Wound shock: a history of its study and treatment by military surgeons.

The treatment of wounds has received considerable attention from the time of the Trojan War. However, it was not until the American Civil War that shock was described as an entity distinct from the wounds themselves and that efforts were directed at more than just treatment of the wound. The need for fluid resuscitation in the treatment of hemorrhagic shock was first recognized in the Spanish American War, as was the association of sepsis with shock. World War I showed the need for blood in the treatment of "wound shock," a lesson that had to be relearned in World War II through bitter experie…

Response to Brinkmann et al. "Re-assembly of 19th century smallpox vaccine genomes reveals the contemporaneous use of horsepox and horsepox-related viruses in the United States".

We thank Brinkmann and colleagues for their correspondence and their further investigation into these American Civil War Era vaccination strains. Here, we summarize the difficulties and caveats of work with ancient DNA.

Disrupting the Reproductive Lives of Japanese American Families During Wartime: The Overlapping Histories of Removal, Incarceration, and Eugenic Sterilization, 1942-1946.

The World War II mass incarceration of 120,000 Japanese Americans disrupted families, enacted intergenerational trauma, and characterized Japanese Americans as inherent racial threats. This federal exercise in biopolitical management intersected with state-sponsored eugenic sterilization in California institutions. Linking War Relocation Authority and California Department of Institutions records, we document 32 Japanese American sterilization survivors during the wartime incarceration period (1942-1946). We show how federal and state authority converged to deprive Japanese Americans of civil

The role of gender, phrenology, discrimination and nervous prostration in Clara Barton's career.

Clara Barton was 39 years old before she became involved in Civil War humanitarian activities. Prior to that time, three factors shaped her personality and her future: phrenology gave her philosophical principles to live by, especially "Know Thyself"; sex discrimination on her first two jobs steeled her for living in a male dominated Victorian era; and psychohygienic therapy for her long term nervous prostration changed her behavioral approach to illness and to life. With these three influences in hand, Barton went on to gain world-wide adulation. During the Civil War, she was revered as the A

Case studies in pathology from the National Museum of Health and Medicine, Armed Forces Institute of Pathology.

The National Museum of Health and Medicine was founded as the Army Medical Museum during the American Civil War to document the effects of war wounds and disease on the human body. Since then, the Museum has created a collection of documented pathologic specimens that can be used to study the gross and microscopic appearance of disease conditions. The Museum's collections are a vital link to the past and the future of medical research and form a unique national medical repository that is used continuously for research, education, and exhibit purposes. Today, the Museum in association with its …

A stranger to our camps: Typhus in American history.

Medical observers during the American Civil War were happily surprised to find that typhus fever rarely made an appearance, and was not a major killer in the prisoner-of-war camps where the crowded, filthy, and malnourished populations appeared to offer an ideal breeding ground for the disease. Through a review of apparent typhus outbreaks in America north of the Mexican border, this article argues that typhus fever rarely if ever extended to the established populations of the United States, even when imported on immigrant ships into densely populated and unsanitary slums. It suggests that som…

Jacob Mendez DaCosta: medical teacher, clinician, and clinical investigator.

J. M. DaCosta, a scholarly, well-trained and observant clinician, was recognized during his lifetime as a well-known authority on physical diagnosis and had an unexcelled reputation as a clinical teacher. Chairman of Medicine at the Jefferson Medical College for 19 years, president of the College of Physicians of Philadelphia in 1884 and again in 1895, he was one of the original members of the Association of American Physicians and its president in 1897. Earlier in his career, his extensive Civil War study of "a form of cardiac malady common among soldiers . . . the study of which is equally i

Epidemiological Isolation as an Infection Mortality Risk Factor in U.S. Soldiers from Late Nineteenth to Early Twentieth Centuries.

It remains uncertain why most infectious disease mortalities disappeared before modern medical interventions. Historical epidemiology using prospectively collected U.S. Army data from the Civil War (1860-1861), Spanish-American War (1898-1899), and First World War (1917-1918) suggests that epidemiological isolation was a major mortality risk factor for soldiers. Morbidity and mortality due to common infections decreased progressively from 1860 to 1918, except for influenza during the 1918 pandemic. Adult measles or mumps infections are indicative of isolated rural populations and correlated wi

Urology in pre-civil war Charleston.

PURPOSE: With a history spanning more than 3 centuries, Charleston, South Carolina was one of the initial locations of urological teaching in the southern United States. The Medical University of South Carolina was chartered in 1823 and is the oldest medical school in the South. We reviewed the historical archives of the Waring Library of the Medical University of South Carolina, specifically the history of urological practice in the city, including doctoral dissertations from medical school students regarding the teaching and practice of urology in pre-Civil War Charleston, to better understa

Orthostatic panic attacks among Vietnamese refugees.

Viewed historically and cross-culturally, orthostatic-induced dizziness, i.e., dizziness caused by standing up from a sitting or a lying position, forms a key aspect of many syndromes: irritable heart (American Civil War), effort syndrome (World War I and World War II), chronic fatigue syndrome (contemporary USA), Gulf War syndrome (contemporary USA), and orthostatic dysregulation (contemporary Japan). Among Vietnamese refugees attending a psychiatric clinic, this study documents a high rate of orthostatic panic (OP), as well as certain processes seemingly generating these panic attacks, viz.,…

Review of Treatment of Gunshot Wounds to Head in Late 19th Century.

INTRODUCTION: During the late 19th century, the seeds of modern neurosurgery were planted to bloom into what it is now known. Wars such as the American Civil War and Crimean War drove the need to find better ways of preventing mortality from gunshot wounds to the head. However, the mortality rate from all major surgical procedures to the head, neck, and face remained staggering. Herein, we describe the surgical treatments for head and neck injuries in order to improve our understanding of neurosurgical procedures performed during the late 19th century. METHODS: A literature search was conduct…

Suicide, alcoholism, and psychiatric illness among union forces during the U.S. Civil War.

Little is known about post-combat psychological reactions of warriors prior to the Twentieth Century. We estimated rates of suicide, alcohol abuse, and probable psychiatric illness among Union Forces during the U.S. Civil War via examination of data compiled by the Union Army. White active-duty military personnel suicide rates ranged from 8.74 to 14.54 per 100,000 during the war, and surged to 30.4 the year after the war. For blacks, rates ranged from 17.7 in the first year of their entry into the war (1863), to 0 in their second year, and 1.8 in the year after the war. Rates for most other re

In praise of anesthesia: Two case studies of pain and suffering during major surgical procedures with and without anesthesia in the United States Civil War-1861-65.

Background The United States Civil War (1861-1865) pitted the more populous industrialized North (Union) against the mainly agricultural slaveholding South (Confederacy). This conflict cost an enormous number of lives, with recent estimates mentioning a total mortality greater than 700,000 combatants [1]. Although sulfuric ether (ETH) and chloroform (CHL) were available since Morton's use of the former in 1846 and the employment of the latter in 1847, and even though inhalational agents were used in Crimean war (1853-1856) and the Mexican-American War (1846-1848), the United States Civil War g

The chief seat of mischief: soldier's heart in the First World War.

Soldier's heart was a medico-psychiatric condition that was first documented during the American Civil War. This condition affected British and American soldiers during the late-nineteenth and early-twentieth centuries; doctors recorded patients experiencing palpitations, breathlessness, headaches, and praecordial pain among other symptoms. While the number of cases of this disorder reached its peak in the First World War, it disappeared shortly afterwards. Based on an analysis of experimental results published in generalist and specialized medical journals as well as the correspondence betwee…

[Wars in the history of rheumatology].

Some important discoveries in the history of rheumatology happened during war periods. It is well known that arthritis associated with conjunctivitis and urethritis, following dysenteric episodes, has been described during the First World War from the German Hans Reiter and, nearly contemporarily, from the French Nöel Fiessinger and Edgar Leroy. Less known is instead the fact that the first cases of sympathetic algoneurodystrophy have been reported by the American Silas Weir Mitchell in soldiers wounded by fire-arms, during the Civil War of Secession. Other war episodes have been crucial for t

"Harmonies of Form and Color": Race and the Prosthetic Body in Civil War America.

This article considers the cultural meanings of Civil War injury, particularly amputation, with regard to race. Putting elements of print and visual culture in conversation with the material history of prostheses, the article explores competing understandings of war-acquired disability as a unifying category that could cross the color line or, alternatively, as a site of (white) racial distinction. While a number of depictions of "Empty Sleeves" in the Northern press in the early years following the war depicted black veterans' battle injuries as proof of their fitness for citizenship, represe

Abraham Lincoln's Gettysburg illness.

When Abraham Lincoln delivered the Gettysburg Address, he was weak and dizzy; his face had a ghastly colour. That evening on the train to Washington, DC, he was febrile and weak, and suffered severe headaches. The symptoms continued; back pains developed. On the fourth day of the illness, a widespread scarlet rash appeared that soon became vesicular. By the tenth day, the lesions itched and peeled. The illness lasted three weeks. The final diagnosis, a touch of varioloid, was an old name for smallpox that was later used in the 20th century to denote mild smallpox in a partially immune individu

Albert Sidney Johnston's sciatic dueling injury did not contribute to his death at the Battle of Shiloh.

OBJECTIVE: To determine whether General Albert Sidney Johnston's dueling wound and nerve injury (1837) contributed to his death at the Battle of Shiloh (1862). General A.S. Johnston was commander of the Confederate Army at Shiloh and was killed by a bullet that severed his right popliteal artery. The location of this wound in the popliteal fossa region was largely unnoticed and, consequently, was not treated expeditiously. It has been widely assumed that the sciatic nerve was injured in a duel 3 decades before and that this injury resulted in a loss of sensation in the right posterior thigh an
