Palliative care in systemic sclerosis: incorporating advance care planning into longitudinal care.

Despite advances in rheumatological treatments, patients with systemic sclerosis (SSc) suffer with myriad symptoms and experience associated quality of life decrements. Although SSc-related mortality is reported around 53%, there is a paucity of existing literature describing advance care planning and palliative care in SSc. Here we review symptom management burden and strategies by organ manifestation: interstitial lung disease, pulmonary hypertension, gastrointestinal involvement, cutaneous and musculoskeletal manifestations and digital ischaemia. Then we review domains of palliative care in

Advance Care Planning in Huntington Disease: The Elderly Care Physician's Perspective.

OBJECTIVE: Huntington disease (HD) has a poor prognosis. Decision-making capacity and communication ability may become impaired as the disease progresses. Therefore, HD patients are encouraged to engage in advance care planning (ACP). Elderly care physicians (ECPs) can play an important role in ACP in HD patients. However, little is known about their experiences in this role. The aim of this study is to gain insight into how ECPs practice ACP with HD patients. DESIGN: A qualitative interview study. SETTING AND PARTICIPANTS: Nine ECPs working in HD-specialized nursing homes in the Netherlands

Feasibility of Advance Care Planning in Primary Care for Homeless Adults.

Objectives: As the homeless population ages, it is imperative to improve access to advance care planning (ACP) and document preferences in case medical decision-making capacity is lost. Methods: We implemented an ACP Project to discuss and document advance care plans with all patients aged 45 and older who received primary care at our adult Homeless Program clinics. Results: Over 14 months, ACP was discussed with 48% (n = 138) of the population and health care proxy (HCP) appointment with 91% (n = 125) of these patients. Most (62%; n = 77) appointed a HCP from personal relationships, though a

Advance Care Planning in Indigenous Populations: A Scoping Review.

BACKGROUND: Advance care planning has proven to be beneficial to medical care as it elicits patient preferences at the end of life, yet Indigenous populations have low rates of participation. OBJECTIVE: This gap map scoping review serves to assess the current literature regarding the prevalence of advance care planning and palliative care characteristics in Indigenous populations of North America and Oceania. DESIGN: Articles were identified from searches performed in five databases spanning the period from 2013 to 2023. RESULTS: Twenty-eight articles met the inclusion criteria, representin

Advance Care Planning Challenges in Adult Congenital Heart Disease.

Due to advances in congenital heart disease management, there are now more adults than children living with congenital heart disease in the United States. This population of over 1.4 million people is expected to continue to grow rapidly as a result of improving survival. However, patients with adult congenital heart disease continue to face increased rates of morbidity, hospitalizations, and medical interventions compared to the general population, in addition to elevated mortality. Therefore, comprehensive advance care planning is integral to caring for this patient population. Yet despite r

Advance Care Planning: Perspectives of People Living in Prison.

A person-centered approach to advance care planning is recognized as a fundamental need, yet its routine implementation remains a challenge across disparate settings, such as prisons. The purpose of this study was to gain the perspectives of people who are incarcerated about advance care planning. Four focus groups were conducted with people living in 1 men's and 1 women's state prison (n = 26). Handwritten field notes were taken, transcribed, deidentified, and verified before the completion of independent thematic analysis by 2 experienced qualitative researchers. Key themes regarding advance

Advance care planning for patients with advanced neurology diseases.

BACKGROUND: Advanced neurology diseases including motor neuron disease (MND) are usually progressive life-limiting illness and could be devastating for patients, families and caregivers. Although medical technologies, such as enteral feeding and non-invasive ventilation, may prolong life expectancy of the patients, their utilization prompts important ethical questions in regard to their quality of life (QoL). Little attention had been paid on how ACP practice would practically help with patients suffering from different neurology diseases. We are unaware of any published studies on ACP practic

Advance care planning in people with early cognitive impairment.

BACKGROUND/AIM: People with early cognitive impairment (ECI), comprising mild cognitive impairment and early dementia face the risk of diminished mental capacity with progressive disease. Advance care planning (ACP) is advocated as a means to uphold the autonomy of individuals and enable them to express their wishes for the future while they still possess mental capacity. This study aims to explore the perspectives of patients with ECI regarding ACP, and in particular, understand their reasons for resistance. METHOD: Consecutive patients undergoing a counselling service for persons with ECI w

Hearing their Voice: Advance Care Planning for the Homeless.

Homeless individuals are seldom offered the opportunity to complete advance directives and designate decision makers. During a catastrophic illness, health care providers are left to do aggressive, life-prolonging treatments that the patient may not want while seeking decision makers who may not be familiar with the patient or what their wishes would be. The authors spearheaded a program to offer homeless individuals the opportunity to review and complete an advance directive and parlayed that work into a secondary project to provide a comfortable end-of-life experience for homeless patients.

Facilitating advance care planning in oncology: study protocol for the evaluation of a tailored implementation of an advance care planning tool (the INDEED study).

BACKGROUND: Advance care planning (ACP) improves patient outcomes, but integration of ACP in daily practice for patients with cancer in hospitals remains suboptimal. Systematic implementation of tools to facilitate ACP could overcome the barriers for ACP. In this study we will evaluate the multifaceted tailored implementation of a digital ACP tool embedded in the electronic health record (EHR) in daily clinical cancer care. We will analyse the implementation process, its effect on use of the ACP tool, and the impact of use of the tool on healthcare use and healthcare costs. METHODS: We will p

Advance care planning and surrogate health care decision making for older adults.

Older adults have a legally recognized right to determine and control the health care they receive and are legally presumed to possess the capacity necessary to make health care decisions. However, older adults are at major risk for life-threatening and serious health problems leading to a loss of their capacity to make end-of-life and other health care decisions. Advance care planning is a process that enables older adults to specify who they want to act as a surrogate decision maker and what decisions they want made for them in the event of incapacitation. Geriatric nurses can play a signifi

Impact of nurse-led advance care planning in a primary care setting.

OBJECTIVES: We aimed to evaluate patient and clinician experiences of advance care planning (ACP) conversations facilitated by an ACP nurse coordinator (ACP-NC). METHODS: We used a sequential mixed-methods approach that included a patient survey, patient interviews, and clinician interviews at a family medicine clinic. Patients were invited into the study if they had reached a stable point in their ACP decision-making conversations with the ACP-NC or PCP (i.e. their goals and preferences were considered settled at the time) and were not imminently dying. Invitations to complete a survey were

Development of a Dutch Information Standard for Advance Care Planning.

Advance care planning (ACP) information, concerning goals and preferences for future medical treatment and care, is often recorded but its digital exchange between healthcare providers is not yet self-evident. This paper describes the process of the development of a Dutch information standard for ACP, aimed to support this information exchange. For the current release of the information standard, a functional design with a dataset description based on Dutch Health Care Information Models and FHIR implementation guides for technical specifications were developed. This release underwent public c

Disparities in Advance Care Planning: Did COVID-19 Change Anything?

BACKGROUND: Rates of advance care planning (ACP) are lower and preferences for life-prolonging treatment are higher among Black compared to White older adults. We examined whether these differences persisted during the COVID-19 pandemic. METHODS: Between February 2021 and September 2022, we conducted a cross-sectional COVID-19-focused survey of seriously ill adults ≥ 65 years in 10 primary care clinics participating in a clinical trial of two ACP interventions. Logistic regression models examined associations between COVID-19 related ACP discussion (defined as discussions with family, friends

Advance Care Planning in Sickle Cell Disease: A Scoping Review.

Sickle cell disease (SCD) is an inherited hemoglobinopathy characterized by abnormal red blood cell sickling, leading to pain, organ dysfunction, and early mortality. Its severe, unpredictable course and the emergence of complex decisions surrounding transformative therapies have prompted recommendations to integrate palliative care (PC) to support patients and families. Advance care planning (ACP) is a component of PC that seeks to align medical care with patient values and preferences. Individuals with SCD may benefit from ACP throughout the lifespan, yet best practices remain poorly defined

Workforce Readiness for Advance Care Planning in Intellectual Disability Services.

CONTEXT: Advance Care Planning (ACP) improves communication, goal-concordant care, and end-of-life outcomes for people with serious illness. For people with intellectual disabilities (ID), ACP is essential for promoting autonomy but remains inconsistently implemented. Professional caregivers in disability services often provide long-term support and may be key facilitators of ACP; however, their preparedness for this role remains unclear. OBJECTIVES: To evaluate workforce readiness for ACP in intellectual disability services by examining caregivers' conceptual knowledge, self-perceived compet
