State calculations of cultural survival in environmental risk assessment: consequences for Alaska Natives.

In 2007, the Alaska Division of Public Health issued their first-ever fish consumption advisory to reduce exposure to methylmercury. Interestingly, they utilized a toxicity level in their calculations of risk that is four times higher than the U.S. Environmental Protection Agency's (EPA) level, arguing that the EPA's calculation is "inappropriately restrictive" for Alaskans. This article explores the institutional reasoning and scientific calculations behind the state's fish consumption advice, with special attention paid to the consequences for Alaska Natives. I argue that a discourse of "Ala

Creutzfeldt-Jakob disease among American Indians and Alaska Natives in the United States.

The occurrence of Creutzfeldt-Jakob disease (CJD) among American Indians and Alaska Natives in the United States was evaluated using national multiple cause-of-death data and medical information obtained from state health departments. Twelve CJD deaths were identified for 1981 through 2002, and the average annual age-adjusted death rate was 0.47 per million population. This rate was significantly lower than that for whites and similar to the rate for African Americans.

Botulism among Alaska Natives. The role of changing food preparation and consumption practices.

Alaska Natives have one of the highest rates of food-borne botulism worldwide. All outbreaks have been associated with the consumption of native foods, but in recent years outbreaks have occurred in previously unaffected areas and have involved new food items. Five botulism outbreaks occurred between 1975 and 1985 in an area of southwestern Alaska without previous confirmed outbreaks and among one ethnic group, the Yupik Eskimo. Of the 5 outbreaks, 3 were associated with fermented beaver tail, a nontraditional native food recently introduced into the region. Preparation techniques vary widely

Wy'east: An Innovative, Culturally Responsive Postbaccalaureate Pathway for American Indians and Alaska Natives.

PROBLEM: American Indians and Alaska Natives (AIANs) face significant health disparities that are exacerbated by limited access to high-quality, culturally congruent health care providers. There are no premedical postbaccalaureate programs focused on AIAN students. APPROACH: The Northwest Native American Center of Excellence designed the Wy'east Pathway in 2018 to increase the number of AIANs matriculating to U.S. medical schools by supporting those on the cusp of matriculation. Wy'east scholars undertake 10 months of structured programming to augment their academic preparation, improve their

Substance use disorder and homelessness among American Indians and Alaska Natives in California.

American Indian and Alaska Native (AIAN) communities have higher rates of substance use than other racial and ethnic groups. Substance use disorder (SUD) is tied to the increased risk of experiencing homelessness. National policies have also led to the disproportionate rates of homelessness among AIAN communities. However, specific experiences related to the occurrence of SUD and homelessness among AIAN in California, as well as seeking and accessing SUD treatment, are not well understood. This study explored potential SUD risk and resilience factors for AIANs experiencing homelessness and the

Patterns in cancer incidence among American Indians/Alaska Natives, United States, 1992-1999.

OBJECTIVE: Cancer is a major public health concern in American Indian and Alaska Native (AI/AN) communities. However, information on the incidence of cancer is lacking for this group. The purpose of this study is to report cancer incidence patterns for the U.S. AI/AN population. METHODS: Age-adjusted annual cancer incidence rates for 1992 through 1999 were calculated for 12 Surveillance, Epidemiology and End Results (SEER) areas, representing a sample (42%) of the U.S. AI/AN population. Trends in cancer incidence rates for the AI/AN sample were determined using standard linear regression of l

Prevalence and Incidence of Sjögren's Disease in Alaska Native and American Indian Peoples of Alaska.

OBJECTIVE: Our objective was to determine the prevalence, incidence, and clinical characteristics of Sjögren's disease (SjD) in Alaska Native and American Indian (AN/AI) peoples of Alaska. METHODS: We identified adults with SjD by querying electronic health records from participating tribal health organizations within the Alaska Tribal Health System (ATHS). Medical records were abstracted for adults with diagnostic codes for SjD. Individuals were included if they were diagnosed with SjD by a rheumatologist. Prevalence and incidence were calculated using the ATHS user population in 2019 (point

Endemic foodborne botulism among Alaska Native persons--Alaska, 1947-2007.

BACKGROUND: Foodborne botulism resulting from consumption of uncooked aquatic game foods has been an endemic hazard among Alaska Native populations for centuries. Our review was conducted to help target botulism prevention and response activities. METHODS: Records of Alaska botulism investigations for the period 1947-2007 were reviewed. We used the Centers for Disease Control and Prevention case definitions for foodborne botulism and linear regression to evaluate incidence trends and χ(2) or Fisher's Exact tests to evaluate categorical data. RESULTS: A total of 317 patients (61% of whom were

Research and educational approaches to reducing health disparities among American Indians and Alaska Natives.

Scimitar syndrome is a rare congenital anomaly characterized by anomalous pulmonary venous drainage to the inferior vena cava, causing a left-to-right shunt. Six patients with scimitar syndrome were diagnosed in our hospital between 2002 and 2008. There were 4 girls and 2 boys; 4 < 5 kg in weight, 2 < 8 kg in weight. Scimitar syndrome was suspected in 5 cases because of dextroversion, and diagnosed by color Doppler echocardiography in all 6 when a scimitar vein was detected entering the inferior vena cava. Computed tomography confirmed the diagnosis in all patients. Two patients had horseshoe

Peptic ulcer disease in the Alaska Natives: a four-year (1967-1971) retrospective study.

A four-year retrospective study of peptic ulcer disease in the Alaska Native confirmed our suspicion of a decreased ratio of duodenal to gastric ulcers. There were 92 patients with duo-denal ulcers, 75 with gastric ulcers, 6 with carcinoma of the stomach and 1 with gastric sarcoma. Ulcer disease in the Alaska Natives differs from ulcer disease among the Southwestern Indians as reported by Sievers.

Intersecting discourses on race and sexuality: compounded colonization among LGBTTQ American Indians/Alaska Natives.

This article examines discourses on race and sexuality in scientific literature during the late-nineteenth and early-twentieth centuries in context of U.S. settler colonialism. It uses a theoretical and methodological intersectional perspective to identify rhetorical strategies deployed in discursive representations salient to lesbian, gay, bisexual, transgender, Two-Spirit, and queer American Indians and Alaska Natives. These representations reflect a context of compounded colonization, a historical configuration of co-constituting discourses based on cultural and ideological assumptions that

Mortality of American Indian and Alaska native infants.

Accurate determination of infant mortality rates among Indians in seriously hampered by variations in the identification of Indian persons and use of different subsets of the Indian population for various purposes. Lack of consistency in the reporting of racial origin on birth and death records is a source of substantial error. Because of these factors, more than the usual care must attend comparisons and inferences drawn from data in which these differences are present. At present, it would seem prudent to regard all data about American Indians as provisional. Even though Indian infant mortal

Management of Type 2 Diabetes Mellitus in Native Americans and Alaska Natives: A Systematic Review.

American Indian and Alaska Native (AI/AN) populations face a disproportionately high prevalence of type 2 diabetes mellitus (T2DM) compared to other U.S. racial and ethnic groups. Despite this burden, limited literature exists on effective, culturally sensitive interventions, highlighting a critical need for focused research in this population. This systematic review examines 28 studies on how self-management, clinical management, and community-based programs improve glycemic control, medication adherence, and patient engagement in AI/AN populations. Clinical management reveals widespread use

Chromoblastomycosis, Phaeohyphomycotic Abscess, and Sporotrichosis Among American Indian/Alaska Natives, Indian Health Service, 2016-2022.

Chromoblastomycosis, phaeohyphomycotic abscesses, and sporotrichosis are fungal diseases usually caused by traumatic inoculation that cause substantial morbidity and can lead to long-term sequalae and inability to work. Few studies have analyzed the epidemiological and clinical characteristics of these mycoses in the United States, particularly among minority populations. We estimated prevalence and described features of American Indian and Alaska Native (AI/AN) persons with these diseases using Indian Health Service (IHS) data during 2016-2022. Chromoblastomycosis and phaeohyphomycotic absces

Future directions for medication assisted treatment for opioid use disorder with American Indian/Alaska Natives.

The U.S. is experiencing an alarming opioid epidemic, and although American Indians and Alaska Natives (AI/ANs) are especially hard hit, there is a paucity of opioid-related treatment research with these communities. AI/ANs are second only to Whites in the U.S. for overdose mortality. Thus, the National Institute on Drug Abuse convened a meeting of key stakeholders to elicit feedback on the acceptability and uptake of medication assisted treatment (MAT) for opioid use disorders (OUDs) among AI/ANs. Five themes from this one-day meeting emerged: 1) the mismatch between Western secular and reduc
