Neurosyphilis in acquired immunodeficiency syndrome.

In a retrospective study encompassing 42 months (January 1984 through July 1987), 12 patients were identified as having both neurosyphilis and acquired immunodeficiency syndrome. These patients constituted 44% of the entire group meeting rigorous diagnostic criteria for neurosyphilis and 1.5% of all patients hospitalized with acquired immunodeficiency syndrome. The typical patient with acquired immunodeficiency syndrome and neurosyphilis was young (mean age, 37 years) and male (83%). All had serological evidence of syphilis in both blood and cerebrospinal fluid. Syphilitic eye disease was surp…

Acquired immunodeficiency syndrome-associated arthritis.

A subacute, oligoarthritic syndrome developed in four patients with human immunodeficiency virus (HIV) infection. Three had true acquired immunodeficiency syndrome (AIDS) and all had lymphocyte abnormalities. The arthritis was characterized by extreme pain and disability in three patients and moderate pain in one. Knees and ankles were affected. Symptoms developed over a one- to six-week interval; response to treatment was rapid, especially to intra-articular corticosteroids. Despite the clinical severity of the arthritis, synovial fluids were non-inflammatory and biopsy specimens revealed onl…

CT of acquired immunodeficiency syndrome.

Thirty patients with acquired immunodeficiency syndrome were examined by computed tomography. In addition to systemic disease, these patients had a variety of neurologic symptoms and signs. Cerebral toxoplasmosis (six cases) was generally manifested by ring-enhancing lesions with surrounding decreased attenuation. Lymphoma (one case) exhibited a solid enhancing nodule, and progressive multifocal leukoencephalopathy (two cases) showed periventricular decreased attenuation. Atrophy (15 cases) was very common and invariably indicated a poor prognosis; the autopsy examinations of the latter cases …

Acquired immunodeficiency syndrome: neuroradiologic findings.

Central nervous system complications depicted by CT in ten patients with acquired immunodeficiency syndrome are described. Three patients had multifocal intra-axial enhancing lesions representing atypical brain abscesses (two with toxoplasmosis, one with candidiasis). A fourth patient with multifocal "ring" lesions whose biopsy was interpreted as suggestive of toxoplasmosis responded poorly to treatment. Following his death three months later of Pneumocystis carinii pneumonia, autopsy revealed primary intracerebral immunoblastic lymphoma. One patient had Kaposi sarcoma involving the right fron…

Headache and acquired immunodeficiency syndrome.

Because the acquired immunodeficiency syndrome (AIDS) virus is neurotropic, physicians will continue to see a rise in the number of neurologic complications of this syndrome. Much of this increase will be accompanied by headache, not only as a primary symptom of HIV infection or opportunistic disease but also as a result of diagnostic tests and therapeutic efforts. Complete understanding of the ramifications of headache in AIDS will be important in the 1990s as we continue to treat a younger population, usually affected by benign vascular and muscle contraction-type headache.

Hypouricemia in acquired immunodeficiency syndrome.

Clinical evaluations of hypouricemia in patients with the acquired immunodeficiency syndrome (AIDS) have shown that it is a common disorder resulting from defective renal handling of uric acid. We prospectively studied renal urate handling in 23 patients and reviewed the records of 73 consecutive patients with AIDS or AIDS-related complex (ARC), who were seen in our AIDS clinic between March 1985 and April 1988, to determine the incidence, significance, and, when possible, the cause of hypouricemia. Hypouricemia was defined as serum urate less than or equal to 0.18 mmol/L (3 mg/dL). Renal clea…

Neuropathology of acquired immunodeficiency syndrome.

The nervous system is involved in the acquired immunodeficiency syndrome (AIDS) by infections and infestations, by neoplasms, and by several diseases of uncertain pathogenesis. The most common pathological abnormalities are the changes associated with the HIV agent itself, the 'HIV subacute encephalitis'; the most specific change is demyelination with multinucleate giant cells. Cytomegalovirus (CMV) is the most common secondary virus, but progressive multifocal leucoencephalopathy (PML) is more frequently seen at biopsy. Toxoplasmosis is the most common cause of abscess formation, but fungal i…

Neuroepidemiology of acquired immunodeficiency syndrome.

Data from the Centers for Disease Control (CDC) and from the hospitals affiliated with the University of California, San Francisco show a significant incidence of neurological complications in AIDS patients and suggest that patients from different risk groups and geographic regions are at different relative risk for specific neurological complications. CDC national surveillance data show that Haitian-born AIDS patients are 3.7 times more likely to have neurological complications than are patients in other risk groups; neurological illness is also reported more often in intravenous drug abusers

Botryomycosis in acquired immunodeficiency syndrome.

The first case, to our knowledge, of an integumentary form of botryomycosis is reported in a homosexual man with acquired immunodeficiency syndrome. Anal fistula and ischiorectal and gluteal abscesses developed following severe cryptosporidial diarrhea. Grains composed of gram-positive cocci were identified in the suppurative exudate. The grains had attached to multinucleated macrophages, many of which contained clusters of cocci in their cytoplasm. It is postulated that the cocci were able to survive and probably replicate in the cytoplasm of multinucleated macrophages, and were subsequently …

Acquired immunodeficiency syndrome and the eye.

This incidence of AIDS is rapidly increasing in the whole world. India is emerging as the country with largest number of AIDS patients. The life time cumulative risk of at least one abnormal ocular lesion developing in an HIV positive ranges from 52% to 100% in various studies. The role of ophthalmologist in the management of an HIV infected patient is becoming increasingly important. This review article describes the common ocular lesions in AIDS patients, particularly seen in Indian patients and available treatment modalities.

Pyomyositis in the acquired immunodeficiency syndrome.

Pyomyositis, a purulent infection of skeletal muscle, is usually caused by Staphylococcus aureus. Many cases of pyomyositis in human immunodeficiency virus (HIV) seronegative patients have been reported in North America and have been reviewed extensively. Moreover, pyomyositis has been reported in association with HIV infection in patients with or without the acquired immunodeficiency syndrome (AIDS). We describe two patients with pyomyositis and HIV and review the available English language literature. Leukocytosis and bacteremia tend to occur less frequently in those with HIV infection and p…

The Thyroid in Acquired Immunodeficiency Syndrome.

Forty-seven thyroids obtained at autopsy from patients with acquired immunodeficiency syndrome (AIDS) with no clinical manifestations of thyroid disease were analyzed systematically in order to determine the frequency and the major pathological characteristics of thyroid involvement in these individuals. The glands were obtained from 38 men and 9 women aged (on average) 33.6 yr. The specimens were weighed, measured, and evaluated after fixation in formalin Histological examination was performed on at least 10 macroscopically normal and altered areas. The anatomopathological lesions detected in…

Coccidioidomycosis in the acquired immunodeficiency syndrome.

Of 27 patients with the acquired immunodeficiency syndrome (AIDS) in Tucson, Arizona, 7 had concurrent coccidioidomycosis. Early manifestations of infection in 6 patients included diffuse nodular pulmonary infiltrates and Coccidioides immitis in many extrathoracic sites. By comparison, a retrospective review of the cases of 300 patients hospitalized with coccidioidal infection identified only 13 patients without AIDS who had the same extent of infection, and only 3 of these patients had no immunosuppressing conditions. Antibodies for coccidioidal antigens at serum dilutions as high as 1:2048 w…
