Predictors of mortality in Acinetobacter baumannii bacteremia.

This study retrospectively investigated 149 episodes of Acinetobacter baumannii bacteremia which occurred during a 41-month period from September 1997 to January 2001. Bacteremia was nosocomial in 139 (93%) of the episodes and community-acquired in 10 (7%). Thirty three deaths (22.1%) were attributed to these episodes of A. baumannii bacteremia. The mean age of survivors was younger than that of patients who died of bacteremia (60.4 +/- 19.9 vs 67.1 +/- 17.4) but this result was not significant on univariate analysis (p=0.084). Previous intensive care unit stay was longer among survivors than

Advances in antibiotic heteroresistance in Acinetobacter baumannii.

Acinetobacter baumannii is a major nosocomial pathogen, and its increasing multidrug-resistant and pandrug-resistant profiles pose a serious challenge to anti-infective therapy. In recent years, heteroresistance to multiple antibiotics in A. baumannii has attracted growing attention. Heteroresistance refers to the presence of low-frequency resistant subpopulations within an isolate that is classified as susceptible by conventional susceptibility testing. These resistant subpopulations can survive under antibiotic selective pressure and subsequently expand, potentially leading to treatment fail

Use of adjuvants in the treatment of Acinetobacter baumannii.

The current antibiotic crisis to treat infections by Acinetobacter baumannii is linked with the increase of antimicrobial resistance and the lack of development of new antimicrobial drugs. For this reason, new alternatives for the treatment and control of infections by A. baumannii are necessary. Several studies have reported the effect of adjuvants to restore the efficacy of existing antimicrobial agents. Herein, we analyzed the main results on the development of adjuvant drugs, as monotherapy or in combination therapy with existing antimicrobial agents, which have shown promising results in

Acinetobacter baumannii: no more a choosy intruder?

CONTEXT: Acinetobacter baumannii, a non-fermenter, is fast coming up the list of pathogens causing nosocomial infections. Earlier considered to be a harmless colonizer, or at the most, a pathogen causing mostly respiratory tract infections, it has slowly but successfully emerged as a ubiquitous pathogen causing both community as well as health care associated infections. It has acquired multidrug resistance, and seems to be no more selective in mainly attacking any one or two body systems. According to recent reports, a shy selective bacterium has turned into a dangerous pathogen, capable of c

Microbe Profile: Acinetobacter baumannii: persist and resist.

Acinetobacter baumannii is a Gram-negative opportunistic pathogen associated with a wide variety of healthcare-associated infections, including ventilator-associated pneumonia, urinary tract infections, bacteraemia and wound infections. It adopts a 'persist and resist' pathogenic strategy to establish infection in the host. It can form robust biofilms and survive in a desiccated state for months at a time, driving its persistence in the hospital-built environment. It also has a remarkable capacity to rapidly develop antimicrobial resistance, with >90% of contemporary clinical isolates being mu

[Phage therapy for multidrug-resistant Acinetobacter baumannii].

Acinetobacter baumannii is a Gram-negative opportunistic pathogen widely distributed in hospital settings. It can survive for a long time and cause a variety of infections, including pneumonia, septicemia, urinary tract infections, and meningitis. The bacterium demonstrates extensive resistance, particularly to critical antibiotics like carbapenems and polymyxins, posing a serious threat to the recovery of severely ill patients. Carbapenem-resistant A. baumannii has been designated as a pathogen of critical priority on the World Health Organization (WHO) Bacterial Pathogen Priority List, requi

Lipidome of Acinetobacter baumannii antibiotic persister cells.

Persister cells constitute a bacterial subpopulation able to survive to high concentrations of antibiotics. This phenotype is temporary and reversible, and thus could be involved in the recurrence of infections and emergence of antibiotic resistance. To better understand how persister cells survive to such high antibiotic concentration, we examined changes in their lipid composition. We thus compared the lipidome of Acinetobacter baumannii ATCC 19606T persister cells formed under ciprofloxacin treatment with the lipidome of control cells grown without antibiotic. Using matrix assisted laser de

Macrophage activation syndrome following Acinetobacter baumannii sepsis.

Macrophage activation syndrome (MAS) is a systemic disorder with a high mortality, commonly associated with rheumatological conditions, but which can also occur as a complication of several infections. Here we present a case of MAS following Acinetobacter baumannii sepsis. Early institution of therapy with prednisolone, cyclosporine, colistin, and polymyxin resulted in a prompt clinical recovery. There are very few reported cases of Acinetobacter-related MAS that have been successfully treated.

Genome sequence of Acinetobacter baumannii MDR-TJ.

Acinetobacter baumannii is a pathogenic species of bacteria, identified as an aerobic gram-negative bacterium, that is resistant to most antibiotics. In this study, the MDR-TJ strain was isolated at the Second Hospital of Tianjin Medical University, China, and was found to be resistant to penicillin, cephalosporins, aminoglycosides, quinolones, and also imipenem. The genome sequence of Acinetobacter baumannii strain MDR-TJ was determined by using a combination of 454 pyrosequencing and paired-end sequencing performed with the Roche Genome Sequencer FLX system to generate a scaffolded assembly.

Acinetobacter baumannii colonization in ventilated preterm infants.

The role of Acinetobacter baumannii in infections in ventilated preterm infants was evaluated in 15 colonized infants (11 male, 4 female) in a pediatric intensive care unit. These cases were randomly matched by birth weight and gestational age with ventilated non-colonized controls (8 male, 7 female). Case records were reviewed for signs and symptoms of infection. Colonized infants were ventilated significantly longer (p < 0.05) than controls, and had body temperatures of > 37 degrees C for a significantly longer period of time (p < 0.05). No other parameter of infection differed significantly

Outsmarting Nightmare: Drug Resistance in Acinetobacter baumannii.

Acinetobacter baumannii is a critical nosocomial pathogen recognized by the WHO as a top-priority threat due to its extensive drug resistance, particularly in carbapenem-resistant Acinetobacter baumannii (CRAB) strains. According to recent CDC reports, CRAB is one of the most urgent causes of hospital-acquired infections, driving longer hospital stays, higher costs, and mortality. This pathogen employs multiple resistance mechanisms, including efflux pumps, porins like OmpA, making it highly difficult to treat. Resistance is further shaped by β-lactamases, integrons, and mobile genetic element

How to treat severe Acinetobacter baumannii infections.

PURPOSE OF REVIEW: To update the management of severe Acinetobacter baumannii infections (ABI), particularly those caused by multi-resistant isolates. RECENT FINDINGS: The in vitro activity of the various antimicrobial agents potentially helpful in treating ABI is highly variable and has progressively decreased for many of them, limiting current therapeutic options. The combination of more than one drug is still advisable in most circumstances. Ideally, two active first-line drugs should be used. Alternatively, a first-line and a second-line drug and, if this is not possible, two or more seco

Genome organization of epidemic Acinetobacter baumannii strains.

BackgroundAcinetobacter baumannii is an opportunistic pathogen responsible for hospital-acquired infections. A. baumannii epidemics described world-wide were caused by few genotypic clusters of strains. The occurrence of epidemics caused by multi-drug resistant strains assigned to novel genotypes have been reported over the last few years.ResultsIn the present study, we compared whole genome sequences of three A. baumannii strains assigned to genotypes ST2, ST25 and ST78, representative of the most frequent genotypes responsible for epidemics in several Mediterranean hospitals, and four comple
