[Excimer laser angioplasty for subclavian artery stenosis--case report].

Endovascular angioplasty for subclavian artery stenosis is effective and less invasive than other methods; however, it is difficult to advance guidewires or balloon catheters through severely stenotic lesions. We report a case of subclavian artery stenosis treated with excimer laser angioplasty. A 58-year-old man with hypertension and hyperlipidemia was admitted to a hospital with different blood pressure in each arm. A CT scan revealed left subclavian artery stenosis, and the patient was transferred to our hospital. An angiogram revealed a severely stenotic lesion in his left subclavian arter

Treatment of in-stent restenosis with excimer laser coronary angioplasty: benefits over scoring balloon angioplasty alone.

Treatment of in-stent restenosis (ISR) is associated with a high incidence of recurrence. This study evaluated the clinical safety and 6-month efficacy of excimer laser coronary angioplasty (ELCA) before scoring balloon dilatation for the treatment of ISR. Twenty-three patients with ISR were included and treatment strategy of ISR was dependent on each operator. Twelve patients among those were treated with ELCA before scoring balloon dilatation (ELCA group) and 11 patients were treated with scoring balloon alone (non-ELCA group). Acute procedural results were evaluated by quantitative coronary

Effects of laser thermal angioplasty on arterial contractions and mechanics.

In each of 8 rabbits, a fiberoptic with a 1.5 mm diameter metal tip was inserted into the abdominal aorta. After preheating with 8-10 watts of argon laser energy, the metal tip was passed through the external iliac artery. Thermal injury was evaluated by histology and by measuring arterial contractions and wall mechanics. The contralateral iliac artery was used as a control. Laser thermal angioplasty (LTA) produced foci of coagulation necrosis, medial thinning, and wall rupture. Concomitantly, maximal arterial contractile force was reduced by 80% (p less than 0.01) and arterial wall stiffness

Coronary Perforation and Late Pseudoaneurysm Outcomes After Excimer Laser Angioplasty.

BACKGROUND: Excimer laser coronary angioplasty (ELCA) is used in complex percutaneous coronary intervention (PCI). ELCA-related coronary artery perforation (CAP) is uncommon, and its long-term outcomes remain poorly defined. AIM: The aim of this study was to characterize the long-term clinical and angiographic course of ELCA-related CAP. METHODS: We retrospectively reviewed 1023 consecutive ELCA-assisted PCI procedures. CAP incidence was assessed, and the clinical and procedural features of CAP and non-CAP cases were compared. The predictors of CAP were explored using univariable logistic re

Application of excimer laser coronary angioplasty (ELCA) in bifurcation lesions.

Excimer laser coronary angioplasty (ELCA) offers a unique approach to the treatment of bifurcation lesions that continues to present a challenge in percutaneous coronary intervention. Debulking plaque prior to stenting or balloon angioplasty has demonstrated significant improvement in the treatment of bifurcation lesions. Clot dissipation properties of excimer laser combined with its ability to debulk, makes this device unique when applied to thrombus-laden bifurcation lesions. ELCA is the only debulking technique that allows retention of two guide wires with resultant protection of the bifurc

Peripheral artery recanalization in humans using balloon and laser angioplasty.

The treatment of patients with complex peripheral arterial disease and those who have had previous unsuccessful attempted revascularization procedures can be clinically challenging. Initial treatment was begun using therapy by percutaneous balloon and laser angioplasty, then proceeding to bypass surgery if severe ischemia persisted. Both percutaneous and cut-down approaches were used to access totally occluded arteries. An attempt was made to cross the occlusion mechanically with either a guide wire or an activated laser probe. If laser recanalization was not successful, the patient underwent

[Perforation of the vascular wall after coronary excimer laser angioplasty].

A 56-year-old patient with recurrent episodes of angina pectoris at rest was admitted for left-heart catheterization. The coronary angiogram revealed two-vessel disease with a subtotal stenosis in the circumflex artery and a long 90% stenosis in the proximal part of the left anterior descending artery. On the basis of symptoms and coronary morphology laser angioplasty was performed. After a total of 99 s of energy delivery extravasation of dye occurred at the site of ablation. Due to acute vessel occlusion subsequent PTCA was performed, resulting in a satisfactory angiographic result. Extravas

Laser angioplasty for superficial femoral and proximal popliteal artery occlusion.

A prospective study of percutaneous laser assisted thermal angioplasty for superficial femoral and proximal popliteal artery occlusions was performed on 93 consecutive patients with a mean length of occluded segment of 7.6 cm. Technical success was achieved in 78 patients (84%). Subsequent patency or occlusion has been confirmed by duplex ultrasound scanning or arteriography. The crude patency rate for successfully recanalised vessels was 48% during a mean follow-up period of 18 months. Subsequent femoropopliteal bypass or amputation was required in 20% of these patients. In vivo photosensitizer-enhanced laser angioplasty in atherosclerotic Yucatan miniswine.

Photosensitization of atherosclerotic (Ath) plaque with exogenous chromophores may confine laser irradiation (LI) injury to Ath plaque and allow safer and more efficacious laser angioplasty. Dihematoporphyrin ether (DHE) accumulates preferentially in plaque, and previous in vitro LI studies have shown confinement of LI injury to plaque. To further evaluate DHE enhanced laser angioplasty, 5 Ath Yucatan miniswine with 8 iliofemoral stenoses (Sten) were treated with 2.5 mg/kg of DHE IV. Twenty-four hours later, 632 nm continuous wave LI was delivered to Sten using 200mu core optical fiber with a

Laser in infra-popliteal and popliteal stenosis (LIPS): retrospective review of laser-assisted balloon angioplasty versus balloon angioplasty alone for below knee peripheral arterial disease.

Despite the use of laser technology over the last decade, there are limited data to show its procedural and clinical success in infra-popliteal disease. We hypothesized that laser-assisted balloon angioplasty (LABA) is at least similar or better in procedural and peri-procedural outcomes compared to balloon angioplasty (BA) alone, despite adverse lesion characteristics prior to intervention. Retrospective chart and angiogram review of consecutive critical limb ischemia (CLI) patients who underwent endovascular revascularization in the popliteal or infra-popliteal vessels between 2007 and 2012

Percutaneous laser thermal angioplasty: initial clinical results with a laser probe in total peripheral artery occlusions.

A metal-tipped laser fibre was used during percutaneous angioplasty of femoral/popliteal or iliac artery occlusions in 56 patients. Primary success was achieved in 50 (89%) of these total occlusions, providing a channel for subsequent balloon dilatation. Before the procedure, 18 lesions had been judged untreatable by conventional angioplasty and four of the six failures were in these. Complications directly attributable to the laser probe were one case of vessel perforation and two cases of entry into vessel walls; these had no sequelae. Other acute complications were a distal thrombosis in a

Laser in infrapopliteal and popliteal stenosis 2 study (LIPS2): Long-term outcomes of laser-assisted balloon angioplasty versus balloon angioplasty for below knee peripheral arterial disease.

BACKGROUND: Laser-assisted balloon angioplasty (LABA) has been shown to be more effective in achieving angiographic success for treatment for below knee peripheral artery disease (PAD) compared with balloon angioplasty alone(BA). However, long-term outcomes of LABA compared with BA for popliteal and infrapopliteal PAD are unknown. METHODS: We evaluated data on 726 patients undergoing LABA (n = 395) and BA (n = 331) for popliteal and infrapopliteal PAD retrospectively at a single center (2007-2012). Outcomes included long-term ipsilateral major limb amputation, revascularization and mortality

Perforation thresholds and safety factors in in vivo coronary laser angioplasty.

Laser angioplasty can cause early (acute perforation) or late (stenosis or aneurysm) complications. To find how much intravascular laser energy can be delivered via a 100 microns core optical fibre passed down a balloon angioplasty catheter without causing angiographic abnormalities up to 10 days later, argon laser energy was delivered percutaneously under radiographic screening to the coronary circulation of 12 normal closed chest dogs. With the balloon inflated, sequential laser pulses were delivered to the same site. Angiograms were recorded before, immediately, and again at one week, after

Thermal laser-assisted angioplasty of renal artery stenosis for renovascular hypertension.

Percutaneous transluminal laser-assisted angioplasty of a renal artery stenosis was performed in a 16-year-old woman with renovascular hypertension. The stenotic portion of the renal artery was predilated by delivering Nd-YAG laser energy to the terminal tip of a laser catheter. Although the luminal diameter did not increase sufficiently with laser angioplasty alone, it allowed passage of the balloon catheter and subsequent successful balloon angioplasty. Immediately after dilatation, the patient's blood pressure fell to normal, and plasma renin activity decreased. There were no serious compli

Percutaneous angioscopic evaluation of luminal changes induced by excimer laser angioplasty.

Angioscopy has been shown to provide more detailed information on lesion morphology before and after interventional procedures than angiography. Therefore to evaluate the effects of laser angioplasty, angioscopy was performed in five patients with peripheral or coronary vascular disease who underwent excimer laser angioplasty. The excimer laser was operated at 308 nm, 135 nsec, 25 Hz, and 40 to 60 mjoules/mm2 and was coupled into multifiber wire-guided catheters of 1.4 to 2.0 mm diameter for coronary lesions and into catheters of 2.2 mm diameter for peripheral lesions. Acute biologic response to excimer versus thermal laser angioplasty in experimental atherosclerosis.

Vascular injury and platelet accumulation after balloon angioplasty are two potentially important triggers of the process of restenosis that may be minimized by the use of laser energy to ablate atherosclerotic plaque. The type of laser most suitable to achieve these goals remains unknown. Accordingly, angiographic and histologic studies and quantitative platelet deposition analysis were performed on 27 atherosclerotic rabbit iliac arteries randomized to treatment with excimer laser or thermal laser angioplasty. Excimer laser angioplasty was achieved with 35 to 40 mJ/mm2 of 308 nm xenon chlori

Analysis of solid phase debris from laser angioplasty: potential risks of atheroembolism.

Laser energy can vaporize biologic tissues, and this unique method of ablation has been considered for the disobstruction of the occlusive lesion in atherosclerotic disease. To assess the potential embolic sequelae from laser angioplasty, solid phase debris (SPD) was analyzed. Specimens of human cadaver aorta were subjected to standardized argon laser injury, and SPD was quantified by weight in four types of ablated tissue: normal aortic intima, fatty streaks, fibrous plaque, and calcified plaque (CP). The debris by weight of tissue ablated was significantly higher for CP (p less than 0.05), m
