INTRODUCTION: The aim of this study was to investigate the allele and genotype frequencies of 8 human platelet antigen (HPA) systems among blood donors from the Blood Transfusion Institute of Serbia and to compare them with published studies. These data would be useful to establish the basis for a platelet apheresis donor registry.
MATERIAL AND METHODS: Seventy-two unrelated male platelet apheresis/blood donors from Serbia were typed for 8 HPA systems (HPA-1 to HPA-6, HPA-9, and HPA-15) via the FluoGene method, based on polymerase chain reaction-sequence-specific amplification (PCR-SSP; PCR u

Platelet antigens are essential cellular components in the coagulation cascade and the immune response. Platelets are small anucleate cellular-fragments, measuring 2 to 3 μm, resulting from the fragmentation of megakaryocyte cytoplasm. Their role in hemostasis involves clot formation to halt bleeding in case of vascular injuries. Platelet antigens are proteins found on the surface of these cell fragments, participating in interactions with other blood cells and the immune response. Genetic variability and individual differences in antigens have led to the discovery of various antigen systems,

Platelet transfusions are a cornerstone of therapy for patients who develop thrombocytopenia while undergoing Hematopoietic Stem Cell Transplantation (HSCT). Many patients who develop Platelet Transfusion Refractoriness (PTR) require HLA-matched platelets. Identifying these patients early could lead to better utilization of platelets as well as increased platelet counts. We built a SMART on FHIR visualization tool to aid the oncology, blood bank, and blood donor center teams in identifying these patients by showing trends in thrombocytopenia along with a computer generated calculated Panel Rea

Antibodies to Class I human leukocyte antigens (HLAs) are a major immunologic cause of platelet transfusion refractoriness (PTR). HLA inheritance is highly related to ethnicity. Because most platelet donors are White, Hispanic patients may be at an increased risk of developing HLA antibodies due to mismatch between donor and recipient. This retrospective study analyzed 16 Hispanic patients with PTR to evaluate HLA antibodies associated with PTR. Baseline mean fluorescent intensity (MFI) was used to measure HLA antibody detection. Additive MFI values and clinical cut points were evaluated for C

OBJECTIVES: Platelet refractoriness complicates the platelet transfusion, which is essential for managing thrombocytopenia in patients with hematological disorders. It is associated with adverse clinical outcomes and increased health care costs. We conducted a prospective study to determine the effectiveness of cross-matched compatible platelets in a group of patients refractory to platelets from random donors and to evaluate human leukocyte antigen (HLA)-mediated refractoriness.
METHODS: This prospective study was conducted on 40 patients with different hematological disorders requiring plat

BACKGROUND: Alloimmunization against human platelet antigens (HPAs) during pregnancy is rare but can lead to severe bleeding disorders, such as fetal and neonatal alloimmune thrombocytopenia.
STUDY DESIGN AND METHODS: In a cohort of 241 uncomplicated pregnancies, we investigated the immunogenicity of HPA mismatches and correlated HLA sensitization with HPA antibody formation. HPA antibodies were measured with a Luminex-based multiplex assay.
RESULTS: HPA mismatches were observed in 109 of 241 pregnancies (45%), but child-specific HPA antibodies were only found in two of 109 cases (2%), indic

BACKGROUND AND OBJECTIVES: Foetal and neonatal alloimmune thrombocytopaenia (FNAIT) is a potentially severe immune-mediated condition that is believed to be under-identified in the Canadian population. The Canadian Blood Services National Platelet Immunology Reference Laboratory (NPIRL) serves as a centralized referral laboratory for FNAIT investigations across Canada. Diagnostic evaluation includes human platelet antigen (HPA) genotyping and maternal anti-HPA antibody testing. This study is a review of all FNAIT investigations referred to NPIRL to assess testing patterns, antibody detection r

BACKGROUND AND OBJECTIVES: Population study on immunogenetic polymorphism of human platelet antigens (HPAs) may aid in predicting the risk of alloimmune thrombocytopaenia syndromes. We aimed to characterize the diversity of HPAs in Saudis and estimate the alloimmunization risk.
MATERIALS AND METHODS: We assessed the genotype and allele frequencies of HPA-1 to -11 and HPA-15 in 118 Saudi blood donors using a real-time polymerase chain reaction assay. We estimated the mismatch risk associated with blood transfusions and pregnancies for each individual allele. Furthermore, we evaluated the misma

Recently, human platelet antigens (HPAs) polymorphisms are found to play a role in susceptibility to hepatitis C virus (HCV) infection and fibrosis progression. The aim of the current study was to evaluate the possible association between the HPAs polymorphisms with liver fibrosis progression in HCV patients. HPAs polymorphisms genotyping was performed in HCV patients (n = 71) by Sequence-specific primers-polymerase chain reaction. Fibrosis progression was evaluated using the Metavir scoring system and liver biopsy, and the patients were assigned to two groups, namely, G1 (n = 35) that include

Human platelet alloantigens (HPA) are important in neonatal alloimmune thrombocytopenia (NAIT), posttransfusion purpura (PTP), platelet transfusion refractoriness, passive alloimmune thrombocytopenia, and transplantation-associated alloimmune thrombocytopenia. Thus, HPA genotyping is essential in diagnosis and treatment. We analyzed HPA-1 to 6 and Gov alleles, using PCR with sequence specific primers (PCR-SSP) in 500 Thai blood donors who had been HLA class I antigen typed. HPA-4a was present in all samples. HPA-1b, -2b, -5b, and -6b were rare, and HPA-4b was not found. HPA-3a and -3b showed f

Platelet transfusions are a critical and life-saving intervention utilized in bleeding patients and those at risk of bleeding. The laboratory assessment of fresh or stored platelets, both before and after transfusion, is of interest in licensing and basic science research. Platelets can be given either allogeneic (i.e., from a genetically different donor) or autologous (i.e., donor and recipient are the same, or genetically identical). In this protocol, we demonstrate how to distinguish circulating endogenous platelets from transfused platelets using biotinylation (useful for autologous and al

BACKGROUND: In this study, the frequency of human platelet antigen (HPA) alleles and genotypes of platelet donors in Wuhan was evaluated and differences in HPA distribution between Wuhan and other regions in China were compared.
STUDY DESIGN AND METHODS: HPA genotyping was performed using RT-qPCR in 1516 platelet donors in Wuhan. The allele frequencies of HPA-1 to -6, -10, -15, and -21 were calculated and evaluated according to the Hardy-Weinberg principle. Mismatch probability was calculated for each HPA system. The distribution and features of HPA systems in Wuhan were compared to those of

BACKGROUND: The Ley antigen is a carbohydrate chain belonging to the ABH-Lewis blood group family. Ley has been reported to be present on red blood cells (RBCs) and granulocytes, but its distribution and function in platelets remain unknown. There are a variety of glycoproteins on platelets, which may carry the Ley antigen. This study aims to investigate the expression pattern and the function of Ley in human platelets.
STUDY DESIGN AND METHODS: Flow cytometry, Western blot, and immunofluorescence assays were performed to determine Ley expression on human platelets. ADP (1.25-10 μM) and throm

BACKGROUND: Neonatal alloimmune thrombocytopenia (NATP) caused by fetomaternal mismatch for human platelet (PLT) alloantigens (HPAs) complicates approximately 1 in 1000 to 1 in 2000 pregnancies and can lead to a serious bleeding diathesis, intracranial hemorrhage, and sometimes death of the fetus or neonate. As a national reference center for NATP investigations, our experience with this entity over a 12-year period was reviewed.
STUDY DESIGN AND METHODS: The laboratory records of all cases of suspected NATP referred for evaluation from January 1, 1990, to December 31, 2002, were analyzed. Th

BACKGROUND: Neonatal alloimmune thrombocytopenia (NAIT) has been reported only rarely in twins and not at all, to our knowledge, in triplets.
CASE REPORT: Nonidentical triplets were born with severe thrombocytopenia. Nadir platelet (PLT) counts were 17 × 109 , 12 × 109 , and 10 × 109 /L. NAIT was confirmed by an incompatibility for human PLT antigen (HPA)-1a and the presence of maternal anti-HPA-1a. The maternal genotype was HPA-1bb and the paternal genotype was HPA-1aa; thus, all children were affected.
RESULTS: PLT counts for each infant improved with the administration of random-donor PLT

Fifty-eight of 89 serum samples (65.17%) from HIV-1-infected individuals at various disease stages contain antibodies that react with a platelet peptide located in the cytoplasmic domain of integrin beta3, glycoprotein GPIIIa (aa749-761; sequence DRKEFAKFEEERA). Rabbit polyclonal antibodies raised against the synthetic platelet peptide also react with the structurally homologous HIV-1 gp41-derived peptide (EKNEQELLELDKW(A)) and bind to a Western blot band with molecular weight corresponding to HIV-1 gp41. These findings point to molecular mimicry between HIV-1 and a human membrane protein foun

INTRODUCTION: Pregnant women negative for human platelet antigen 1a (HPA-1a) are at risk of alloimmunization with fetal HPA-1a antigen inherited from the father, and their offspring may develop fetal and neonatal alloimmune thrombocytopenia (FNAIT). The aim of this study was to analyze the frequency of HPA-1a alloimmunization in pregnant Polish women, the feasibility of using maternal platelets for intrauterine transfusions in women subjected to diagnostic fetal blood sampling (FBS) and to discuss potential consequences of alloimmunization.

Immunofluorescence microscopy was used to study the localization of platelet factor 4 (PF4), fibronectin (Fn) and factor VIII related antigen in washed normal platelets and those from patients with severe von Willebrand's disease (vWd). Platelets were prepared by an improved multi-unit modification of the Sayk chamber. This facilitated the preparation of samples with minimum disruption of platelets and readily permitted the demonstration of intact platelet granules by the immunological techniques used. The antigens were demonstrated by treating the same preparations sequentially with appropria

BACKGROUND: Immune-mediated platelet transfusion refractoriness (IPTR) remains a major clinical challenge, with immune alloimmunization against HLA and HPA as the predominant causes. Although HLA cross-reactive groups (CREGs) provide a practical alternative to allele-level matching, methods for simultaneous HLA-CREG and HPA genotyping remain limited despite their potential to improve platelet matching.
METHODS: Specific U-anchoring primers (UAPs) and amplification refractory mutation system (ARMS) primers were designed and optimized based on one or more SNPs within HLA-CREG subgroups and HPA
