Structural anatomy of pure and hemianopic alexia.

BACKGROUND: The two most common types of acquired reading disorder resulting from damage to the territory of the dominant posterior cerebral artery are hemianopic and pure alexia. Patients with pronounced hemianopic alexia have a right homonymous hemianopia that encroaches into central or parafoveal vision; they read individual words well, but generate inefficient reading saccades when reading along a line of text. Patients with pure alexia also often have a hemianopia but are more disabled, making frequent errors on individual words; they have sustained damage to a brain region that supports

Reading in pure alexia. The effect of strategy.

A number of investigators have demonstrated that patients with pure alexia comprehend briefly presented words which they are unable to explicitly identify. We suggested previously that these patients may read by means of two distinct procedures: a laborious letter-by-letter method and a 'whole-word' procedure which, at least initially, does not support explicit word identification. We report a test of this proposal in a patient with pure alexia. We reasoned that if the patient had access to two distinct and incompatible procedures, he might be induced to switch from one to the other by changin

Evidence for preserved reading in 'pure alexia'.

We describe 4 patients who developed pure alexia after infarctions of the left cerebral hemisphere. All subjects employed a letter-by-letter strategy (with varying degrees of success) to explicitly identify visually presented words. Although all 4 subjects explicitly denied that they could identify briefly presented words, they all performed significantly better than chance on lexical decision and forced-choice semantic categorization tasks with briefly presented words which they could not explicitly identify. Three subjects regained the ability to explicitly identify briefly presented words;

Visual complexity in letter-by-letter reading: "pure" alexia is not pure.

Standard accounts of pure alexia have favoured the view that this acquired disorder of reading arises from damage to a left posterior occipital cortex mechanism dedicated to the processing of alphanumeric symbols. We challenge these accounts in two experiments and demonstrate that patients with this reading deficit are also impaired at object identification. In the first experiment, we show that a single subject, EL, who shows all the hallmark features of pure alexia, is impaired at picture identification across a large set of stimuli. As the visual complexity of pictures increases, so EL's re

Pure alexia from a posterior occipital lesion.

The authors report a patient with pure alexia (letter-by-letter reading) selectively impaired for kana (Japanese phonograms), cerebral achromatopsia, and right lower quadrantanopsia after hemorrhage in the left posterior occipital lobe, mainly under the lateral occipital gyri. The patient also could not recognize some single-character kana, nor could he discriminate between two shapes of a similar size. The authors believe that the posterior occipital lobe, including the lateral occipital gyri, is specialized to recognize kana characters in this patient.

Jules Dejerine and his interpretation of pure alexia.

Dejerine's interpretation of pure alexia is routinely mentioned in all neuropsychological textbooks, yet the details of his account and the evidence on which it is based have never been subjected to a critical analysis. We provide such an evaluation in this paper, summarizing the behavioral data that Dejerine presented in his now famous case report and the theoretical framework he adopted to explain the phenomenon of alexia without agraphia. We also provide a link between Dejerine's work and current hypotheses on the nature of the syndrome.

Visual command hallucinations in a patient with pure alexia.

Around 25% of patients with visual hallucinations secondary to eye disease report hallucinations of text. The hallucinated text conveys little if any meaning, typically consisting of individual letters, words, or nonsense letter strings (orthographic hallucinations). A patient is described with textual visual hallucinations of a very different linguistic content following bilateral occipito-temporal infarcts. The hallucinations consisted of grammatically correct, meaningful written sentences or phrases, often in the second person and with a threatening and command-like nature (syntacto-semanti

Single-character processing in a case of pure alexia.

The processing of single characters in a pure alexic patient was studied in an attempt to identify the impairment responsible for his reading disorder. Observations from Experiments 1 to 4 suggested a deficit of identification of alphanumeric stimuli without any impairment affecting the elaboration of a structural description of visual stimulation. Experiment 5 indicated that the identification disorder results from a defect in the selective processes--activation and/or inhibition--that must come into play for achieving an appropriate match between a structural description of the stimulation a

The evolution of pure alexia: a longitudinal study of recovery.

This case report documents the partial recovery, over a 12-month period, of pure alexia in an adult female following a left occipital infarction. Measures of speed and accuracy were obtained on an oral reading and a lexical decision task immediately postonset and then on 10 subsequent occasions. Explicit letter-by-letter reading was observed only during the first week poststroke but a significant effect of word length was seen in all testing sessions. Reading accuracy was relatively good at all stages and reading latency showed a remarkable decrease over time but did not reach normal reading r

Visual crowding in pure alexia and acquired prosopagnosia.

Visual crowding is a phenomenon that impairs object recognition when the features of an object are positioned too closely together. Crowding limits recognition in normal peripheral vision and it has been suggested to be the core deficit in visual agnosia, leading to a domain-general deficit in object recognition. Using a recently developed tool, we test whether crowding is the underlying deficit in four patients with category specific agnosias: Two with pure alexia and two with acquired prosopagnosia. We expected all patients to show abnormal crowding. We find that the two patients with acquir

Visual processing in pure alexia: a case study.

Whether pure alexia is a selective disorder that affects reading only, or if it reflects a more general visual disturbance, is highly debated. We have investigated the selectivity of visual deficits in a pure alexic patient (NN) using a combination of psychophysical measures, mathematical modelling and more standard experimental paradigms. NN's naming and categorization of line drawings were normal with regards to both errors and reaction times (RTs). Psychophysical experiments revealed that NN's recognition of single letters at fixation was clearly impaired, and recognition of single digits w

Large acute cerebral hemorrhage presenting with pure alexia.

Alexia (the acquired inability to read) is an uncommon presenting complaint in the emergency department (ED). It is usually associated with a lesion in the brain located within the dominant hemisphere near the parieto-occipital junction, with some involvement of the splenium of the corpus callosum. Our review of the literature revealed multiple distinct causes for the finding of alexia, and a majority of the cases uncovered also involved agraphia (the acquired inability to write) and frequently visual field defects. We present the case of an otherwise healthy 35-year-old white man who arrived

[Pure alexia due to a fusiform gyrus lesion].

We present a patient with pure alexia following a hemorrhagic infarction in the left fusiform gyrus. The symptom began with alexia preferentially disturbed for kana, but during the course of recovery slight agraphia for kanji became pronounced. In the earlier phase, alexia was more severe than agraphia and he could write kanji that he could not read. Furthermore, kinesthetic reading was effective. These findings are consistent with the symptoms of pure alexia, although the fact that a writing disturbance for kanji persisted resembles the profile of alexia with agraphia for kanji due to a poste

Pure somaesthetic alexia: somaesthetic-verbal disconnection for letters.

We studied a patient who manifested a bilateral reading disorder through the somaesthetic modality, without deficit of elementary tactile sensation or tactile object naming, due to a left parietal infarct. Detailed investigation established the following points. (i) The patient showed normal function on elementary somaesthetic examination, normal function on high level tactile perception, except for minimal impairment of the right hand on the two-point discrimination test, and normal latencies on the somatosensory evoked potential in both hands. (ii) The patient had difficulty in reading lette

Pure alexia and right hemiachromatopsia in posterior dementia.

A 66 year old, right handed woman presented with pure alexia and right hemiachromatopsia (PARH) in the context of a posterior dementia. PARH was accompanied by prosopagnosia, 2-D object agnosia, and environmental agnosia. Visual fields were normal to confrontation testing. The pathological anatomy of PARH involves circumscribed damage to the lingual and fusiform gyri and paraventricular white matter of the left occipital cortex, two contiguous cortical regions functionally specialised for processing colour and word form information, respectively.
