Heart Failure Transitional Care and Ambulatory Monitoring.

Chronic heart failure is characterized by periods of stability punctuated by clinical decline often requiring hospitalization. Following hospitalization, patients remain at an increased risk of morbidity and mortality, often requiring rehospitalization due to recurrent deterioration. Transitional care interventions aim to improve the movement from the inpatient to ambulatory setting. Promising interventions, such as patient education, novel uses of loop diuretics, and invasive hemodynamic monitoring, have been developed over the past 20 years all with varying degrees of success.

Ambulatory blood pressure monitoring in primary care.

The accurate assessment and appropriate management of blood pressure (BP) is critical for primary care physicians. Traditional office-based BP measurement has limitations that can be addressed, in part, through the use of ambulatory BP monitoring (ABPM). Because BP readings are provided at specific time intervals throughout a 24-hour period, ABPM represents a better picture of the normal fluctuations in BP levels associated with daily activities, including sleep. Blood pressure values obtained by 24-hour ABPM are a better predictor of cardiovascular risk than office-based BP measurements, and

[What is the care path for a patient in ambulatory surgery?].

The inmate is a special patient. Indeed, his status requires a specific organization. The Versailles hospital centre (78) is the reference centre for the Yvelines prison and the women's prison. The majority of interventions for detained patients are performed on an outpatient basis.

The role of ambulatory BP monitoring in clinical care.

Hypertension (HTN) is a powerful predictor of cardiovascular disease and death, with treatment reducing these risks. Ambulatory BP is a better predictor of target-organ damage and cardiovascular events than office BP (OBP). Clinically, ambulatory BP monitoring (ABPM) can be used to detect white-coat hypertension (WCH) and masked HTN and to more accurately determine whether BP is appropriately controlled in patients with established HTN. This is particularly valuable in the elderly, in whom BP is more variable. ABPM can also identify abnormal circadian rhythm of BP, which is more common in the

[The ambulatory care of patients with precancerous diseases of the stomach].

Two hundred and ninety-four patients with precancerous lesions of the stomach were followed and treated. The group included cases of gastric ulcer (101), polyps (78), atrophic-hyperplastic gastritis (81) and 34 patients who had undergone gastric resection for duodenal ulcer. Pathologic foci were surrounded by areas of epithelial dysplasia of varying degree in almost all patients. In the course of follow-up and treatment, dysplasia regressed in several cases and progressed to early cancer of the stomach in three. Recommendations for periodicity of check-ups of patients with chronic cancer of th

The role of ambulatory medical care in hypertension screening.

Data from the 1975-1976 National Ambulatory Medical Care Survey conducted by the National Center for Health Statistics were examined retrospectively to determine the extent to which blood pressure was measured during visits to office-based physicians in the conterminous United States. Blood pressure was more often measured for females (especially black) than for males although males (especially black) in certain age groups had a higher prevalence of hypertension and comprised the higher proportion of undiagnosed hypertensives. Blood pressure measurement increased with age, but was rarely measu

Transport Access for Ambulatory Care for Older People.

AIM: Lack of transport has been shown to be a key barrier to accessing healthcare, with poorer associated health outcomes in those experiencing transport poverty. Our health survey aimed to assess the transportation needs of a population attending a geriatric ambulatory care unit and the role of transport as a barrier to healthcare attainment. METHODS: Data collection was via survey; the survey used was adapted from the Irish Translational Study on Aging (TILDA) and the questionnaire devised by Locatelli, Sharp, Syed, Bhansari, and Gerber in their research on urban transport barriers to healt…

Consensus guidelines for ambulatory care for neutropenic fever.

These guidelines provide updated recommendations on ambulatory care for low-risk neutropenic fever in adult and paediatric patients. National and international data support the clinical, economic and psychosocial benefits of risk stratification, and home-based or reduced intensity treatment of neutropenic fever. Benefits include shortened in-hospital length of stay, reduced healthcare costs, early intravenous to oral switch and improved patient and caregiver quality of life. Since publication of the 2011 Australian consensus guidelines, additional adult and paediatric neutropenic fever clinica…

Ambulatory Assessment in Concussion Clinical Care and Rehabilitation.

Concussion is a mild traumatic brain injury that is characterized by a wide range of physical, emotional, and cognitive symptoms as well as neurocognitive, vestibular, and ocular impairments that can negatively affect daily functioning and quality of life. Clinical consensus statements recommend a targeted, clinical profile-based approach for management and treatment. This approach requires that clinicians utilize information obtained via a clinical interview and a multi-domain assessment battery to identify clinical profile(s) (e.g., vestibular, mood/anxiety, ocular, migraine, cognitive fatig

[Innovations in ambulatory care: screening, treatment and iatrogenicity].

This year we have selected six studies useful for the day to day practice. A mammography in women 40 to 49 years of age should be evaluated taking into account the patient's profile and the possible risks and benefits of this exam. In patients over 65 years of age, a systematic atrial fibrillation screening, with pulse rate measuring then ECG (if irregular beat) should be realised on a regular basis. The risks for complications following colonoscopies do exist, especially after biopsies/polypectomies and this risk should be discussed. Long term proton pump inhibitor treatment is a risk factor
