Systematic literature review on which maternal alcohol behaviours are related to fetal alcohol spectrum disorders (FASD).

OBJECTIVES: Fetal alcohol spectrum disorders (FASD) is a worldwide problem. Maternal alcohol consumption is an important risk factor for FASD. It remains unknown which alcohol consumption patterns most strongly predict FASD. The objective of this study was to identify these. DESIGN: Systematic literature review. METHODS: We searched in PubMed, PsychINFO, PsycARTICLES, ERIC, CINAHL, Embase and MEDLINE up to August 2018. The query consisted of keywords and their synonyms related to FASD, pregnancy and behaviour. Studies were excluded when not published in English, were reviews or involved non-

Latent dimensions of posttraumatic stress disorder and their relations with alcohol use disorder.

PURPOSE: The objective of this study was to evaluate the relationship between factors of posttraumatic stress disorder (PTSD) and alcohol use disorder (AUD) using confirmatory factor analysis (CFA) in order to further our understanding of the substantial comorbidity between these two disorders. METHODS: CFA was used to examine which factors of PTSD's dysphoria model were most related to AUD in a military sample. Ohio National Guard soldiers with a history of overseas deployment participated in the survey (n = 1215). Participants completed the PTSD Checklist and a 12-item survey from the Natio

Characterizing Alcohol-Related Neurodevelopmental Disorder: Prenatal Alcohol Exposure and the Spectrum of Outcomes.

BACKGROUND: The effects of prenatal alcohol exposure (PAE) are conceptualized as fetal alcohol spectrum disorder, with fetal alcohol syndrome (FAS) as the most severe. Many find it more difficult to characterize behavioral and cognitive effects of exposure on the central nervous system when physical signs are not present. In the current study, an operational definition of alcohol-related neurodevelopmental disorder (ARND) was examined to determine its usefulness in discrimination of children classified as ARND based on behavior (ARND/B) and cognition (ARND/C) from children in 4 contrast groups

Medications for Alcohol Use Disorder Among Birthing People With an Alcohol-related Diagnosis.

OBJECTIVES: Although safety and effectiveness of medications for alcohol use disorder (AUD) are well established for adults, literature on these medications in pregnancy is limited. Given known adverse effects of untreated AUD during pregnancy, clinicians and researchers have recently begun to call for reconsidering use of medications for AUD in pregnancy. Thus, we sought to estimate the proportion of birthing people with an alcohol-related diagnosis who received a prescription for medication related to AUD treatment. METHODS: Data were from Meritive MarketScan, a national private insurance c

Misconceptions, preferences and barriers to alcohol use disorder treatment in alcohol-related cirrhosis.

BACKGROUND: While alcohol cessation improves mortality in alcoholic liver disease (ALD), many patients struggle to achieve abstinence. Our aim was to characterize ALD patients' preferences, misconceptions, and barriers to alcohol use treatment options. METHODS: This mixed-methods study included outpatients with a history of alcohol-related cirrhosis or alcoholic hepatitis recruited from a hepatology clinic for a survey or an in-depth semi-structured interview. We purposefully sampled men and women, compensated and decompensated patients to ensure adequate representation of gender and severity

Calcitonin Gene-Related Peptide Signalling in Alcohol Use Disorders: A Scoping Review.

ISSUE: Alcohol use disorders (AUD) represent a major global health burden, and currently available pharmacological treatments show limited and heterogeneous efficacy. Calcitonin gene-related peptide (CGRP) is a pleiotropic neuropeptide involved in stress regulation and neuroimmune signalling, and is an established therapeutic target in migraine. CGRP signalling may also be involved in alcohol-related mechanisms, raising interest in its potential relevance for AUDs. APPROACH: We conducted a scoping review of original studies investigating CGRP and CGRP-related pathways in the context of alcoho

Panic attacks and related disorders in alcohol-dependent, depressed, and nonclinical samples.

Although several studies have indicated that a substantial portion of alcoholics have an anxiety disorder, relatively little information exists specifically regarding panic disorders. In addition, prior studies have been marred by the absence of appropriate contrast groups. The present investigation compared the lifetime prevalence of panic attacks and panic-related disorders diagnosed according to DSM-III criteria in a group of 79 alcohol-dependent patients, 64 depressed patients, and 70 nonclinical subjects. Panic attacks, panic disorder, and agoraphobia with panic attacks were more prevalen

Disordered Bile Acid Metabolism in Alcohol-Related Hepatitis.

BACKGROUND: Alcohol-related hepatitis (AH) is characterised by acute cholestasis and liver dysfunction in patients consuming alcohol. AIMS: To define the bile acid (BA) profile in AH compared to decompensated alcohol-related cirrhosis (DC) and healthy controls (HC). METHODS: Serum and faecal BAs were measured by UHPLC-MS; FGF19 by ELISA; RNA-sequencing data obtained from liver biopsies; serum cytokines and growth factors quantified by multiplex immunoassay. Hepatocyte growth factor (HGF) was applied to primary human hepatocytes (PHH) and BA transporter expression was assessed by RT-qPCR. RE

Hippocampal abnormalities in youth with alcohol-related neurodevelopmental disorder.

Individuals diagnosed with alcohol-related neurodevelopmental disorder (ARND) exhibit difficulty on hippocampally mediated memory tasks and show reduced hippocampal size. However inconsistencies exist regarding the affected memory functions and where within the hippocampi effects occur. Given recent studies showing anterior and posterior segments support distinct memory functions and sex dimorphisms in hippocampal function, we asked whether these factors influence memory performance in youth with ARND (n = 18) and typically developing controls (n = 17). Participants received a battery of memor

Cortical morphology in children with alcohol-related neurodevelopmental disorder.

INTRODUCTION: It is well established that individuals exposed to alcohol in utero have reduced cortical grey matter volumes. However, the candidate determinants of these reductions, cortical thickness (CT) and surface area (SA), have not been investigated exclusively in alcohol-related neurodevelopmental disorder (ARND), the most prevalent fetal alcohol spectrum disorder subgroup that lacks the characteristic facial dysmorphology. METHODS: T1-weighted magnetic resonance imaging scans were obtained from 88 participants (8-16 years), 36 diagnosed with ARND and 52 typically developing controls.

Medications for Alcohol Use Disorder Among Patients With Severe Alcohol-Related Liver Disease.

IMPORTANCE: Alcohol-related liver disease (ALD) is the leading indication for liver transplant in the US; however, use of medications for alcohol use disorder (MAUDs) remains very low overall. Few studies have examined the use of MAUDs for patients with severe ALD, and lack of clinical knowledge around the benefits of MAUDs for severe ALD remains a barrier to their use. OBJECTIVE: To assess the association between use of MAUDs and mortality among patients with severe ALD referred for liver transplant. DESIGN, SETTING, AND PARTICIPANTS: This retrospective single-center cohort study was conduc

Pharmacological Treatments in Alcohol Use Disorder and Risk of Alcohol-Related Hospitalizations: A Register Study.

OBJECTIVES: Despite the high prevalence of alcohol use disorder (AUD), only a minority of patients receive recommended pharmacological treatments, possibly owing to uncertainty about the real-world effectiveness of these medications. Here, we analyzed nationwide, register-based data to investigate the association between approved AUD medications (naltrexone, acamprosate, disulfiram, and nalmefene) and the risk of alcohol-related hospitalizations among individuals with AUD. METHODS: People aged 18-64 with a registered first-time diagnosis of AUD between 2009 and 2019 (N = 93,727) were identifi

Alcohol Use Disorder Pharmacotherapy in Patients With Alcohol-Related Liver Disease: A Scoping Review.

Introduction: Alcohol-associated liver disease (ALD) is one of the most common causes of cirrhosis. Pharmacotherapy for alcohol use disorder (AUD) can improve abstinence rates in patients with cirrhosis, however, there is limited data on how these therapies affect liver-related outcomes. Methods: A scoping review was completed using multiple electronic search databases. Articles exploring pharmacotherapy for AUD and outcomes for ALD were included. The primary outcome of this study was liver outcomes after receiving pharmacotherapy for AUD, including decompensated cirrhosis, mortality, progress

Diagnostic validity of alcohol demand and monetary delay discounting in relation to alcohol use disorder.

BACKGROUND: A reinforcer pathology approach to alcohol use disorder (AUD) proposes that high alcohol reinforcing value (high alcohol demand) and overvaluation of immediate rewards (high discounting of future rewards) are critical determinants of problematic drinking. Applied to clinical settings, these indicators have not been examined as potential assessments for use in clinical practice. Toward that end, the current study examined whether reinforcer pathology indicators accurately classify individuals with AUD from recreational drinkers without AUD at levels that would meet clinical accuracy

Alcohol demand, delayed reward discounting, and craving in relation to drinking and alcohol use disorders.

A behavioral economic approach to alcohol use disorders (AUDs) emphasizes both individual and environmental determinants of alcohol use. The current study examined individual differences in alcohol demand (i.e., motivation for alcohol under escalating conditions of price) and delayed reward discounting (i.e., preference for immediate small rewards compared to delayed larger rewards) in 61 heavy drinkers (62% with an AUD). In addition, based on theoretical accounts that emphasize the role of craving in reward valuation and preferences for immediate rewards, craving for alcohol was also examined

Evaluating creatine kinase as a biomarker in alcohol-related disorders: promise and constraints.

Alcohol use contributes substantially to global morbidity and mortality, necessitating reliable biomarkers for detection and monitoring. Existing biomarkers, including indirect markers and direct markers, have important limitations related to specificity, sensitivity and temporal detection windows. As a biomarker, creatine kinase (CK) demonstrates strong biological plausibility, as alcohol exerts direct and indirect myotoxic effects through oxidative stress, impaired protein synthesis and metabolic disruption, leading to muscle injury and enzyme leakage. However, its diagnostic specificity is

Behavioural cue reactivity to alcohol-related and non-alcohol-related stimuli among individuals with alcohol use disorder: An fMRI study with a visual task.

Patients with alcohol use disorder (AUD) have difficulty controlling their alcohol cravings and thus exhibit increased use and early relapse. Although patients tend to respond more strongly to alcohol-related images than to non-alcohol-related images, few researchers have examined the factors that modulate cravings. Here, we examined whole-brain blood oxygen level-dependent (BOLD) responses to behavioural cues in individuals with AUD and in healthy controls (HCs). The participants included 24 patients with AUD and 15 HCs. We presented visual cues consisting of four beverage-related images (jui
