A patient aged 73 had repeated episodes of melaena and a systolic murmur suggesting mitral insufficiency. An obstructive cardiomyopathy was diagnosed on echocardiography, while the gastro-intestinal tract investigations proved negative (gastro-duodenal endoscopy, barium meal and follow-through), until ileal vascular tumours were diagnosed by selective mesenteric arteriography. Diagnosis could only be confirmed on exploratory laparotomy, and definitive treatment was undertaken at the same time. The authors discuss the practical and theoretical consequences, including the need for thorough gastr

PURPOSE: The entity of diffuse microscopic angiodysplasia is described, and a patient with severe gastrointestinal hemorrhage because of this submucosal source of bleeding is reported.
METHOD: Case records of a patient with severe gastrointestinal hemorrhage were reviewed, and histologic findings were compared with colonoscopic and operative findings. The patient received 51 units of packed red blood cells over 3.5 months and remained undiagnosed, despite an exhaustive evaluation, until autopsy.
RESULTS: Ectatic veins, venules, and capillaries were present within the submucosa in virtually e

Heyde syndrome is characterized by the association of aortic stenosis, gastrointestinal bleeding from angiodysplasia, and acquired von Willebrand syndrome. We report two contrasting cases illustrating how lesion distribution and therapeutic accessibility may influence immediate bleeding management options and clinical course. The first patient was a 72-year-old male with severe calcific aortic stenosis who presented with recurrent melena and severe anemia. Upper gastrointestinal endoscopy and colonoscopy were negative, whereas capsule endoscopy revealed numerous angiodysplastic lesions diffuse

UNLABELLED: Gastrointestinal bleeding of unknown origin presents a difficult diagnostic problem. The authors show 2 angiodys plasiastic cases. In the first case the clinical history contains multiple examinations and transfusions because of anaemia. The diagnosis was achieved by selective angiography. In the antrum of the stomach justified angiodysplasia was accompanied by aortic valve disease. Billroth I. operation was performed. In the other case anoscopy, colonoscopy and gastroscopy was necessary because of massive rectal bleeding but in the lack of bleeding source they performed right hemi

Ascending colon angiodysplasia is a frequent cause of colorrhagia or chronic blood loss in old patients, but also possible under the age of 55. Angiodysplasia diagnosis is often underestimated. For a long time colorrhagia or lower intestinal bleeding were generally diagnosed like diverticular bleeding. This conclusion was the result of several conditions: the conservative management of distal gastrointestinal bleeding is in most cases successful; the double dye clysma was the first and the main diagnostic procedure for many years and the result was a not correct diagnosis of diverticular bleed

Vascular malformations usually have an impact on bones because of hemodynamic disturbances in the muscular and osteoperiosteal systems. In fact, even when these structures are not involved by arterial or venous malformations, the blood flow is altered in them. The consequences for bones affect the longitudinal growth of the skeleton of the malformed limb. Growth rarely becomes slower. On the contrary, the condition usually results in an acceleration of bone growth, which results in an excess in the length of the limb presenting with the vascular malformation. This overlengthening may be variab

Angiodysplasia, a recently discovered disease, is a pathology of scant incidence in our environment and difficult to find because of the inconclusiveness of diagnostic proofs. The disease, generally related to adults and the elderly, and preferentially located in the large intestine, does not always manifest this way. The case we present is one of the youngest in the world literature and of ectopic localization in the terminal ileum. This confirms the enigmatic aspect of this pathology, which still has characteristics to be delimited. Its etiology, incidence and prevalence are unknown and ther

Twenty-five angiodysplasias in the stomach and duodenum were identified with endoscopy in 12 patients. The average age of the patients was 70 years. Two patients had aortic stenosis. Six of the patients were treated with electrocoagulation, three of whom required surgery. Angiodysplasia of the upper gastrointestinal tract is considered to be more common than previously thought and an important cause of both obscure and overt bleeding.

Gastrointestinal haemorrhage from angiodysplastic lesions is not only difficult to identify, but often refractory to endoscopic intervention. Patients often require substantial transfusion support. Thalidomide has emerged as a promising medical strategy in angiodysplasia-related bleeding. We present our experience and report the findings from a review of the literature. Despite its side-effect profile, thalidomide remains the therapeutic modality with the best evidence in this difficult clinical scenario.

We describe a patient in whom endoscopy failed to determine the origin of gastrointestinal bleeding, and magnetic resonance angiography showed dilated inferior pancreaticoduodenal veins that were considered indirect signs of the duodenal angiodysplasia. Incidentally, a connection between the inferior vena cava and the inferior pancreaticoduodenal veins were also noted. Repeat endoscopy and catheter angiography confirmed the presence of the angiodysplasias.

Twelve patients with angiodysplasia of the gastrointestinal tract were seen at The Mary Imogene Bassett Hospital are presented. Six share the features of gastric or duodenal angiodysplasia, advanced age, and aortic valve disease. Of these 6 patients, 4 who bled repeatedly were treated with endoscopic coagulation of areas of gastric and duodenal angiodysplasia. Six patients with other types of gastrointestinal angiodysplasia are presented for comparison. Two had gastric angiodysplasia and no aortic valve disease, 2 had hereditary hemorrhagic telangiectasia, 1 had received irradiation, and 1 cou

Gastrointestinal angiodysplasia (GIAD) are red flat arborized lesions that are found throughout the entire gastrointestinal tract. GIAD can vary in size and have a range of presentation from occult to life-threatening bleeding. The typical presentation is intermittent bleeding in the setting of iron deficiency anemia. Endoscopy is the primary means of diagnosis and endoscopic therapy is noted to be initially effective. However, rebleeding can be as high as 40% to 50% in patients with small bowel GIAD. This review describes the pathophysiology for the development of GIAD and the current roles o

We identified angiodysplasia as a cause of gastrointestinal bleeding in 15 of 47 patients (32%) with chronic renal failure (CRF) treated with dialysis who were evaluated for upper or lower gastrointestinal hemorrhage during a 5-year period. Nine patients had lesions in the upper, four in the lower, and two in the upper and lower gastrointestinal tract. Ten patients with angiodysplasia experienced more than one bleeding episode during that time. Active hemorrhage from a vascular lesion was seen in nine patients (60%). Erosive gastritis and/or duodenitis was the second most common finding. These

Intraoperative angiography is quite useful in locating the site of bleeding in patients with angiodysplasia of the small intestine. We recommend the placement preoperatively of an angiographic catheter in these patients. We have described a technique that complements this procedure with metallic clip labeling to locate precisely the area of bleeding. This method of treatment is illustrated in the present study by reports of patients bleeding from vascular malformations of the ileum, duodenum and the head of the pancreas.
