Key Collaborative Factors When Medicaid Accountable Care Organizations Work With Primary Care Clinics to Improve Colorectal Cancer Screening: Relationships, Data, and Quality Improvement Infrastructure.

PURPOSE: Accountable Care Organizations (ACOs) are implementing interventions to achieve triple-aim objectives of improved quality and experience of care while maintaining costs. Partnering across organizational boundaries is perceived as critical to ACO success. METHODS: We conducted a comparative case study of 14 Medicaid ACOs in Oregon and their contracted primary care clinics using public performance data, key informant interviews, and consultation field notes. We focused on how ACOs work with clinics to improve colorectal cancer (CRC) screening - one incentivized performance metric. RES

Medicaid Accountable Care Organization Implementation and Behavioral Health Care for Children.

IMPORTANCE: Medicaid covers almost 40% of US children; state Medicaid programs are increasingly implementing financing and delivery system reforms through Accountable Care Organizations (ACOs). Little is known about how Medicaid ACOs are associated with outcomes for children with behavioral health conditions. OBJECTIVE: To examine the association between Medicaid ACO implementation and behavioral health care access, unmet mental health needs, and care experience for Medicaid-insured children with behavioral health conditions. DESIGN, SETTING, AND PARTICIPANTS: This repeated cross-sectional s

Evaluating a Pay-for-Performance Program for Medicaid Children in an Accountable Care Organization.

IMPORTANCE: Pay for performance (P4P) is a mechanism by which purchasers of health care offer greater financial rewards to physicians for improving processes or outcomes of care. To our knowledge, P4P has not been studied within the context of a pediatric accountable care organization (ACO). OBJECTIVE: To determine whether P4P promotes pediatric performance improvement in primary care physicians. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study was conducted from January 1, 2010, to December 31, 2013. A differences-in-differences design was used to test whether P4P improved

Multi-EMR Quality Aggregation to Drive Accountable Care Organization Performance.

BACKGROUND: Accountable care organizations operating across multiple electronic medical record (EMR) systems face fragmented quality reporting that limits transparency and shared accountability. PROJECT RATIONALE: Inconsistent measure definitions, nonstandard reporting formats, and resource-intensive reconciliation across EMR platforms prevented reliable performance comparison and delayed identification of care gaps. PROJECT SUMMARY: A multidisciplinary team developed and implemented a simplified aggregation framework to extract, normalize, attribute, aggregate, validate, and distribute qual

The Effect of An Integrated Accountable Model on Healthcare Utilization During Pregnancy Among Low-Income Women with Comorbid Mental and Physical Conditions: Evidence from Oregon's Coordinated Care Organizations.

INTRODUCTION: The prevalence of chronic conditions continues to increase in the United States, disproportionately affecting low-income women and Medicaid beneficiaries. This study examined whether an accountable care approach implemented in Oregon Medicaid, known as Coordinated Care Organizations (CCOs), which integrates the delivery of physical and mental health services, may improve high-value healthcare utilization during pregnancy among female Oregon Medicaid beneficiaries with chronic mental-physical comorbidities (MPCs). METHODS: We performed a retrospective analysis of Oregon Medicaid

Availability of Treatment for Alcohol and Opioid Use Disorders Among Accountable Care Organization Participants.

OBJECTIVE: This study examined whether an organization's investment in opioid use disorder services complements or detracts from its capacity to provide alcohol use services. METHODS: Using data from the 2022 National Survey of Accountable Care Organizations (N=200 respondents to alcohol-related questions), the authors estimated the percentage of organizations participating in accountable care organization contracts that provided alcohol-related services (brief counseling or medications), either by direct provision or via referral. They tested whether organizations offering buprenorphine for

Changes in Low-Value Services in Year 1 of the Medicare Pioneer Accountable Care Organization Program.

IMPORTANCE: Wasteful practices are widespread in the US health care system. It is unclear if payment models intended to improve health care efficiency, such as the Medicare accountable care organization (ACO) programs, discourage the provision of low-value services. OBJECTIVE: To assess whether the first year of the Medicare Pioneer ACO program was associated with a reduction in use of low-value services. DESIGN, SETTING, AND PARTICIPANTS: In a difference-in-differences analysis, we compared use of low-value services between Medicare fee-for-service beneficiaries attributed to health care pr

Accountable Care Organization Medicare SNF 3-Day Waiver Improves Subacute Rehabilitation Access and Decreases Cost of Care.

The Medicare Skilled Nursing Facility 3-Day Rule Waiver Benefit Enhancement (SNF Waiver) has been in use in Vermont since 2018 through a Centers for Medicare and Medicaid Services (CMS) waiver with the OneCare Vermont Accountable Care Organization (ACO). This article describes the usage of the waiver in 2024-2025 and calculates a cost savings from eliminating the Medicare SNF 3-Day Rule. OneCare conducted a usage and cost savings analysis from the implementation of the SNF Waiver. Hospitalization costs and number of hospital days were estimated. The use of the SNF Waiver on an annual basis nea

Changes in documentation of social risk factor diagnosis codes: comparison of Medicaid Accountable Care Organization model types.

INTRODUCTION: Identifying social risk factors during the perinatal period may improve pregnancy-related outcomes. In 2018, the Massachusetts Medicaid program implemented an accountable care organization (ACO)-which required social risk factor screening-under 2 ACO model types: Model A (health system/Medicaid managed care plan partnership model) and Model B (primary care practice model). METHODS: Using the 2016-2020 Massachusetts All Payer Claims Database and a difference-in-differences (DiD) design, we compared changes in any social risk factor documentation (ie, ICD-10 Z-codes) for Medicaid-

Impact of Early Stages of the COVID-19 Pandemic on Sexually Transmitted Infection Screening Claims Among Adolescent Females in a Pediatric Accountable Care Organization in Ohio, United States.

Background Testing for sexually transmitted infections (STIs) decreased during the early months of the coronavirus disease 2019 (COVID-19) pandemic. Less is known about the extent to which screening of asymptomatic adolescents for STIs was specifically affected. Our aim was to describe the impacts of early stages of the COVID-19 pandemic on asymptomatic STI screening and overall STI testing among adolescent females aged 13 to 19. We hypothesized that screening would decrease more than overall testing. Methods We evaluated claims data from a pediatric accountable care organization responsible f

The Impact of Payment Reform on Medicaid Access and Quality: A National Survey of Physicians.

INTRODUCTION: Addressing health inequity should involve improving access and quality of care for individuals with low income. In the United States, programs that involve accountable care incentives and broader risk adjustment are poised to help address these issues. However, there is limited understanding of perspectives among physicians as key stakeholders in determining access and quality. METHODS: The authors surveyed physicians about how they perceived accountable care incentives and broader risk adjustment (greater inclusion of clinical, social, and behavioral risk factors) would impact

Designing Complex Care Management Programs to Support Patients With Substance Use Disorder: An Essential and Overlooked Opportunity.

BACKGROUND: Individuals with substance use disorders (SUDs) have high rates of medical and mental health comorbidities, health care utilization, and costs. Improving the capacity of care management programs to meet the needs of those with SUDs could benefit Medicaid accountable care organizations, but there are few reports of strategies to do so. PROGRAM APPROACH: The Boston Medical Center Health System Complex Care Management (CCM) program aims to build trusting relationships with the highest-risk patients across five Massachusetts Medicaid accountable care organizations and work with those
