Zone-specific bone density evaluation of the acromion may predict postoperative acromion stress fracture in patients undergoing a reverse total shoulder arthroplasty.

BACKGROUND: The goal of this study was to use preoperative computed tomography (CT) scans to identify differences in the Hounsfield units (HU) of the acromion in patients who did and did not develop a postoperative acromial and scapular spine fracture (ASF) after primary reverse total shoulder arthroplasty (rTSA). METHODS: A retrospective analysis was performed at a single institution. All patients undergoing an rTSA with either a 135° neck-shaft angle (NSA) humeral inlay design combined with a lateralized center-of-rotation (COR) glenosphere or a 145° NSA onlay combined with a medialized COR

Rotator cuff tears: association with acromion angulation on MRI.

OBJECTIVE: Using magnetic resonance imaging (MRI), evaluate the correlation of acromion angulation with thickening of the coracoacromial ligament (CAL) and narrowing of the subacromial space resulting in impingement upon the rotator cuff tendons. MATERIALS AND METHODS: Eighty-nine shoulder MRI studies performed on a 3T scanner were retrospectively analyzed by two blinded independent reviewers. Measurements of the acromion angle (delta angle), CAL thickness and distance between the CAL and humeral head were obtained. The data were categorized into two groups, delta angle less that and greater

Extra-articular Synovial Chondromatosis Eroding and Penetrating the Acromion.

Synovial chondromatosis of the shoulder is an uncommon disorder. It usually affects the glenohumeral joint and is characterized by metaplasia of the synovium leading to the formation of osteochondral loose bodies. Few cases of extra-articular subacromial synovial chondromatosis involving the rotator cuff tendon have been reported in the literature. The treatment of previously reported cases consisted of open bursectomy and removal of loose bodies. We report a case of subacromial synovial chondromatosis without rotator cuff involvement but with severe erosion and fracture of the acromion. Treat

[Reverse prosthesis and os acromiale or acromion stress fracture].

PURPOSE OF THE STUDY: We reviewed cases of acromion stress fracture or os acromiale among a consecutive series of 240 reverse prostheses implanted between 1995 and 2003. MATERIAL AND PATIENTS: Ten patients presented os acromiale, ten fracture-fragmentation of the acromion, and one nonunion of the scapular spine. The reverse prosthesis was implanted via a deltopectoral approach with no specific procedure on the acromion. There were 17 women and four men, mean age 72 years. The dominant side was involved for 70%. The prosthesis was indicated for excentered glenoid deterioration (n=14), massive

Acromion-fixation of glenoid components in total shoulder arthroplasty.

Successful design of components for total shoulder arthroplasty has proven to be challenging. This is because of the difficulties in maintaining fixation of the component that inserts into the scapula; i.e., the glenoid component. Glenoid components that are fixated to both the glenoid and acromion (a long process extending medially on the dorsal aspect of the scapula) have the possible advantage of greater stability over those that are fixated to the glenoid alone. In this study, a finite element analysis is used to investigate whether or not acromion fixation is advantageous for glenoid comp

Primary lymphoma of the acromion presenting with isolated shoulder pain.

Primary lymphoma of the bone is an uncommon malignancy that most frequently involves the long bones while the flat bone involvement is rare. We report the case of a man in his 20s who presented with progressive shoulder pain without systemic symptoms. Imaging demonstrated an intramedullary lytic lesion of the acromion with cortical thinning and minimal soft tissue involvement. Histopathological examination confirmed germinal centre type diffuse large B-cell lymphoma. Fluorine-18 fluorodeoxyglucose positron emission tomography/CT demonstrated a solitary hypermetabolic acromial lesion without no

Is the lateral extension of the acromion related to the outcome of shoulder injections?

OBJECTIVE: To assess patients' outcomes after subacromial or glenohumeral injections based on the degree of lateral extension of the acromion. METHODS: 307 patients were prospectively included after therapeutic fluoroscopy-guided subacromial (n = 148) or glenohumeral (n = 159) injections with anaesthetic and long-acting corticosteroids. Pre- and post-injection outcomes at 1 week and 1 month were obtained using the 11-point numerical rating scale (NRS) for pain. Lateral extension of the acromion was quantified and categorized by the critical shoulder angle (CSA) and the acromion index (AI) on

Pseudarthrosis of the acromion--a differential diagnosis in anterolateral shoulder pain.

A case of longstanding posttraumatic anterolateral shoulder pain in a 25-year-old woman is presented. It was primarily unsuccessfully handled as impingement syndrome but turned out to be caused by pseudarthrosis of the acromion. The abnormality was not apparent on plain X-ray film. The suspicion was raised after scintigraphy, which showed increased density in the acromion. The diagnosis was made by computerized tomography and confirmed and treated successfully surgically.

Is there an association between a low acromion index and osteoarthritis of the shoulder?

The aim of this study was to evaluate the relationship between a low acromion index and osteoarthritis of the shoulder. Three patient groups were used: (I) instability, n = 53; (II) calcifying tendonitis, n = 109; and (III) osteoarthritis, n = 120. Standardised digital X-rays were evaluated from the true anteroposterior and axillary views. Joint space width at three levels in each plane and the size of humeral osteophytes were measured and osteoarthritis was graded according to Samilson. The acromion index was calculated according to Nyffeler et al. (J Bone Joint Surg Am 88:800-805, 2006) in t

Arthroscopic treatment of isolated fracture of the posterolateral angle of the acromion.

Isolated fractures of the acromion are rare. Fracture avulsion of the posterolateral part of the acromion is even more rare. In 1996, a classification of Types I, II, and III was proposed, along with a recommendation for surgical treatment with open reduction and internal fixation for type III fracture. This report describes arthroscopic treatment provided according to a specialized technique that can be used for displaced avulsion fracture of the posterolateral angle of the acromion with reduction of the posterior subacromial space. This injury occurred in a 43-year-old recreational cyclist w

Acromion and Scapular Spine Fractures Following Reverse Total Shoulder Arthroplasty.

Reverse total shoulder arthroplasty (RSA) continues to see tremendous growth as the indications have expanded. A variety of complications have been described, including fractures of the acromion or scapular spine. These fractures are painful and can compromise shoulder stability and functional outcomes following RSA. Multiple studies have recently investigated the incidence, risk factors, and treatment strategies for these injuries. In this article, the authors review current literature and discuss the incidence, cause, associated risk factors, treatment options, and outcomes following fractur

Anatomical reconstruction to treat acromion fractures following reverse shoulder arthroplasty.

PURPOSE: Scapular fractures after reverse shoulder arthroplasty (RSA) are often associated with substantial shoulder impairment. Patient outcomes following either conservative or operative treatment have not been favourable, and consensus is lacking on the best treatment approach. We describe a technique for anatomic reconstruction of fractured lateral and basal acromion in patients at higher risk for diminished function or those for which conservative treatment has already failed. METHODS: Of the 95 patients who underwent RSA at our institution between December 2013 and December 2016, three

Giant Cell Tumor of the Acromion: Case Report and Literature Review.

BACKGROUND/AIM: Giant cell tumor of bone (GCTB) is a locally aggressive neoplasm that typically occurs in the ends (epiphyses) of long bones of young adults. Flat bones are uncommon sites of involvement. Herein, we describe an unusual case of pathologically proven GCT of the acromion. CASE REPORT: The patient was a 39-year-old woman with no history of trauma who presented with a 3-month history of right posterior shoulder pain. Physical examination revealed mild swelling and tenderness in the posterior aspect of the right shoulder. Plain radiograph showed a purely lytic lesion, suggestive of
