Global strategies to fight carbapenem-resistant Acinetobacter baumannii (CRAB) infections.

Carbapenem-resistant Acinetobacter baumannii (CRAB) has emerged as a formidable healthcare-associated pathogen, particularly in intensive care units, where it causes severe infections associated with prolonged hospitalization, limited treatment options, and high mortality. This review provides an integrated analysis of the global epidemiology of CRAB, the molecular mechanisms driving carbapenem resistance, and the increasing clinical significance of antibiotic hetero-resistance, which contributes to diagnostic failure and poor therapeutic outcomes. We highlight advances in diagnostic technolog

Sulbactam for carbapenem-resistant Acinetobacter baumannii infections: a literature review.

Carbapenem-resistant Acinetobacter baumannii (CRAB) is characterized as a critical priority pathogen with restricted therapeutic options. To date, the most effective antimicrobial treatment against this difficult-to-treat bacterial strain has not been established. Sulbactam is a β-lactamase inhibitor with intrinsic activity against this pathogen, however, as a β-lactam, it can be hydrolysed by β-lactamases produced by A. baumannii. High-dose, extended-infusion treatment with sulbactam can overcome this hydrolysis by β-lactamases and is considered an effective therapeutic strategy against CRAB.

Gene dependence during mammalian Acinetobacter baumannii pneumonia and septicaemia infections.

With a limited number of traditional virulence factors, the success of the nosocomial pathogen Acinetobacter baumannii is largely attributed to its ability to persist and resist. The niches encountered during infection vary significantly from the more commonly studied laboratory setting, and consequently, the genes responsible for in vivo pathogenesis have yet to be fully elucidated. This study utilized the A. baumannii AB5075-UW transposon mutant library with unbiased genome-wide transposon sequencing to identify the genetic basis for survival and fitness during pneumonia and septicaemia infe

Modeling Acinetobacter baumannii wound infections: The critical role of iron.

BACKGROUND: Acinetobacter baumannii has emerged as an increasingly important and successful opportunistic human pathogen due to its ability to withstand harsh environmental conditions, its characteristic virulence factors, and quick adaptability to stress. METHODS: We developed a clinically relevant murine model of A. baumannii traumatic wound infection to determine the effect of local wound environment on A. baumannii virulence. Mice underwent rectus muscle crush injury combined with ischemia created by epigastric vessel ligation, followed by A. baumannii inoculation. Reiterative experiments

[Epidemiology and clinical features of Acinetobacter baumannii infections in humans].

Over the past 30 years, Acinetobacter (A.) baumannii has been increasingly recognized as an important human pathogen. In contrast to the "non-baumannii" Acinetobacter spp. which rarely cause diseases in humans, the members of the A. baumannii-group (A. baumannii, A. pittii and A. nosocomialis) are important nosocomial pathogens. A. baumannii itself has been implicated as a causative agent in various severe infections such as bloodstream infections, pneumonia, or meningitis, mainly in immunocompromised patients in the intensive care unit setting. The combination of intrinsic and increasing acqu

Acinetobacter - the trojan horse of infection control?

BACKGROUND: Five cases of multi-resistant Acinetobacter baumanii (MRA) producing OXA-23 and OXA-51 occurred in a regional burn intensive care unit (BICU). Three were repatriated from other parts of the world (Dubai and Mumbai) and colonized on admission. Despite optimal precautions, two patients acquired MRA. Both had been nursed in the same room. METHODS: Multi-disciplinary outbreak investigation of MRA in a regional BICU. FINDINGS: The mechanism of transfer for the first case is thought to have been contaminated air from theatre activity releasing MRA bacteria into the communal corridor. N

Acinetobacter baumannii Complex Infections: New Treatment Options in the Antibiotic Pipeline.

Acinetobacter baumannii complex (ABC) can result in a panoply of severe syndromes, including pneumonia and septic shock. Options available for treating infections caused by ABC and, more importantly, by carbapenem-resistant ABC (CRAB) are limited because of the increasing prevalence of antimicrobial resistance. Furthermore, many older agents, such as polymyxin and colistin, have limited lung penetration and are associated with significant toxicities. These factors underscore the urgent need for new paradigms to address ABC and CRAB. Two agents, cefiderocol and sulbactam-durlobactam, are now av

The role of sulbactam-durlobactam in treating carbapenem-resistant Acinetobacter infections.

PURPOSE OF REVIEW: Infections caused by multidrug-resistant Acinetobacter baumannii present a significant global health challenge. Available treatment options are limited and frequently constrained by unfavourable safety and pharmacokinetic profiles. Sulbactam-durlobactam is a novel β-lactamase inhibitors combination specifically developed to target A. baumannii , including carbapenem-resistant strains. The purpose of this review is to assess the current evidence supporting the role of sulbactam-durlobactam in the management of A. baumannii infections. RECENT FINDINGS: We summarize the availa

Mortality associated with Acinetobacter baumannii infections experienced by lung transplant recipients.

Lung transplantation (LTX) requires continual systemic immunosuppression, which can result in infections that may compromise recipient survival. A recent outbreak of Acinetobacter baumannii at our institution resulted in infections experienced in both LTX recipients and nontransplant patients. A retrospective review was conducted of patients who had A. baumannii recovered from blood, other normally sterile body fluids, and/or respiratory secretions and who had clinical follow-up extending to 1 year postinfection. A. baumannii was considered "multidrug-resistant" when its growth was not inhibit

Vaginal Acinetobacter infection as a cause of cervical collagenolysis.

BACKGROUND: The vaginal microbiota demonstrates considerable heterogeneity and is shaped by factors such as race, geographic location, and individual variation. Although its central role in maintaining host health is well established, the precise mechanisms through which it contributes to the onset and progression of preterm birth remain incompletely understood and continue to be debated. OBJECTIVE: This study aimed to characterize the vaginal microbiota of pregnant women who experienced preterm birth compared with those with full-term pregnancies. Through both in vivo and in vitro experiment

Infective endocarditis by Acinetobacter species: a systematic review.

A. baumannii - A. calcoaceticus complex infections are increasingly frequent, especially in intensive care units. Such infections are associated with a mortality that can be as high as 62%. On the other hand, infective endocarditis (IE) is an uncommon disease with notable morbidity and mortality. Even though IE is rarely caused by Acinetobacter species, these infections can be particularly problematic due to increasing antimicrobial resistance. The purpose of this study was to systemically review all published cases of IE by Acinetobacter species in the literature. A systematic review of PubMe

Colistin Resistance Mechanism and Management Strategies of Colistin-Resistant Acinetobacter baumannii Infections.

The emergence of antibiotic-resistant Acinetobacter baumannii (A. baumannii) is a pressing threat in clinical settings. Colistin is currently a widely used treatment for multidrug-resistant A. baumannii, serving as the last line of defense. However, reports of colistin-resistant strains of A. baumannii have emerged, underscoring the urgent need to develop alternative medications to combat these serious pathogens. To resist colistin, A. baumannii has developed several mechanisms. These include the loss of outer membrane lipopolysaccharides (LPSs) due to mutation of LPS biosynthetic genes, modif

Navigating Available Treatment Options for Carbapenem-Resistant Acinetobacter baumannii-calcoaceticus Complex Infections.

Carbapenem-resistant Acinetobacter baumannii-calcoaceticus complex (CRAB) is one of the top-priority pathogens for new antibiotic development. Unlike other antibiotic-resistant threats, none of the available therapies have been shown to consistently reduce mortality or improve patient outcomes in clinical trials. Antibiotic combination therapy is routinely used in clinical practice; however, the preferred combination has not been defined. This narrative review focuses on evidence-based solutions for the treatment of invasive CRAB infections. We dissect the promise and perils of traditional age

New molecules and adjuvants in the treatment of infections by Acinetobacter baumannii.

INTRODUCTION: The current problems of the treatment of infections by Acinetobacter baumannii are linked with the increase of multidrug- and extensive-drug resistance and the lack of development of new antimicrobial drugs for Gram-negative bacilli. For these reasons, new alternatives for the treatment and control of severe infections by A. baumannii are necessary. Several studies have reported the effect of adjuvants to restore the efficacy of existing antimicrobial agents. AREAS COVERED: In the present review, the authors describe the main results in the development of adjuvant drugs as well
