This review deals with the different methods of lengthening the clinical crown and discusses the advantages and/or disadvantages of each method. Different flap procedures, including ostectomy and osteoplasty, reduce the height of the alveolar crest aiming at a distance of about 3 mm to a future reconstruction margin. Root planing may avoid reattachment of surgically separated fibers. Other methods like gingivectomy, electrosurgery, intra-alveolar transplantation, and forced eruption might contain considerable biological disadvantages. However, if the extrusion is combined with regular fiberoto

When intraoral access is adequate and other physical characteristics are satisfactory, total mandibular alveolar osteotomy can be used effectively to correct skeletal (infantile) apertognathia. The procedure, not frequently used, resulted in significant improvement of a massive open bite in a 15-year-old girl.

Management of an impacted canine often leads to an inadequate width of attached gingiva, which can be a possible risk for future gingival recession and associated complications. Uncovering a labially impacted maxillary canine can be performed by gingivectomy, apically positioned flap surgery, or a closed eruption technique. Choosing the right technique is sometimes confusing. The authors present a case that was managed by apically positioned flap surgery followed by orthodontic treatment.

OBJECTIVE: The purpose of this study was to answer the following two questions: (1) Can hydroxyapatite cement in combination with demineralized freeze dried bone feasibly augment the dimension of an atrophic edentulous canine mandible? (2) What is the histologic fate of an augmentation graft composed of hydroxyapatite cement and demineralized freeze dried bone placed on the surface of an atrophic edentulous canine mandible?
STUDY DESIGN: Each of four mixed-breed canines (weighing 50 to 60 pounds) underwent bilateral mandibular dental extraction (canine to second molar) and radical alveolectom

The aim of this article was to present the treatment of an unesthetic smile in a 27-year-old woman showing gingival recession, gingival asymmetry, and an altered passive eruption in the natural dentition. First, an apically positioned flap with osseous resective surgery was applied to correct the gingival asymmetry at the maxillary right incisors. The amount of bone resection was determined by means of bone sounding at the maxillary left incisors. Six weeks later, a coronally advanced flap procedure was performed to cover the gingival recession at the maxillary right canine, along with an enam

OBJECTIVE: To review cases of bisphosphonate-related osteonecrosis of the jaw (BRONJ) occurring in association with benign disease and to describe and compare the clinical course and outcome for patients with BRONJ and rheumatoid arthritis (RA) or osteoporosis.
METHODS: We retrospectively reviewed observations of all patients referred for treatment and followup for BRONJ from January 2007 to December 2011. Only patients with malignant disease were excluded. Demographic data, medical history, maxillofacial findings, BRONJ treatment, and followup were reviewed for each case.
RESULTS: Over a 5-

Two examples are presented of osteotomies performed simultaneously with surgical recontouring in cases of fibrous dysplasia of the jaws. Osteotomies are necessary when a coexistent discrepancy in jaw relationship is present. This provides a functional as well as aesthetic result. Rigid miniplate fixation was utilised, and dysplastic bone healing was uneventful, and found clinically not to differ from that of normal bone. Osteotomy is thus feasible in dysplastic bone, and may complement recontouring in achieving a better result.

Spindle cell malignancies of the oral cavity are uncommon and pose considerable diagnostic challenges due to overlapping histomorphology and immunohistochemical features. Malignant peripheral nerve sheath tumor (MPNST) with heterologous differentiation is particularly rare. Moreover, its coexistence with a synchronous hematological malignancy such as small cell lymphoma (SLL) is exceedingly unusual. A male 62 year, presented with a progressively enlarging, pigmented, ulcero-proliferative painless mass over the right upper alveolus extending to the hard palate. Imaging revealed a heterogeneousl

Malignant tumours of the upper gum and hard palate account for 1-5% of malignant neoplasms of the oral cavity; two thirds of the lesions which involve these areas are squamous cell carcinomas. Most of these carcinomas are diagnosed late, when they invade the underlying bone. The procedures of choice for removal are: alveolectomy, palatectomy, maxillectomy, which may be total or partial. Surgical reconstruction of the defect may be carried out using a wide range of microvascularized flaps: osteomuscolocutaneous of the internal iliac crest, an osteocutaneous flap of the fibula or scapula, fascia

Damage to the lingual nerve, resulting in transient or permanent paraesthesia or anaesthesia, is a common undesirable complication of surgical interventions to the lower third molar region. The anatomy of the nerve, as it travels from its origin high in the infra-temporal fossa, to the floor of the mouth is quite variable. The most critical part of its course is where it enters the sublingual region just alongside the lingual alveolar plate of the lower third molar. A significant number of lingual nerves are located above the alveolar bone in the gingival tissues, or very close to the bone. Re

A review of twenty-five patients (nine boys and sixteen girls with a mean age of 13.6 years) with infraclusion of primary molars is reported. There were thirty-two infracluded teeth, twenty-one (65.6 percent) maxillary primary molars and eleven (34.4 percent) mandibular primary molars. The primary first molar was found to be in infraclusion in four (12.5 percent) cases and the primary second molar in twenty-eight (87.5 percent) cases. Submergence was mild in fifteen cases, moderate in five, and severe in twelve. The permanent successor was lacking in four (12.5 percent) infracluded teeth. Apla

UNLABELLED: Bisphosphonate-related osteonecrosis of the jaw (BRONJ) is a serious side effect of bisphosphonate therapy. The incidence of BRONJ is known to be low among patients treated with oral bisphosphonates. We investigated the prevalence, demographics, clinical manifestations, and treatment outcome of 24 patients with oral BRONJ in Asian populations.
INTRODUCTION: The long-term safety of oral bisphosphonates is clinically important considering the rare but potentially serious complications such as bisphosphonate-related osteonecrosis of the jaw (BRONJ) versus the effect of reducing and p

Transverse and oblique intra-alveolar fractures constitute a major therapeutic problem, particularly in young dentitions. Very few treatment alternatives apart from extraction have been available. This report is a clinical and roentgenological evaluation of a series of intra-alveolar transplantations which were carried out on 56 teeth with complicated crown-root fractures. The patients ranged in age from 9-33 years and were observed over a period of 4 years. Of the 8 teeth which had to be extracted due to further traumatic/prosthetic complications, 7 were retained for histologic examination. A

Impacted teeth present many problems for the orthodontist. They can compromise tooth movement, esthetics, and functional outcomes. The second most commonly impacted tooth, after the maxillary third molar, is the maxillary canine, with an incidence from 1% to 2.5%. Maxillary canines can be impacted facially or palatally and are more common in female patients than in male patients. Therefore, the purpose of this paper is to address the incidence and etiology associated with impacted maxillary canines, the clinical and radiographic evaluation of the situation, and the techniques for managing this

Surgery for cancer of the floor of the mouth often results in alteration of the muscles of the tongue and floor of the mouth. Both primary and secondary surgical procedures often result in scar formation with reduced mobility of the tongue during speech and deglutition. Speech is often used as a diagnostic tool in the placement of the anterior teeth during fabrication of a prosthesis. Speech can similarly be used to help determine the proper placement of a speech portion of the prosthesis. The prosthetic rehabilitation approach described lowers the palatal vault with a false palate to enable t

A case of peripheral ameloblastoma in a 57-years-old woman is presented, along with a discussion of the clinical and histological characteristics of the lesion. After clinical and radiographic examinations, and with a differential diagnosis of pyogenic granuloma, an excisional biopsy was performed and the material collected was sent for histological examination. On the basis of the histopathological diagnosis, a second operation was performed with a wide safety margin, including bone tissue, which did not show any involvement with the odontogenic neoplasm.
