Factors influencing advance directives among psychiatric inpatients in India.

OBJECTIVES: Advance directives are documents stating treatment preferences in case of future lack of decision making capacity. In India, as in many other countries, legislators advocate Psychiatric Advance Directives (PADs), while evidence on its use is limited. This study examined factors influencing PADs by gathering inpatients perspectives on PADs at discharge and investigating patient characteristics associated with the expression of treatment wishes in PADs. METHODS: We conducted a hospital based descriptive study in Bangalore. 200 patients were included. The Mini International Neuropsyc

Advance treatment directives for people with severe mental illness.

BACKGROUND: An advance directive is a document specifying a person's preferences for treatment, should he or she lose capacity to make such decisions in the future. They have been used in end-of-life settings to direct care but should be well suited to the mental health setting. OBJECTIVES: To examine the effects of advance treatment directives for people with severe mental illness. SEARCH STRATEGY: We searched the Cochrane Schizophrenia Group's Register (February 2008), the Cochrane Library (Issue 1 2008), BIOSIS (1985 to February 2008), CINAHL (1982 to February 2008), EMBASE (1980 to Febru

The content and clinical utility of psychiatric advance directives.

OBJECTIVE: This paper provides the first systematic examination of the content and clinical utility of psychiatric advance directives, which are documents that specify treatment preferences in advance of periods of compromised decision making. METHODS: Directives were completed by 106 community mental health center outpatients with at least two psychiatric hospitalizations or emergency department visits within two years. Participants used AD-Maker software in groups of up to six people led by peer trainers. Clinical utility was defined as the degree to which instructions are clinically feasib

Attitudes and knowledge relating to advance healthcare directives among psychiatry staff in Ireland.

BACKGROUND: Advance healthcare directives are the subject of increased interest in psychiatry. A new legislative framework for advance healthcare directives came into effect in Ireland in 2023 in the Assisted Decision Making (Capacity) Act, 2015. AIM: To investigate levels of knowledge about, and attitudes towards, advance healthcare directives among multidisciplinary psychiatry inpatient staff in Ireland one year after commencement of the new legislation. METHODS: A survey was completed among psychiatry inpatient staff (n = 51) in Tallaght University Hospital, Dublin, Ireland between June a

Factors influencing the completion of advance directives in cancer patients: a descriptive survey.

AIM: This study aimed to investigate the factors associated with the completion of advance directives among cancer patients. BACKGROUND: Despite legislation to support end-of-life decision-making, the completion rate of advance directives (AD) in South Korea remains low. In many cases, family members make end-of-life decisions on behalf of patients. To promote patient autonomy and reduce family burden, early initiation of AD discussions is essential. METHODS: Survey data on demographics and factors related to AD completion were collected through a written survey administered by the researche

Patient perspectives on advance euthanasia directives in Huntington's disease. A qualitative interview study.

BACKGROUND: Huntington's disease (HD) has a poor prognosis. For HD patients in the Netherlands, one way of dealing with their poor prognosis is by drawing up an advance euthanasia directive (AED). Little is known about the perspectives of HD patients on their AED. AIM: To gain insight into patients' views on and attitudes towards their AED, and changes over time. METHODS: A longitudinal qualitative interview study using 1 to 6 semi-structured interviews over a period of maximum three years. Nine HD patients (5 outpatient clinic, 3 day care, 1 assisted living facility) who either had an AED o

Euthanasia and advance directives in Huntington's disease: qualitative analysis of interviews with patients.

BACKGROUND: In the literature there are few reports on euthanasia or physician-assisted suicide (PAS) or other matters concerning the end-of-life in patients with Huntington's disease (HD), although clinical experience suggests these issues do arise. OBJECTIVE: To obtain in-depth information about patients' thoughts on and attitudes to euthanasia, PAS and the use of advance directives in HD. To assess the difficulties patients encounter when thinking about end-of-life wishes. METHODS: Semi-structured in-depth interviews with 14 unselected HD patients from our out-patient clinic based on a to

Advance directives: can patients' stated treatment choices be used to infer unstated choices?

Advance directives have been widely endorsed as a method to match medical interventions to patients' preferences. However, applying advance directives to unspecified future decisions may be problematic. We wondered how well scenario-based treatment choices in advance directives can be used to infer other choices specified by the same patients. We asked 495 out-patients from the Massachusetts General Hospital to state 11 treatment choices in each of four illness scenarios. We calculated likelihood ratios (LRs) to see how well one choice could predict another. Predictions within a given scenario

Implementing the living will and other advance directives: guidelines for health care providers.

Living wills and other advance directives for the refusal of medical treatment by patients no longer able to make their own medical decisions have gained increasing acceptance in the last decade. In addition to constitutional and common law rights to refuse treatment, living will statutes have been enacted in many states to permit advance directives under specified circumstances. This article explores both statutory and non-statutory advance directives and offers guidelines for hospitals and medical staffs when decisions regarding life-sustaining treatment must be made for adults with terminal

Basic care, pain relief, and advance healthcare directives in Ireland.

The Assisted Decision-Making (Capacity) Act 2015 (ADMCA) provides a legislative basis for advance healthcare directives (AHDs) in Ireland. AHDs offer important benefits to patients, including preserving autonomy and allowing for greater control over end-of-life care. However, they can also create difficult ethical dilemmas, especially in respect of AHDs refusing basic care, pain relief and palliative symptom management. We use a hypothetical case study to interrogate how such directives should be addressed under the ADMCA and identify some uncertainties and ambiguities within the ADMCA concern

Anorexia nervosa, advance directives, and the law: A British perspective.

This article will explore whether the law should allow people with anorexia nervosa to refuse nutrition and hydration with special reference to the English decision in Re E (Medical Treatment: Anorexia). It argues that the judge in that case made the correct decision in holding that the patient, who suffered from severe anorexia nervosa, lacked capacity to make valid advance directives under the Mental Capacity Act 2005 of the United Kingdom, and that medical procedures that are apparently against her wishes should be carried out for the sake of preserving her life. The law should generally no

Threats to "informed" advance directives for the severely physically challenged?

The neuromuscular diseases, such as infantile spinal muscular atrophy, Duchenne's muscular dystrophy, and amyotrophic lateral sclerosis, are widely considered to be terminal illnesses. However, as with many neuromuscular and neurologic diseases, morbidity and mortality are caused by dysfunction of inspiratory, expiratory, and bulbar musculature. This article will discuss how inspiratory and expiratory musculature can be supported by simple, noninvasive means that are rarely considered when, as with the general population, individuals with disabilities are counseled about advance directives. ["Let's decide together": facilitating advance directives in nursing homes and long-term care facilities].

The formalization of advance directives remains uncommon in institutions, hampered by insufficient information, the taboo surrounding the end of life, and communication barriers. "Décidons ensemble" (Let's decide together), a co-constructed educational tool (a board game structured around six themes and online modules), aims to reduce these barriers. An exploratory phase in residential care facilities for dependent elderly people and long-term care units is assessing feasibility, acceptability, and feedback. It is an ethical tool that can be replicated and adapted to other care contexts.

Advance directives prior to COVID-19 diagnosis in a United States national healthcare system.

BACKGROUND: An advance directive (AD) document allows a patient to indicate their health care preferences and identify an agent to make decisions on their behalf if they lose their ability to communicate. Due to the substantially elevated risk of acute respiratory failure and death during the COVID-19 pandemic, ADs were especially relevant. The objective of this study was to describe AD completed prior to COVID-19 infection (COVID-19) among patients receiving care in a national healthcare system. METHODS: We conducted a cohort study of United States Veterans Health Administration (VA) patient

Reproductive psychiatric advance directives: promoting autonomy for perinatal people with serious mental illness diagnoses.

People with serious mental illness (SMI) diagnoses who become pregnant are particularly vulnerable to symptom recurrence and resulting potential lack of decision-making capacity (Taylor et al. J Psychiatr Res 104:100-107, 2018; Bagadia et al. Int J Soc Psychiatry 66:792-798, 2020). In these situations, prenatal and behavioral health providers have little legally viable guidance on what medical and/or psychiatric care the patient desires (Aneja and Arora Indian J Med Ethics V:133-139, 2020). We created a "Reproductive Psychiatric Advance Directive (PAD)," grounded in Reproductive Justice princi

Advance directives and the Patient Self-Determination Act: what is a nurse to do?

Since the Patient Self-Determination Act was enacted in 1991, asking patients whether they have advance directives (eg, a Living Will and a Durable Power of Attorney for Health Care Decisions) has become part of the admission procedures at health care facilities. However, questions may still remain about what exactly is the role of the health care provider in this part of the admission process. In many health care settings, the nurse has the responsibility to ask the patient about advance directives. If this occurs during preparation for surgery or other medical procedures, it adds responsibil

Advance Directives Revisited (2002 vs 2023): Are We Closer to Uniformity?

BACKGROUND AND OBJECTIVES: Advance directives (ADs) are based on state-specific statutes that vary in structure, terminology, and options. This variability leads to inconsistent end-of-life (EOL) care for patients who have executed an AD in 1 state but fall ill in another state. This study revisits a 2002 article that identified considerable differences in ADs to determine whether ADs have become more uniform. RESEARCH DESIGN AND METHODS: ADs from all 50 states and the District of Columbia were examined to determine the frequency of document types and seven key issues. The results were compar

Knowledge of advance care planning and advance directives among US adults: Implications for health policy and practice.

BACKGROUND: Advance care planning (ACP) is the process of identifying and communicating values and goals in preparation for future shared decision-making in medical settings. Decisions are legally supported by an advance directive (AD) document designating healthcare agent(s) and future medical treatment preferences. Although important, ACP engagement in the United States remains low. Previous US research focuses heavily on ACP among older adults and those with life-limiting illness. Few studies have sought to understand knowledge of and engagement with ACP among the general population in prim

Barriers and facilitators for mental-health professionals in the management and implementation of advance directives.

BACKGROUND: The Advance Directive (AD) serves as a fundamental legal and ethical instrument for safeguarding patient autonomy, ensuring that clinical decisions align with individual values when decision-making capacity is compromised. Despite the established legal framework in Spain since 2002, its practical implementation within mental health services remains largely under-researched. The objective of our study is to describe the barriers and facilitators perceived by mental-health professionals regarding the clinical management and implementation of the Advance Directive (AD). METHODS: A de

The Directiveness of Advance Directives. Performing Euthanasia on Individuals with Severe Dementia.

This paper inquires whether performing euthanasia in accordance with an advance directive is acceptable in the case of individuals with severe dementia. Specifically, the Convention on the rights of persons with disabilities is examined to determine whether it provides the basis to perform euthanasia in these cases. The meaning of certain values in particular is analysed, namely, equality, (legal) personhood, autonomy, freedom, dignity and physical and mental integrity. The intricate aspect of the identity of individuals with severe dementia merits special attention.
