Snake antivenin administration in a patient allergic to horse serum.

We have described a case in which a potent epinephrine drip was useful in the management of a patient who needed antivenin and was allergic to horse serum. With 50 ml of 1:10,000 epinephrine in 500 ml of 5% dextrose in water, repeated anaphylactoid reactions were controlled. We believe that such a solution would be helpful in other patients having severe anaphylactic reactions to antivenin.

Rattlesnake bites in children: antivenin treatment and surgical indications.

BACKGROUND: Orthopaedic surgeons working in the Americas may be consulted in the care of patients bitten by venomous rattlesnakes (genus Crotalus ), particularly with regard to the possibilities of compartment syndrome and soft-tissue destruction. Despite considerable evidence regarding the safety and efficacy of antivenin in the treatment of rattlesnake bites in adults, controversy persists regarding the roles of antivenin and surgery in the treatment of rattlesnake envenomations in children. Our hypothesis is that aggressive use of antivenin is just as effective and safe for children as it i

Levels of therapeutic antivenin and venom in a human snakebite victim.

An enzyme-linked immunosorbent assay was used to measure the levels of therapeutic antivenin (Antivenin [Crotalidae] Polyvalent, Wyeth-Ayerst) in serum and the levels of venom in the urine of a patient bitten by a western diamondback rattlesnake (Crotalus atrox). Serum and urine samples were taken on admission, during hospitalization, and during follow-up until 5 months after the bite. Photographs were taken of the bite site on admission to the hospital and during follow-up. Serum levels of therapeutic antivenin were highest between 1 and 3 days after the bite but were still detectable 46 days

Comparison of a new ovine antigen binding fragment (Fab) antivenin for United States Crotalidae with the commercial antivenin for protection against venom-induced lethality in mice.

Snake venom poisoning is a medical emergency requiring immediate attention and the exercise of considerable judgment. Of the estimated 8,000 bites inflicted by venomous snakes in the United States each year, approximately 6,000 are treated with commercial antivenin. The only commercially available antivenin for North American Crotalidae envenomation is Antivenin (Crotalidae) Polyvalent (equine origin) (ACP; Wyeth Laboratories, Philadelphia, PA). A common complication is the high incidence of hypersensitivity reactions, occurring in more than 75% of patients treated with ACP. To minimize these

An unusual localization of snakebite treated without antivenin: case report.

Snakebite is one of the commonest causes of morbidity and mortality in tropical regions. This paper focuses on the medical treatment of a patient who had been bitten by a snake on the vertex of his head. It was a severe envenomation with ensuing marked edema associated with coagulation abnormalities. Although the antivenin was extensively sought in various medical centers, it was unavailable. We observed and treated the patient, with special regard to his hematological findings and potential complications. Specifically, tetanus prophylaxis and intravenous fluids were administered, and the woun

The effect of antivenin on intramuscular pressure elevations induced by rattlesnake venom.

The dose of an antivenin required to neutralize a clinical case of venom poisoning, as well as determining the timing or need to initiate antivenin treatment, is frequently difficult to objectively ascertain. In this study, venom from the southern Pacific rattlesnake, Crotalus viridis helleri, was injected into 29 dog hind limb anterolateral compartments. A solution of C. v. helleri venom (15 mg/ml) was prepared using dessicated venom and saline; 0.2 ml were injected sub-fascially so that each of the compartments received 3 mg of venom. In one group no antivenin was given, in a second group fo

Elevated compartment pressures from copperhead envenomation successfully treated with antivenin.

BACKGROUND: Copperhead envenomation causes local soft tissue effects; however, associated compartment syndrome is rare. We report a case of a 17-month-old with significantly elevated compartment pressures successfully treated with antivenin and supportive care. CASE REPORT: A 17-month-old girl sustained a copperhead bite to the foot and presented with circumferential edema, erythema, and ecchymosis of the foot and distal ankle. The patient had palpable pulses and was neurologically intact. Four vials of Crotalidae polyvalent immune Fab was initiated and additional doses were administered in a

Use of CroFab antivenin in the management of a very young pediatric copperhead envenomation.

The use of crotalid Fab antivenin (CroFab) in the treatment of snake envenomations in the pediatric population is still an underexplored area. There are very limited data to confirm the efficacy and safety of dosing children the same as adults and even less information available to evaluate this antivenin use in copperhead snake bites in children. We report the first use of crotalid Fab antivenin in an adult dose for a copperhead snake envenomation in a 2-year-old child. She had rapid resolution of symptoms with no adverse effects. The report serves to increase the literature supporting the cu

Venoms of coral snakes (Micrurus spp.): report on a multivalent antivenin for the Americas.

A multivalent coral snake antivenin was prepared in horses immunized with a mixture of venoms from the species Micrurus nigrocinctus, M. mipartitus, and M. frontalis, following immunization procedures previously reported (2). Plasma from the horses was fractionated with ammonium sulfate. The antivenin produced was then tested against venoms from ten species. The neutralization titers obtained indicate it would be useful in treating bites received from most of the important coral snake species in North and South America, namely: M. fulvius, M. alleni, M. carinicaudus dumerilii, M. corallinus, M

Use of antivenin to treat priapism after a black widow spider bite.

Black widow spider envenomation (BWSE) is commonly associated with severe abdominal pain, muscle cramping, and hypertension. Treatment is primarily symptomatic with the use of opiates and benzodiazepines. Priapism is a complication of BWSE that has only rarely been reported. We describe a 17-month-old male who developed priapism after known BWSE. His priapism did not respond to opiates or benzodiazepines, and he was treated with black widow spider antivenin. Complete detumescence followed within several hours. The patient required no additional opiates for pain and was discharged from the hosp

Recognition and successful treatment of priapism and suspected black widow spider bite with antivenin.

Priapism, although uncommon in preadolescent children, is considered a true emergency. Envenomation by a black widow spider bite has been reported to induce priapism as a manifestation of its toxicity. Early recognition and timely administration of antivenin have been reported to be effective in relieving priapism. Clinicians who care for children need to be aware of this unusual presentation. The diagnosis is traditionally from either direct observation of a spider bite or capture of a spider. We report a case of a previously healthy 2-year-old boy who presented with severe irritability, leg

Sympathetic nervous system activation, antivenin administration and cardiovascular manifestations of scorpion envenomation.

UNLABELLED: We performed two-dimensional echocardiograms and determined plasma norepinephrine levels on admission and at 24h after hospitalization, in 16 children with scorpion envenomation. All patients came from areas where scorpions have been identified as Tityus zulianus and received antivenin at the site of the accident or upon admission. Based on the presence or absence of cardiovascular manifestations, patients were divided into two groups. GROUP A: 10 patients had cardiovascular manifestations of pulmonary edema. Four patients had mild pulmonary edema (Left ventricular ejection fracti
