Actinium-225-PSMA versus lutetium-177-PSMA radioligand therapy for metastatic castration-resistant prostate cancer: results of an observational study.

BACKGROUND: Prostate-specific membrane antigen (PSMA)-targeted radioligand therapy (RLT) with Lutetium-177 (177Lu) and Actinium-225 (225Ac) is increasingly used in metastatic castration-resistant prostate cancer (mCRPC), but head-to-head prospective data remain limited. METHODS: We conducted an observational study of mCRPC patients treated with 177Lu-PSMA (n=116; 2022-2023) or 225Ac-PSMA (n=43; 2023). Primary outcomes were PSA response (≥50% decline) and overall survival (OS); secondary outcomes included hematologic toxicity. Treatments were given every 8 ± 2 weeks (1-6 cycles; median 2) with

Actinium-225-PSMA-617 treatment in a patient with advanced prostate cancer causes secondary myelofibrosis: a case report and literature review.

Advanced prostate cancer (PCa) is still an incurable disease. Still, the field of PSMA-targeted radioligand therapy is developing rapidly and is playing an increasingly important role in the treatment of advanced Pca in the future. As an α -particle emitter, Ac-225 shows a potent killing ability for tumor cells due to its short range and high energy deposition in the tumor micrometastasis focus. However, the secondary myelofibrosis (SMF) associated with Ac-225-PSMA-617 treatment is a significant concern. We present a case of metastatic castration-resistant prostate cancer (mCRPC) who developed

Radiochemistry and comparative in vitro assessment of PSMA-617 labeled with lead-212 (212Pb), actinium-225 (225Ac), and lutetium-177 (177Lu).

BACKGROUND: PSMA-targeted radioligand therapy is a promising approach for the treatment of advanced prostate cancer; however, the clinical efficacy of [177Lu]Lu-PSMA-617 (Pluvicto®) is limited by the relatively low cytotoxic potency of the β-emitting radionuclide 177Lu (t1/2: 6.65 d). This has driven high interest in α-emitting radionuclides, such as 212Pb (t1/2: 10.64 h) and 225Ac (t1/2: 9.92 d), which deliver high Linear Energy Transfer (LET) and cause more potent tumor cell killing. In this work, we assessed the radiolabeling and in vitro characteristics of 212Pb- and 225Ac-labeled PSMA-617

Efficacy of actinium-225-labeled fully human anti-MUC-16 radioimmunoconjugate against MUC-16-positive ovarian and pancreatic ductal adenocarcinoma xenograft models.

PURPOSE: MUC-16-targeted antibodies can deliver contrast agents (diagnostic) and/or cytotoxic payloads (therapy) to the tumor site. MUC-16 is overexpressed in 80% of epithelial ovarian cancer (EOC) and 65% of pancreatic ductal adenocarcinomas (PDAC). We previously developed a fully human antibody against MUC-16 (M16AB) and demonstrated the utility of [89Zr]Zr-DFO-M16AB as a PET imaging agent. Here, we developed [225Ac]Ac-Macropa-M16AB radioimmunoconjugate (RIC) and studied its efficacy against MUC-16-positive EOC and PDAC xenografts. EXPERIMENTAL DESIGN: [225Ac]Ac-Macropa-M16AB was characteri

CS1-specific single-domain antibodies labeled with Actinium-225 prolong survival and increase CD8+ T cells and PD-L1 expression in Multiple Myeloma.

Multiple myeloma (MM) is a hematological malignancy characterized by the presence of clonal plasma cells in the bone marrow niche. Despite significant therapeutic advances, MM remains incurable for the majority of patients. Targeted radionuclide therapy (TRNT) has emerged as a promising treatment option to eradicate residual cancer cells. In this study, we developed and characterized single-domain antibodies (sdAbs) against the MM-antigen CS1 and evaluated its therapeutic potential in MM using TRNT. We first validated CS1 as potential target for TRNT. CS1 is expressed in normal and malignant p

Alpha-Particle Therapy with an Actinium-225 Labeled Bivalent Inhibitor of Prostate-Specific Membrane Antigen is Therapeutically Efficacious in a Mouse Model of Prostate Cancer.

Prostate-Specific Membrane Antigen (PSMA) is overexpressed in some prostate cancers and is a viable candidate for targeted radionuclide therapy. PSMA targeted delivery of α2+ particle-emitting actinium-225 to tumors is a potential therapeutic option for prostate cancer. A substituted diazacrown ether macrocycle (Macropa) forms stable complexes with [225Ac]Ac3+. In this work, two PSMA-targeting lysine-ureido-glutamic acid pharmacophores are attached to a single Macropa macrocycle to give MacropaBisPSMA. The new conjugate was radiolabeled with [225Ac]Ac3+ to give [225Ac]AcMacropaBisPSMA. The the

Biodistribution of free Francium-221 and Bismuth-213 in Tumour-bearing SCID mice after successful development of Actinium-225/Francium-221 radionuclide generator Set-up.

PURPOSE: Despite the clinical evidence of actinium-225 (225Ac)-based targeted alpha therapies (TαT) efficacy, optimized treatment regimens are needed to improve overall clinical response rates and decrease toxicities. The nuclear recoil effect of 225Ac and its resulting daughter nuclides have been hypothesized to contribute to non-targeted damage. However, a lack of generator concepts for radionuclidically pure francium-221 (221Fr), involvement of strong acids for elution, and its short half-life (4.8 min), has limited in vivo studies. Here, we report on a successful application of an 225Ac/22

Low-dose temozolomide selectively increases glioblastoma's vascular permeability, tumor microenvironment penetration and the killing potential of systemic actinium-225 α-particle dendrimer-radioconjugates improving treatment efficacy.

PURPOSE: The poor prognosis of glioblastoma is mostly due to the relatively low tumor vascular permeability to therapeutics, the tumor's vicinity to the brain, that limits treatment aggressiveness, and/or drug resistance. METHODS: In this study, the efficacy of systemically injected actinium-225 dendrimer-radioconjugates was evaluated in an immune-competent orthotopic GL261-C57BL/6 mouse model after administration of low-dose, standard-of-care temozolomide, that selectively increased the tumor vascular permeability to dendrimer-radioconjugates. Peptide Receptor Radionuclide Therapy (PRRT) Using Actinium-225- and Ac-225/Lutetium-177-Labeled (TANDEM) Somatostatin Receptor Antagonist DOTA-LM3 in Patients with Neuroendocrine Neoplasm: A Retrospective Study Concerning Safety and Survival.

Peptide Receptor Radionuclide Therapy (PRRT) offers radiomolecular precision medicine for somatostatin receptor (SSTR)-positive advanced neuroendocrine neoplasms (NEN). In cases resistant to Lutetium-177-labeled DOTATATE or DOTATOC PRRT, alpha-therapy with Actinium-225 labeled with SSTR antagonists like DOTA-LM3 can be a notable therapeutic option. This retrospective study aimed to assess [225Ac]Ac-DOTA-LM3 safety in advanced NEN patients (as monotherapy and with Lutetium-177 as TANDEM), survival, and follow-up duration. Thirty-five patients received a total of 57 [225Ac]Ac-DOTA-LM3 cycles (Ma

Exploratory case series of circulating tumor DNA dynamics during tandem therapy in metastatic castration-resistant prostate cancer.

BACKGROUND: Prostate-specific membrane antigen (PSMA)-targeted radiopharmaceutical therapy (RPT) with the alpha-emitter actinium-225 (225Ac) has shown promising activity in metastatic castration-resistant prostate cancer (mCRPC), but its use is limited by toxicity. A tandem approach combining [225Ac]Ac-PSMA-617 and [177Lu]Lu-PSMA-617 (actinium-lutetium) has been developed to mitigate adverse effects and optimize efficacy. Given the scarcity of 225Ac and the emergence of resistance, early identification of non-responders is crucial. Circulating tumor DNA (ctDNA), especially tumor fraction (TFx)

Preclinical evaluation of [225Ac]Ac-PSMA-617 and in vivo effect comparison in combination with [177Lu]Lu-PSMA-617 for prostate cancer.

INTRODUCTION: The interest in targeted alpha therapy (TAT) has grown in the recent years as it can provide new treatment options for advanced- and late-stage cancer. In this sense, the use of actinium-225 is rising showing promising results. Even more, combining alpha and beta (lutetium-177) radionuclides might help to minimize actinium adverse effects, while preserving treatment efficacy. Preclinical studies of actinium-225-PSMA-targeting tracers for advanced prostate cancer and the "cocktail" combination with lutetium-177-PSMA needs to be further explored. METHODS: In vitro properties of [2

MRI detects tubulointerstitial changes in mouse models of radiation-induced nephropathy.

PURPOSE: We hypothesized that radiation-induced tubulointerstitial changes in the kidney can be assessed using MRI-based T2 relaxation time measurements. METHODS: We performed MRI, histology, and serum biochemistry in two mouse models of radiation nephropathy: one involving external beam radiotherapy and the other using internal irradiation with an α-particle-emitting actinium-225 radiolabeled antibody. We compared the mean T2 values of different renal compartments between control and external beam radiotherapy or α-particle-emitting actinium-225 radiolabeled antibody-treated groups and betwe

Preclinical Evaluation on the Radiohybrid Tandem [18F]La- and [225Ac]Ac-rhPSMA-10.1 for Targeted α-Therapy of Prostate Cancer.

Prostate-specific membrane antigen (PSMA)-targeted α-therapy holds promise for the treatment of prostate cancer but has increased side effects compared with therapy using β-emitting PSMA ligands. This emphasizes the need for precise imaging of the biodistribution of 225Ac-labeled ligands for patient selection and dosimetry. However, imaging of [225Ac]Ac-PSMA ligands remains technically challenging, and no radioactive actinium isotopes with favorable imaging characteristics exist. In this study, we therefore developed radiohybrid ligands that can be labeled with 225Ac for therapy and 18F for im
