Design, synthesis and biological evaluation of carbohydrate-based sulphonamide derivatives as topical antiglaucoma agents through selective inhibition of carbonic anhydrase II.

A series of new carbohydrate-based sulphonamide derivatives were designed, synthesised by employing the so-call 'sugar-tail' approach. The compounds were evaluated in vitro against a panel of CAs. Compared to their parent compound p-sulfamoylbenzoic acid, these compounds showed nearly 100-fold improvement in their binding affinities against hCA II in vitro. All of compounds showed great water solubility and the pH value of their water solutions of compounds is 7.0. Such properties are advantageous to make them much less irritating to the eye when applied topical glaucomatous drugs, compared to

Antiglaucoma medications: a review of safety and tolerability issues related to their use.

BACKGROUND: Much experience has been gained with the use of older classes of antiglaucoma agents--topical beta-adrenergic-receptor antagonists, nonselective adrenergic-receptor agonists, oral carbonic anhydrase inhibitors, and cholinergic agents. In the past decade, new drugs and classes of drugs used to treat glaucoma have become available, including topical carbonic anhydrase inhibitors, prostaglandin analogues, and alpha2-adrenergic-receptor agonists. Extensive community-based use of antiglaucoma medications has led to an increased understanding of the acute and long-term safety and tolerab

Carbonic anhydrase inhibition with natural products: novel chemotypes and inhibition mechanisms.

Five genetically distinct classes of carbonic anhydrases (CAs, EC 4.2.1.1) are present in organisms all over the phylogenetic tree (α-, β-, γ-, δ-, and ζ-families). These metalloenzymes catalyze CO(2) hydration to bicarbonate and protons. Inhibition of α-CAs from vertebrates, including humans, with sulfonamides was exploited clinically for decades for various classes of diuretics and systemically acting antiglaucoma agents, whereas newer inhibitors are used as topically acting antiglaucoma drugs, anticonvulsants, antiobesity, antipain and antitumor agents/diagnostic tools. Recently, novel inte

Design, Synthesis and Evaluation of Novel Molecular Hybrids between Antiglaucoma Drugs and H2S Donors.

Glaucoma is a group of eye diseases consisting of optic nerve damage with corresponding loss of field vision and blindness. Hydrogen sulfide (H2S) is a gaseous neurotransmitter implicated in various pathophysiological processes. It is involved in the pathological mechanism of glaucomatous neuropathy and exerts promising effects in the treatment of this disease. In this work, we designed and synthetized new molecular hybrids between antiglaucoma drugs and H2S donors to combine the pharmacological effect of both moieties, providing a heightened therapy. Brinzolamide, betaxolol and brimonidine we

[Contemporary possibilities and perspectives in the treatment of glaucoma].

PURPOSE: The aim of this paper is to present and discuss the contemporary possibilities and perspectives of applying neuroprotective mechanisms in the treatment of glaucoma. MATERIAL AND METHODS: A review of the newest concepts concerning neuroprotective action of antiglaucoma drugs was done. RESULTS: Not until recently was it believed that glaucoma is caused by an increased intraocular pressure. When it comes to diagnosing glaucoma it is nowadays considered that intraocular pressure is not the most important factor. Glaucoma is presently regarded as a condition in which characteristic chang

Carbonic anhydrase inhibitors.

Carbonic anhydrases (CAs, EC 4.2.1.1) are widespread enzymes in all organisms, catalyzing CO2 hydration to bicarbonate and protons. Their inhibition is exploited clinically for decades for various classes of diuretics and systemically acting antiglaucoma agents. In the last years novel applications of CA inhibitors (CAIs) emerged, such as topically acting antiglaucoma, anticonvulsants, antiobesity, antipain, and antitumor agents/diagnostic tools. Such CAIs target diverse isozymes of the 13 catalytically active alpha-CA isoforms present in mammals. CAs belonging to the alpha-, beta-, gamma-, de

Drug interaction considerations in the therapeutic use of carbonic anhydrase inhibitors.

INTRODUCTION: Carbonic anhydrase inhibitors (CAIs) of the sulfonamide and sulfamate type are clinically used drugs as diuretics, antiglaucoma, antiepileptic, antiobesity and anti-high altitude disease agents. Anticancer agents based on CAIs are also in clinical development for the management of hypoxic, metastatic tumors. Acetazolamide, methazolamide, dichlorophenamide, dorzolamide and brinzolamide are mainly used as antiglaucoma drugs, sulthiame, topiramate and zonisamide as antiepileptic/antiobesity agents, celecoxib and polmacoxib are dual carbonic anhydrase/cycloxygenase inhibitors. Gonioscopy-Assisted Transluminal Trabeculotomy in Angle Recession Glaucoma.

This case report outlines the successful management of angle recession (AR) with gonioscopy-assisted transluminal trabeculotomy (GATT). A 53-year-old man with a history of blunt trauma to the left eye resulting in AR presented with an IOP of 38 mm Hg on 4 antiglaucoma agents. After GATT, a notable reduction in IOP to 10 mm Hg was observed on the first postoperative day. Subsequent 1-year follow-up visits demonstrated sustained improvement, with IOP stabilizing at 18 mm Hg without the need for antiglaucoma medication. This is the first case report that demonstrates the role of GATT in the manag

Glaucoma medications in pregnancy.

Glaucoma is a chronic, progressive disease of the optic nerve which is relatively uncommon in childbearing age; however, its management is a real challenge. Although the intraocular pressure decreases in pregnancy, many glaucoma patients continue to require treatment. In addition to the side effects, antiglaucoma medications on the mother, the possible side effects of drugs on the fetus must be taken into account. The only antiglaucoma medication categorized in Category B is brimonidine, and all others are in Category C. Most evidence on glaucoma medications in pregnancy comes from single case

Drug modification of angiogenesis in a rat cornea model.

PURPOSE: To evaluate the influence of some widely used antiglaucoma agents on angiogenesis in a novel rat cornea model. METHODS: Angiogenesis was induced in 32 rats by slow-release polymer pellets containing basic fibroblast growth factor (bFGF) placed in a corneal micropocket. Angiogenesis was later measured and compared in groups of rats given one of four antiglaucoma drug therapies and one control group. The drugs were commercially available preparations of prostaglandins, beta-blockers, alpha-2 agonists, and carbonic anhydrase inhibitors given for 7 days in a manner similar to that used i

Newer therapeutic vistas for antiglaucoma medicines.

Glaucoma is second to cataract as a leading cause of global blindness and is the leading cause of irreversible visual loss. By the year 2020, almost 80 million people are estimated to be affected with open-angle glaucoma and angle-closure glaucoma. Sufficiently high concentrations of a free drug should be achieved within the aqueous humor and at the iris/ciliary body for sufficiently illustrating an antiglaucoma effect. Most drug groups including cholinergic agents, prostaglandin analogs, and carbonic anhydrase inhibitors being used in glaucoma were initially developed for other effects or rou
