0-19 months after trauma (mean 7.2 months). All patients showed paresis/atrophy

Accessory nerve lesion after cervicofacial lift: clinical and electrodiagnostic evaluations of two cases.

Two cases of accessory nerve lesion are reported that occurred within 2.5 weeks and 4.5 weeks, respectively, after surgery for cervicofacial lift. The patients were referred for electrodiagnostic examination because of persistent and unexplained unilateral shoulder pain and disability, 6 and 5 months, respectively, after face lift. In both cases clinical examination revealed severe right-shoulder weakness related to trapezius palsy, without trapezius muscle atrophy in the first case and with trapezius atrophy in the second. Electrodiagnosis revealed bilateral accessory nerve lesions in the fir

Unilateral hyperlucent thorax on plain chest radiographs after neck dissection: importance of atrophy of the trapezius muscle.

OBJECTIVE: A hyperlucent thorax on plain chest radiography indicates a decrease in the radiographic density of the thorax, which can be caused by intra- or extrapulmonary diseases. The purpose of this study was to assess the prevalence and mechanisms that may be responsible for unilateral hyperlucency of the thorax after neck dissection and to determine if atrophy of the trapezius due to the transection of the accessory nerve is a cause of hyperlucent thorax. MATERIALS AND METHODS: Differences in the radiographic density between the right and left lung were evaluated and correlated with trans

[Diagnostic complexities and anatomical surprises in a case of thyroid surgery for papillary carcinoma].

We present the case of a male patient who needed surgery for a large undefined submandibular schwannoma and a small contralateral thyroid carcinoma associated with cervical lymph nodes of a dubious nature. During the operative procedure all the pathological conditions were resolved, with some remarkable surprises. A non-functioning parathyroid adenoma was found and removed. A fairly unusual anatomical complication was also detected with regard to the right inferior laryngeal nerve, i.e. an anastomotic branch connecting the main trunk to the vagus nerve.

Imaging-pathologic correlation of diseases in the axilla.

OBJECTIVE: This article reviews the imaging and histopathologic findings of various axillary diseases and suggests management guidelines for radiologists based on imaging findings with clinical correlation. CONCLUSION: Although axillary diseases may reveal nonspecific imaging findings, a knowledge of the characteristic radiologic manifestations of specific diseases according to anatomic origin (nodal, accessory breast, adipocytic, fibrous, nerve, vascular, stromal, and dermal) and postsurgical lesions aids in establishing an appropriate differential diagnosis and determining whether intervent
