Percutaneous acetabuloplasty for metastatic acetabular lesions.

BACKGROUND: Osteolytic metastases around the acetabulum are frequent in tumour patients, and may cause intense and drug-resistant pain of the hip. These lesions also cause structural weakening of the pelvis, limping, and poor quality of life. Percutaneous acetabuloplasty is a mini-invasive procedure for the management of metastatic lesions due to carcinoma of the acetabulum performed in patients who cannot tolerate major surgery, or in patients towards whom radiotherapy had already proved ineffective. METHODS: We report a retrospective study in 25 such patients (30 acetabuli) who were evaluat

Percutaneous Acetabuloplasty for Metastatic Lesions to the Pelvis.

Metastatic lesions of the acetabulum can be painful and debilitating. First-line treatment is multimodal and consists of disease-specific chemotherapy, osteoclastic inhibitors, analgesics, and radiation therapy. When these therapies fail, surgical intervention usually is indicated and varies from regional defect stabilization to large periacetabular reconstructions that are demanding procedures with high rates of complications. Percutaneous cement augmentation (acetabuloplasty) of lesions in selected patients has been explored as a less invasive method of lesional control. This retrospective r

Clinical experience in threaded-in cup acetabuloplasty.

By utilizing torque and compression to gain purchase of the acetabular cup into the bony margin of the acetabulum, the concept of threaded-in acetabular implant is attractive. However, controversies about long term fixation have recently arisen. From July 1985 to June 1989, 37 cases (42 hips) were treated with Mecron hemispheric self-tapering threaded-in acetabular cup. With a minimum postoperative period of 2 years, 29 cases (32 hips) were followed up clinically and roentgenographically for an average of 39 months. Among these THRs, 15 operations (47%) were performed primarily and 17 operatio

Technique and long-term results of acetabuloplasty.

Acetabuloplasty with lateral osteotomy of the pelvis in combination with intertrochanteric derotation-varus osteotomy was performed in our hospital between 1970 and 1976. Ninety hip joints have been reexamined recently after cessation of growth. Measurements of the acetabula show normal results in more than 80% of the cases. Measurements of the femoral neck and head, however, were normal in many fewer cases. Revalgization to a "head-in-neck position" or "subcapital coxa valga" was found in 40% of the joints, while the position of the acetabuloplasty remained without significant change if the t

Acetabuloplasty for congenital dislocation of the hip in children.

A modified acetabuloplasty, which has been employed at our hospital since 1977, is described for the treatment of congenital dislocation of the hip in children. The procedure has been performed on a total of 74 hips in 61 children (average age 6.7 years), with satisfactory results obtained in 87.8% of hips. The technique, its indications, and pre- and postoperative management are detailed. The procedure possesses the advantages of both the shelf and Pemberton procedures and has proved to be an effective operation for older children with poor acetabular development. Our results show that, when

[Shelf acetabuloplasty in Legg-Perthes-Calve disease].

PURPOSE OF THE STUDY: The purpose of this study was to demonstrate the contribution of shelf acetobuloplasty for the treatment of Legg-Perthes-Calve disease. MATERIAL: Eighteen children with Legg-Perthes-Calve disease were operated on between 1992 and 2001. Mean age at diagnosis was 7 years 2 months, and mean age at surgery was 8 years 5 months (range 5-13 years). Mean motion was limited -25 degrees in abduction and internal rotation preoperatively. The radiological classification was: 2 Catterall II Herring B with signs of risk for the head, 8 IIB, 1 III C, 2 IV B, 3 IV C, and 2 at the seque

No Correlation Between Depth of Acetabuloplasty or Postoperative Lateral Center-Edge Angle on Midterm Outcomes of Hip Arthroscopy With Acetabuloplasty and Labral Repair.

BACKGROUND: The treatment of pincer deformity in hip arthroscopy remains controversial, with some authors advocating that over resection may risk early joint deterioration. The role of acetabular resection depth and postoperative acetabular morphology on postoperative outcomes has yet to be defined. PURPOSE/HYPOTHESIS: This study measures the influence of acetabular resection depth and postoperative lateral center-edge angle (LCEA) on minimum 5-year patient-reported outcomes (PROs), revision rates, and conversion to total hip arthroplasty using a single surgeon's prospective database. We hypo

Does shelf acetabuloplasty influence acetabular growth and remodeling?

BACKGROUND: Shelf acetabuloplasty has the potential to cause iatrogenic acetabular growth arrest, although accelerated acetabular growth has been reported based on plain radiographic evaluations in patients with Legg-Calvé-Perthes disease. Because plain radiographs may be limited in depicting actual acetabular morphology, it is unclear whether there are growth disturbances. QUESTIONS/PURPOSES: We therefore determined (1) whether focal or generalized acetabular growth stimulation or retardation occurred based on CT analysis in combination with plain radiographic assessment; and (2) whether rad

Legg-Calvé-Perthes disease treatment by augmentation acetabuloplasty.

INTRODUCTION: Classical surgical treatments for Legg-Calvé-Perthes (LCP) disease are pelvic or femoral osteotomies, which are not without complications and inconvenience for the patient. An effective, relatively undamaging surgical alternative would improve this disease treatment. HYPOTHESIS: Augmentation acetabuloplasty using shelf acetabuloplasty is a recognized treatment for LCP disease, but its results have never been assessed in view of Herring's lateral pillar classification, the current reference in determining the prognosis of this disease. Assessment of its efficacy based on this syst

[Follow-up results of Dega's acetabuloplasty (author's transl)].

From 1966 to 1974 231 acetabuloplasties were performed in 193 children. Dega's technique was not consequently observed in these operations. The follow-up was made in 166 children with 202 acetabuloplasties. 65 per cent of the operated hips were found within the normal range. The clinical examination revealed excellent and good results in 82 per cent. There was a relatively great number of failures due to technical problems, mainly in the first time. The results obtained allow the conclusion that Dega's acetabuloplasty is a valuable method in the operative treatment of the congenital hip in ear

Acetabuloplasty for neglected dislocation of the hip in older children.

This paper describes the technique and results of an acetabuloplasty in which the false acetabulum is turned down to augment the dysplastic true acetabulum at its most defective part. This operation was performed in 17 hips (16 children), with congenital dislocation and false acetabula. The mean age at operation was 5.1 years (4 to 8). The patients were followed clinically and radiologically for a mean of 6.3 years (5 to 10). A total of 16 hips had excellent results and there was one fair result due to avascular necrosis. The centre-edge angles and the obliquity of the acetabular roof improved

Pemberton's acetabuloplasty for congenital dislocation or subluxation of the hip.

The results obtained in a consecutive series of thirty-seven Pemberton operations for congenital dislocation or subluxation of the hip are reported. Over the period under review, 1967 to 1973, it was the only type of acetabuloplasty employed at Winford. Unless the mandatory concentric reduction could be obtained with ease, preliminary open reduction was favoured, especially in cases of primary care. Femoral rotation osteotomy was added for marked anteversion. The programme was designed to be complete inside fourteen weeks, and was so for eighteen hips. Secondary acetabuloplasty was performed o
