Management of acromioclavicular joint injuries.

Acromioclavicular joint injuries are among the most common shoulder girdle injuries in athletes and most commonly result from a direct force to the acromion with the arm in an adducted position. Acromioclavicular joint injuries often present with associated injuries to the glenohumeral joint, including an increased incidence of superior labrum anterior posterior (SLAP) tears that may warrant further evaluation and treatment. Anteroposterior stability of the acromioclavicular joint is conferred by the capsule and acromioclavicular ligaments, of which the posterior and superior ligaments are the

Acromioclavicular Joint Injury and Repair.

The acromioclavicular (AC) joint is commonly injured in athletes participating in contact and overhead throwing sports. Injuries range from simple sprains to complete ligamentous disruption, and they are classified by the established Rockwood grading system. High-grade injuries are associated with fractures around the AC joint and disruption of the superior shoulder suspensory complex, a ring of osseous and ligamentous structures at the superior aspect of the shoulder. Radiographs are the mainstay of imaging of the AC joint, with magnetic resonance imaging reserved for high-grade injuries to a

Native Coronal Acromioclavicular Joint Orientation Is Associated with Rockwood Grade in Acute Acromioclavicular Joint Dislocations.

Background: The Rockwood classification is the most widely used radiographic framework for grading acromioclavicular (AC) joint dislocations. However, native anatomical variation may be associated with differences in the radiographic appearance of AC joint dislocations. Purpose: To evaluate the association between the acromioclavicular joint coronal orientation angle (AJCO) and Rockwood grade and to characterize the relationship between native coronal orientation and radiographic grade. Methods: This retrospective observational radiographic study included 198 adults with acute AC joint disloca

Subcoracoid dislocation of the acromioclavicular joint.

Subcoracoid acromioclavicular dislocations are rarely seen injuries of the shoulder. We present a patient with multiple injuries and a subcoracoid acromioclavicular dislocation resulting from a falling injury. Physical examination revealed painful mechanical block at 90 degrees of abduction and swelling in the acromioclavicular region. Diagnosis was made with direct radiographs and computerized tomography. Treatment consisted of distal clavicular resection and coracoclavicular reconstruction with the transfer of the coracoacromial ligament over the clavicle. The patient had a pain-free shoulde

Septic arthritis of the acromioclavicular joint.

The acromioclavicular joint is rarely the site of septic arthritis. We conducted a retrospective review at our rheumatology department, which identified five cases within the last 6 years. All five patients were males, and their mean age was 63 years. Risk factors were consistently identified and included intravenous substance abuse, prior joint disease, a recent history of intraarticular injections, and a remote history of surgery. Joint aspiration was performed in all five patients and provided the organism in two patients. Blood cultures recovered Staphylococcus aureus in three patients, a

Septic arthritis of the acromioclavicular joint.

Septic arthritis of the acromioclavicular (AC) joint is a rare but rapidly destructive etiology of acute shoulder pain. We report a case of septic AC joint in the absence of trauma or intravenous drug use. A diabetic man presented with severe right shoulder pain and was later diagnosed with septic arthritis of the AC joint by magnetic resonance imaging (MRI) and positive blood cultures. Chronic diabetic foot ulcers were the source of the patient's group B streptococcus bacteremia.

[Injuries of the acromioclavicular joint in athletes].

Acromioclavicular joint (ACJ) dislocation is a common injury to the shoulder girdle, especially in contact and high velocity sports. Besides the severity of the injury, and particularly in competitive and elite athletes and the type of sports, individual career plans and in and out of season injuries have to be taken into account when advising treatment for athletes. Conservative treatment is reserved for low-grade dislocations and in-season athletes. The aim is fast pain relief and a safe return to competitive sport. High-grade ACJ dislocations in athletes should be treated surgically. Arthro

Gouty arthropathy of the acromioclavicular joint.

Gouty arthropathy of the acromioclavicular joint is uncommon, with only two previously reported cases in the literature. The authors report a case in a 54-year-old man. The differential diagnosis of acromioclavicular arthropathy is reviewed, emphasizing that a high index of suspicion and aspiration of joint fluid are needed to make an accurate diagnosis.

Comparison of Suture Configurations for Acromioclavicular Joint Synthetic Reconstruction in Acromioclavicular Joint Separation: Finite Element Analysis.

BACKGROUND: In acromioclavicular (AC) joint separation, the weight of the upper extremity and scapula causes the scapula to rotate downward and internally along the thoracic cage. This results in displacement of the AC joint along three axes: anteroposterior, mediolateral, and vertical. Finite element models can aid in directing future biomechanical studies on this concept, not just the vertical axis, as has mainly been the focus in the past. This study uses finite element models to evaluate the stress across six suture configurations which reflects the efficacy on restraining AC joint motion

Eponymous terms in acromioclavicular joint surgery.

Eponymization serves as a means of paying tribute to individuals who have made significant contributions to our culture. Each eponym is often linked with a story for everyone to discover. To aid in the retention of these stories, this review offers readers an overview of the individuals behind the eponymous terms, as well as their original descriptions, within the context of acromioclavicular joint pathology and orthopaedic surgery.

[Surgical treatment of acromioclavicular joint injury].

The acromioclavicular joint plate according to Rahmanzadeh allows the safe retention of the reset acromioclavicular joint in stage Tossy III and in exceptional cases also in stage Tossy II traumata when simultaneous early functional post-treatment is done from the first postoperative day on. The clinical results obtained after follow-up examinations of 53 of the 83 patients treated in this way were mostly good or very good. Perfect radiological results were obtained in 50% of these patients; 40% showed a slight subluxation and 10% a marked but pain-free subluxation.

Acromioclavicular Joint Injuries: Evidence-based Treatment.

Injuries to the acromioclavicular (AC) joint are common in the athletic patient population. Most AC joint injuries occur in young males, typically from a direct fall onto the superior aspect of the shoulder when the arm is adducted. Numerous publications describing joint anatomy and biomechanics, surgical techniques for reconstruction, and rehabilitation protocols are available to guide treatment strategies for injuries to the AC joint. Treatment is typically nonsurgical for type I and II injuries and surgical for type IV and VI injuries. Controversy surrounds the indications for nonsurgical v

[Treatment of chronic acromioclavicular joint instability].

The purpose of this paper is to assess the results obtained with the arthroscopy-assisted surgical technique for the treatment of chronic acromioclavicular joint instability (CACJI), based on non-rigid coracoclavicular (CC) fixation and anatomical CC reconstruction with a tendinous allograft. Patients with CACJI who underwent surgery between 2008 and 2012 were included in the study. Clinical assessments included SF36, VAS and DASH, applied at the preoperative visit (POV) and at the last follow-up visit (LFUV). The Constant score and the General Satisfaction Score (0-10) were applied at the las

Acromioclavicular joint injuries: diagnosis and management.

Acromioclavicular joint injuries represent nearly half of all athletic shoulder injuries, often resulting from a fall onto the tip of the shoulder with the arm in adduction. Stability of this joint depends on the integrity of the acromioclavicular ligaments and capsule as well as the coracoclavicular ligaments and the trapezius and deltoid muscles. Along with clinical examination for tenderness and instability, radiographic examination is critical in the evaluation of acromioclavicular joint injuries. Nonsurgical treatment is indicated for type I and II injuries; surgery is almost always recom

[Treatment of acromioclavicular joint dislocation with the Rahmanzadeh joint plate].

Type Tossy III dislocations of the acromioclavicular joint (AC) are treated surgically in German-speaking countries. There is considerable rotation and tilting in the AC joint with elevation or abduction of the shoulder joint over 90 degrees. Thus, in the majority of countless surgical procedures, the abduction and elevation are restricted or immobilized for long periods in abduction strapping or other types of movement restriction. The AC joint plate developed by Prof. Rahmanzadeh enables early functional, complete physical therapy since the plate permits rotation and tilting. Over a period o
