Submucosal lipomas of the large bowel are uncommon. Occasionally, they occur in the rectum and may cause aspecific symptoms; presentation with rectal prolapse is very unusual and may lead to a misdiagnosis of simple mucosal prolapse. The paper describes an additional case of a prolapsing rectal mass that led to diagnosis and surgical treatment of a rectal lipoma under local anesthesia.

BACKGROUND AND AIMS: Sigmoid volvulus is a major cause of large-bowel obstruction in "volvulus belt" countries, where it accounts for up to 50% of cases. Flexible endoscopic detorsion is the first-line therapy for stable patients; however, recurrence rates may reach 75%. Placement of a rectal decompression tube reduces early recurrence and facilitates elective surgery. In many low-resource settings, commercial tubes are unavailable. Rectal expander-assisted transanal endoscopic microsurgery (RE-TEM) was performed for two cases of early rectal cancer and a case of villous tumor under saddle block anesthesia. RE-TEM is the new technique for local excision of rectal tumors with a rectal expander that we developed. BACKGROUND/AIMS: Many patients with a complete rectal prolapse tend to be old. Therefore, surgeons tend to choose a surgical procedure associated with minimal stress. However, the recurrence rate is problematic. Altemeier's procedure causes minimal stress. This procedure is often selected for performing a complete rectal prolapse.
