[Antiphospholipid antibodies in digestive diseases].

UNLABELLED: DIAGNOSIS AND PROGNOSIS: Antiphospholipids comprise a very heterogeneous group of auto-antibodies including anticardiolipids demonstrated by immunological methods and lupus anticoagulants detected by coagulation tests. Antiphospholipids are encountered in various conditions other than dysimmune disease and are frequently involved in thrombotic manifestations. We discuss here the implications of these antibodies in digestive tract diseases and present an analysis of their diagnostic and prognostic value for optimal therapeutic and monitoring approaches. Obstetric complications of antiphospholipid antibodies.

Pregnancies occurring in women with antiphospholipid antibodies are at significantly increased risk of miscarriage, prematurity, intrauterine growth retardation and preeclampsia. Insights into the pathological basis of these conditions suggest that antiphospholipid antibodies impair embryonic implantation and in later gestations cause thrombosis of the uteroplacental vasculature. This offers a rational basis for thromboprophylaxis during pregnancy.

Neurologic complications of antiphospholipid antibodies.

Antibodies directed against phospholipids are highly associated with venous and arterial thrombotic episodes, which are often recurrent. There seems to be a skewed frequency of cerebral and ocular events when the arterial circulation is affected. Other neurologic syndromes, including dementia, migraine, chorea, transverse myelopathy, Guillain Barré syndrome, transient global amnesia, seizures, motor neuron disease, myasthenia gravis, and depression, have also been described in association with aPL. Although some of them (for example, dementia, chorea, seizures, and transient global amnesia) co

Stroke associated with antiphospholipid antibodies.

There is a strong link between antiphospholipid antibodies and stroke. Retrospective studies indicate that these patients are generally young, often have had prior thrombotic events including stroke, frequently complain of headaches, and often experience amaurosis fugax. Cardiac valvular lesions, particularly involving the mitral valve, are frequently found. The limited pathological studies show a noninflammatory thrombotic occlusion of both large and small cerebral vessels and a "thrombotic endocarditis." The underlying pathogenesis involves an immune-mediated prothrombotic state.

Antiphospholipid Antibodies in Lupus Nephritis.

Lupus nephritis (LN) is a major manifestation of systemic lupus erythematosus (SLE). It remains unclear whether antiphospholipid antibodies (aPL) alter the course of LN. We thus investigated the impact of aPL on short-term and long-term renal outcomes in patients with LN. We assessed levels of aPL cross-sectionally in SLE patients diagnosed with (n = 204) or without (n = 294) LN, and prospectively in 64 patients with active biopsy-proven LN (52 proliferative, 12 membranous), before and after induction treatment (short-term outcomes). Long-term renal outcome in the prospective LN cohort was det

Antiphospholipid antibodies and renal involvement.

Antiphospholipid antibodies are a heterogeneous group of autoantibodies associated with the hypercoagulable state affecting all vascular districts with thrombosis named antiphospholipid syndrome (APS). APS is an autoimmune disease with multifactorial etiology that includes cellular, molecular, genetic and pathogenic mechanisms. The APS clinical features are a combination of arterial and/or venous thrombosis, hematological events, recurrent fetal losses, neurological disorders and intra-abdominal manifestations. The renal involvement is associated with both primary and secondary APS. Clinical f

Cerebral haemostasis and antiphospholipid antibodies.

One of the major clinical concerns of the antiphospholipid syndrome (APS) is the propensity of antiphospholipid (aPL) antibodies to cause thrombosis in both the large and small vessels of the brain. In this article, we review the current understanding of haemostasis in cerebral circulation and discuss this in the context of antiphospholipid antibodies. The systemic-defect-local-phenotype paradox is of particular importance in this discussion. In this paradigm, a systemic defect in thrombosis and haemostasis leads to a localized pattern of thrombotic disease because the regional physiological v

[Antiphospholipid antibodies in Behcet's disease].

35 patients with Behcet's disease (BD) were examined for antiphospholipid antibodies (aPL). Elevated levels of antibodies to cardiolipin (aCL) were registered in 20% of the patients (aCL IgG and IgM in 11.4 and 8.6%, respectively). Low positivity of these antibodies prevailed (33.2 GPL and 41.2 MPL). Lupus anticoagulant was present in 6 patients. No distinct correlation was found between positivity by aPL and vascular disorders. It is inferred that aPL do not seem to play a leading role in thrombotic diathesis in BD. This diathesis is brought by a much more complicated and multifactor process.

Antiphospholipid antibodies: findings at arteriography.

PURPOSE: The purpose of this study was to determine the frequency and types of abnormalities at arteriography in patients with antiphospholipid antibodies (APA) and ischemic cerebrovascular events. METHODS: Twenty-three patients with APA and ischemic cerebrovascular events who underwent arteriography were identified. Patients over the age of 65 years were excluded. No patients met diagnostic criteria for systemic lupus erythematosus. All angiograms were reviewed by two neuroradiologists. RESULTS: Seventeen patients (74%) between the ages of 28 and 64 years (average age, 40 years) had abnorma

Significance of IgA antiphospholipid antibodies.

IgA anticardiolipin (IgA aCL) and IgA anti beta2-glycoprotein-I (IgA anti beta2GP1) antibodies are common in SLE and have been associated in some studies with thromboses and thrombocytopenia. Experimental work suggests that IgA aCL are as prothrombotic as the IgG-IgM isotypes. However, in SLE there appears to be less concordance between IgA aCL and IgA anti beta2GP1 as compared with the concordance between IgG and IgM isotypes, suggesting significant differences in their origins and specificities. For example, there may be a greater mucosal contribution to production of IgA antibeta2GP1 than I

Antiphospholipid antibodies and myocardial infarction.

In prospective studies, increased levels of cardiolipin-biding antibodies and autoantibodies to oxidized low-density lipoprotein (LDL) have been observed in patients with myocardial infarction (MI). These findings suggest that antiphospholipid antibodies may contribute to the development of MI. The 'oxidative-modification hypothesis' in the pathogenesis of atherosclerotic heart disease is based on the oxidation of LDL, its accumulation into arterial wall, and the development of chronic inflammation in the atheroma. Evidence of enhanced lipid peroxidation and its association with antiphospholip

[Retinal vasculitis and antiphospholipid antibodies].

Antiphospholipid antibodies (APAb) are often found in systemic lupus erythematosus (LE) (secondary antiphospholipid syndrome), leading to arterial or venous thrombosis. In primary antiphospholipid syndrome other LE-associated symptoms are not detectable. We present the clinical course and therapy of three patients with this rarely reported disease, discussing immunopathology and therapy. Three young patients (28, 29 and 31 years) were seen at the university eye clinic, presenting severe occlusive vasculitis with vitreal haemorrhages and/or thrombosis. The first patient did not respond to any t

Pregnancy complicated by antiphospholipid antibodies.

The manifestations of antiphospholipid antibodies in pregnancy are multiple and include maternal arterial and venous thrombosis, spontaneous abortion, intrauterine fetal death, intrauterine growth retardation, and preeclampsia. Maternal complications may also arise in the puerperium with the development of an autoimmune pleuropulmonary postpartum syndrome. Currently, there is confusion in the literature regarding appropriate treatment of patients known to possess these antibodies. We have reported the case of a patient at 29 weeks' gestation who had elevated blood pressure, proteinuria, and ea

Antiphospholipid antibodies in cerebral ischemia.

In a 2-year prospective study of 146 patients with cerebral ischemia, we compared vascular risk factors for stroke with clinical and laboratory findings, particularly antiphospholipid antibodies. Ten patients (6.8%) were positive for at least one antiphospholipid antibody; one patient had systemic lupus erythematosus, one had rheumatoid arthritis, and the remaining eight fulfilled criteria for the diagnosis of primary antiphospholipid syndrome. These patients were predominantly male, not necessarily young, and 50% of them did not have any other vascular risk factors; there were no significant
