It is too early for adjusting the adjustment disorder category.

PURPOSE OF REVIEW: Despite the relative frequency with which the diagnosis of adjustment disorder is made, there is a very limited research literature in regard to its cause, epidemiology and treatment. This review summarizes recent papers and findings in relation to this diagnostic category. RECENT FINDINGS: The conceptual underpinnings of the diagnosis adjustment disorder is the subject of ongoing debate as is its differentiation from other psychiatric disorders such as depressive disorders. One group has proposed and tested a diagnostic model of adjustment disorder as a stress-response syn

Common psychiatric disorders in cancer patients. I. Adjustment disorders and depressive disorders.

A high prevalence of adjustment and depressive disorders has been reported in oncology. These conditions are highly treatable, therefore the assumption that emotional distress is just a foreseeable and ordinary reaction to cancer had to be reviewed. Clinical presentation, etiology, assessment, and therapy of adjustment and depressive disorders are summarized in order to optimize supportive care.

Readmission rates for adjustment disorders: comparison with other mood disorders.

BACKGROUND: The diagnostic category of adjustment disorders continues to receive little attention in the research literature despite its estimated incidence of 5-21% in psychiatric consultation services for adults and 7.1% in inpatient admissions. METHODS: Ten years of readmission data were reviewed for six diagnostic categories: adjustment disorders, major depressive disorder (single episode and recurrent), dysthymia, any anxiety disorder and depression NOS. Cox regression analysis was used. RESULTS: Admission diagnosis was a significant predictor of readmission, with adjustment disorders r

Adjustment disorders: the state of the art.

Adjustment disorders are common, yet under-researched mental disorders. The present classifications fail to provide specific diagnostic criteria and relegate them to sub-syndromal status. They also fail to provide guidance on distinguishing them from normal adaptive reactions to stress or from recognized mental disorders such as depressive episode or post-traumatic stress disorder. These gaps run the risk of pathologizing normal emotional reactions to stressful events on the one hand and on the other of overdiagnosing depressive disorder with the consequent unnecessary prescription of antidepr

[Adjustment disorders--nosological state and treatment options].

OBJECTIVE: To provide an overview of empirical findings with regard to adjustment disorders. METHODS: Literature was identified by searches in Medline and Cochrane Database of Systematic reviews. RESULTS: Adjustment disorder is a common diagnosis in clinical samples (5% to 24%). There is a high frequency of comorbid somatic and mental disorders, especially of personality disorders (15% to 73%). The diagnostic stability of adjustment disorders in the long-term courses seems to be low. It is unclear to what extent adjustment disorders can be differentiated from affective or anxiety disorders w

Suicidal behavior in patients with adjustment disorders.

This retrospective study characterizes the suicidal behavior in 119 patients with Axis I adjustment disorders as assessed by psychiatrists at the University of Virginia Hospital. Results indicated that 72 patients (60.5%) had documented suicide attempts in the past, 96% had been suicidal during their admission to the hospital, and 50% had attempted suicide before their hospitalization. The most commonly used method of suicide attempts was overdosing. Of the sample group with suicide attempts in the past, 67% had Axis II diagnoses of borderline personality disorder and antisocial personality di

Treatment of adjustment disorders: a comparative evaluation.

With the aim of assessing four forms of therapy with adjustment-disordered outpatients, we randomly assigned 70 subjects to the following treatments: supportive psychotherapy (psychoanalytically oriented), viloxazine (an antidepressant), lormetazepam (a benzodiazepine), and S-adenosylmethionine (a methyl donor with antidepressive properties). A further group of 15 subjects received a placebo, orally administered. The trial lasted 4 wk. None of the treatments had clearly superior effects over others on scores on the Zung Self-rating Depression Scale. All produced a significant improvement. Howe

Early maternal adjustment in women with eating disorders.

OBJECTIVE: To study early adaptation to motherhood in mothers with eating disorders (ED) before pregnancy. METHOD: Forty-four nulliparous nonsmoking women with ED before pregnancy (24 anorexia nervosa, 20 bulimia nervosa) and 67 controls were recruited from the same prenatal clinics. Three months after delivery, the women completed the maternal adjustment and maternal attitude questionnaire (MAMA) and were asked about mental health problems postpartum. RESULTS: Ninety-two percent of mothers with ED before pregnancy reported problems regarding their maternal adjustment compared to 13% in the

[Temporary vision disorders, accentuated by deficient illness adjustment].

A medical expert's opinion had to be given on five patients complaining about temporary visual disturbances. 1. Haze caused by keratoconjunctivitis sicca; 2. changing refraction related to diabetes; 3. phosphenes in a paralyzed monoculus; 4. double vision with fatigue in myasthenia; 5. presumed elevation of the risk for accidents in congenital nystagmus. The loss of earning capacity in these cases should be calculated as the mean of the earning capacity during the phases of least and maximal disturbance, if it is not yet included in the loss caused by additional stationary defects. [Objective adjustment of hearing aids to hearing disorders].

The aim of a functional prosthesis - a hearing aid - is to improve the comprehensibility of speech. The hearing aid is individually adjusted for the patient. Every deaf patient is subjected first to an extensive otolaryngological and audiological examination programme which comprises the recording of the case-history, assessment of the ENT diagnosis, examinations of threshold and suprathreshold sound audiometry, verbal and sentence audiometry in a sound-proof chamber and in space. To set the most important parameters of hearing aids (frequency characteristics, intensification and dynamic range

The adjustment disorders: the conundrums of the diagnoses.

This article discusses the conundrums and the dilemmas of the conceptual framework of the diagnoses of the adjustment disorders (ADs). The psychiatric diagnoses that arise between normal behavior and the major psychiatric morbidities constitute the problematic subthreshold disorders. These subthreshold entities are also juxtaposed between problem-level diagnoses and more clearly defined major disorders. They present important taxonomic and diagnostic dilemmas in that they are often poorly defined, overlap with other diagnostic groupings, and have indefinite symptomatology. ADs were designed as

[Social competence in children with social adjustment disorders].

Teachers and parents of a sample of 1774 students at three grade levels (4th, 6th and 8th grades) were asked to judge the social competence of the children by means of the social competence questionnaire by Buhrmester et al. (1988). These ratings were correlated with the social status of the pupils, with peer statements on aggressive behavior and victimization and with the teachers' ratings of behavior in class and towards fellow pupils. Teachers proved to be more critical than parents, crediting boys with less social competence than girls, though this difference diminished in the higher grade

Screening of adjustment disorder: Scale based on the ICD-11 and the Adjustment Disorder New Module.

In line with ICD-11 new conceptualization of Adjustment disorder (AjD), a self-report Adjustment Disorder-New Module (ADNM) was developed and validated. Nevertheless, the ADNM-20 is a long research tool and potentially problematic in the use in epidemiological and clinical studies. The present study introduces the brief ADNM-8 and the ultra-brief ADNM-4, examines their validity and establishes cut-off scores for their clinical use. The study used a representative national sample of 1003 Israelis who reported on the ICD-11 stress spectrum ranging from AjD, PTSD, complex PTSD and complicated gri

Screening for adjustment disorders and major depressive disorders in cancer in-patients.

The Hospital Anxiety and Depression Scale (HADS), a four-point, 14-item questionnaire, was tested as a screening method for adjustment disorders and major depressive disorders in a sample of 210 cancer in-patients. A receiver operating characteristic (ROC) analysis was performed, giving the relationship between the true positive rate (sensitivity) and the false positive rate (1-specificity). This makes it possible to choose an optimal cut-off point that takes into account the costs and benefits of treatment of psychological distress. For screening for major depressive disorders only, a cut-off

Pharmacotherapy of adjustment disorder: A review.

BACKGROUND: Adjustment disorder has been reconceptualized as a trauma- and stressor-related condition, and there is a growing understanding of the psychobiology of stress responses. Against this context it is timely to review of the pharmacotherapy of adjustment disorder. METHODS: A comprehensive electronic database (Pubmed) was searched for randomised controlled trials of the pharmacotherapy of adjustment disorder. Data from each trial were extracted and collated. RESULTS: To date there have been relatively few controlled trials in this area. Comparator trials provide limited support for a
