Clinical Diagnosis and Differential Diagnosis Between CSF1R- and AARS2-Related Leukoencephalopathy.

CSF1R-related leukoencephalopathy (CSF1R-L) and AARS2-related leukoencephalopathy (AARS2-L) were two disease entities sharing similar phenotype and even pathological changes. Although clinically, radiologically, and pathologically similar, they were caused by mutation of two different genes. As the rarity of the two diseases, the differential diagnosis of them was difficult. 23 CSF1R-L and 6 AARS2-L patients were enrolled from the Leukoencephalopathy Clinic, Peking Union Medical College Hospital in China. Detailed clinical information, neuroimaging manifestations, and genetic data were collect

The emerging neurological spectrum of AARS2-associated disorders.

BACKGROUND: The AARS2 gene encodes a mitochondrial alanyl-transfer RNA synthetase. Defects in this gene have been linked with autosomal recessive inheritance of a variety of different clinical phenotypes. CASE: A 13 year-old boy developed behavioral and psychiatric problems following a mild head injury. At age 21 he developed tremor, parkinsonism, and eye nystagmus. MRI revealed white matter changes consistent with a leukoencephalopathy. Genetic studies revealed two pathogenic mutations in the AARS2 gene (c.647dupG and c.595C > T). LITERATURE REVIEW: Only 47 cases of AARS2-associated disorde

CMT type 2N disease-associated AARS mutant inhibits neurite growth that can be reversed by valproic acid.

Charcot-Marie-Tooth (CMT) disease is composed of a heterogeneous group of hereditary peripheral neuropathies. The peripheral nervous system primarily comprises two types of cells: neuronal cells and myelinating glial Schwann cells. CMT2 N is an autosomal dominant disease and its responsible gene encodes alanyl-tRNA synthetase (AARS), which is a family of cytoplasmic aminoacyl-tRNA synthetases. CMT2 N is associated with the mutation, including a missense mutation, which is known to decrease the enzymatic activity of AARS, but whether and how its mutation affects AARS localization and neuronal p

Non-convulsive status epilepticus revealing acute deterioration in AARS2 mutation-associated progressive leukoencephalopathy.

Nonconvulsive status epilepticus (NCSE) is defined as a condition of prolonged epileptiform activity on electroencephalography (EEG) accompanied by altered mental status without prominent motor manifestations. It is frequently underdiagnosed, particularly in patients with pre-existing cognitive impairment or neurodegenerative disorders [1, 2]. Reports describing NCSE in patients with AARS2-related leukoencephalopathy remain extremely limited. This case identifies NCSE as a potential cause of clinical deterioration in AARS2-related leukoencephalopathy and highlights the importance of considerin

Gait Apraxia with Exaggerated Upper Limb Movements as Presentation of AARS2 Related Leukoencephalopathy.

A 55-year-old male presented with apraxia of gait with exaggerated upper limb movement with relative preservation of cognition and mild spasticity of limbs. His investigations reveal posterior-predominant leukodystrophy in brain magnetic resonance imaging (MRI) and compound heterozygous mutations in mitochondrial alanyl-transfer RNA synthetase 2 (AARS2) by next generation sequencing. His asymptomatic brother also has MRI changes with subtle mild pyramidal signs. AARS2 mutation is a rare cause of mitochondrial encephalopathy which may give rise to leukodystrophy with premature ovarian failure,

Clinical characteristics and proteome modifications in two Charcot-Marie-Tooth families with the AARS1 Arg326Trp mutation.

BACKGROUND: Aminoacyl tRNA-synthetases are ubiquitously-expressed enzymes that attach amino acids to their cognate tRNA molecules. Mutations in several genes encoding aminoacyl tRNA-synthetases, have been associated with peripheral neuropathy, i.e. AARS1, GARS1, HARS1, YARS1 and WARS1. The pathogenic mechanism underlying AARS1-related neuropathy is not known. METHODS: From 2012 onward, all probands presenting at Telemark Hospital (Skien, Norway) with peripheral neuropathy were screened for variants in AARS1 using an "in-house" next-generation sequencing panel. DNA from patient's family member

Evidence that translational control mechanisms operate to optimize antifreeze protein production in the winter flounder.

In fall and winter, the liver of the winter flounder produces large amounts of alanine-rich (60 mol %) antifreeze proteins for export to the circulation. We have examined the tRNA in the liver to see if the seasonal production of antifreeze protein is accompanied by changes in tRNAAla isoacceptors. Total tRNA from the liver of winter fish showed an approximate 40% increase in alanine acceptor capacity over tRNA from summer fish. In contrast, the acceptor capacities for other amino acids showed no seasonal difference. When labeled alanyl-tRNAs were separated by reverse phase chromatography-5 ch

Expedited Exome Reanalysis Following Deep Phenotyping and Muscle Biopsy in Suspected Mitochondrial Disorder.

BACKGROUND: Exome sequencing (ES) is a useful tool in diagnosing suspected mitochondrial disease but can miss pathogenic variants for several reasons. Additional testing, such as muscle biopsy or biochemical testing, can be helpful in exome-negative cases. METHODS: We report a patient who presented with repeated episodes of lactic acidosis and failure to thrive. RESULTS: ES and mitochondrial sequencing were initially negative but clinical suspicion for mitochondrial disease remained high. After muscle biopsy showed evidence of mitochondrial dysfunction, the ES was reanalyzed and revealed nov

Clinical manifestations in patients with antibody to PL-12 antigen (alanyl-tRNA synthetase).

PURPOSE: Anti-PL-12 antibody is directed at the enzyme alanyl-tRNA synthetase (ARS). Studies have clearly associated anti-Jo-1, also directed at an aminoacyl-tRNA synthetase (histidyl-tRNA synthetase), with a subgroup of myositis marked by a high frequency of interstitial lung disease (ILD) and arthritis. A similar syndrome has been reported in patients with antibodies to PL-12, but few patients have been studied. We describe the clinical manifestations in a new series of patients with antibody to PL-12. PATIENTS AND METHODS: Sera from patients with polymyositis and sera found to contain anti

Unconventional biocatalytic strategies orchestrate synthesis of the nucleoside analog sinefungin.

Sinefungin is a potent nucleoside antimetabolite of S -adenosylmethionine (SAM), yet its biosynthesis has remained unclear for decades. Here we detail the identification and characterization of the complete sinefungin biosynthetic gene cluster (BGC) from Streptomyces incarnatus NRRL 8089. In vitro and in vivo analyses demonstrate that the defining carbon-carbon (C-C) bond is formed not by the long-hypothesized PLP-dependent process, but by a vitamin B 12 -dependent radical SAM enzyme. Using isotope-labeled cofactors and substrates, we provide evidence that the adenosyl group of sinefungin atyp

Anti-alanyl tRNA positive antisynthase syndrome with Kaposi sarcoma.

We report a rare case of antisynthase syndrome (ASS) complicated with Kaposi sarcoma, analyze its clinical characteristics, and review the literature on the topic. An 80-year-old male patient developed fever, cough, and shortness of breath. Lung high-resolution computed tomography showed nonspecific interstitial pneumonia in both lungs, and myositis antibody examination showed strongly positive anti-alanyl tRNA synthase (PL-12) antibodies. Based on these findings, the patient was diagnosed with ASS. After full-dose glucocorticoid treatment, the symptoms of fever and cough were relieved, but sk

Shared molecular profiles of anti-PL12 autoantibodies in anti-synthetase syndrome.

OBJECTIVES: Anti-PL12 autoantibodies are targeted against alanyl-tRNA synthetase, a cytosolic enzyme which plays a vital role in protein synthesis. Clinically, such antibodies are strongly associated with interstitial lung disease and confer a poor prognosis. To better understand their molecular composition, anti-PL12 immunoglobulin variable region subfamily expression and their mutational signatures were analysed by mass spectrometry (MS)-based proteomics to provide insights into personalized molecular diagnosis. METHODS: Patients with anti-PL12 autoantibodies were identified from routine my

Integrative Dynamic Proteomics and Acyl-Proteomics Analyses in Clostridium butyricum.

Clostridium butyricum (C. butyricum) is a notable butyrate-producing intestinal probiotic that has been utilized for the enhancement and treatment of various intestinal and extraintestinal diseases. Butyrate and acetate, as significant metabolites of this bacterium, are short-chain fatty acids (SCFAs) that play crucial roles in maintaining intestinal stability and safeguarding host health. However, the regulatory functions of lysine butyrylation (Kbu) and acetylation (Kac), mediated by these metabolites, remain inadequately understood. In this study, we performed a comprehensive dynamic multio
