Panic and generalized anxiety: are they separate disorders?

Evidence that panic and generalized anxiety disorder are separate disorders is presented from a review of epidemiological, family, and twin studies. Generalized anxiety disorder and panic disorder have been shown to have different rates and risk factors in the community and different family aggregation, which is consistent with the separation of the disorders. The one small twin study using Diagnostic and Statistical Manual, Third Edition criteria also supports the separation between these disorders. The concordance for panic disorder is greater among monozygotic than among dizygotic twins. Th

The cognitive anxiety sensitivity treatment (CAST) in anxiety prevention - Focus on separation anxiety and interoception.

Given the high prevalence and considerable clinical and societal burden of anxiety disorders, preventive measures are urgently warranted to reduce their incidence and overall healthcare impact. Anxiety sensitivity (AS) - a key element in learning theories of anxiety disorders in the context of interoceptive conditioning - constitutes a malleable risk factor of particularly panic disorder and separation anxiety, which share developmental, nosological, epidemiological and pathomechanistic characteristics. The computer-assisted 'Cognitive Anxiety Sensitivity Treatment' (CAST) targeting interocept

Subpopulations of early separation anxiety: relevance to risk of adult anxiety disorders.

BACKGROUND: The present study aims to examine whether discrete subpopulations can be identified according to their levels of early separation anxiety (SA), and if so, whether such a typology of SA influences risk to particular adult anxiety disorders. METHODS: Mixture analysis was applied to early SA scores provided retrospectively by a composite group (n=1800) of adult community and patient samples. The distribution of adult anxiety diagnoses across the SA categories was assessed in a community (n=136) and a clinic (n=74) sample. RESULTS: The mixture analysis yielded two subpopulations acco

Spanish Validation of the Separation Anxiety Assessment Scale.

The aim of this study was to investigate the factorial structure and psychometric properties of the Separation Anxiety Assessment Scale (SAAS) with Spanish schoolchildren. The participants in Study 1 were 1281 children aged 8-11. Exploratory factor analysis identified four factors: worry about calamitous events, fear of abandonment, fear of being alone, and fear of physical illness, which explained 47.77 % of the variance. The participants of Study 2 were 4628 schoolchildren aged 8-11. The four related factors model was validated by confirmatory factor analysis. The internal consistency (α = .

Psychopharmacology of maternal separation anxiety in vervet monkeys.

Maternal separation in non-human primates has been proposed as a model of early adversity. The symptoms of separation anxiety were studied in vervet monkeys, during the weaning period, when psychotropic medications were administered. The control group received a normal diet and treatment groups received citalopram, reboxetine or lamotrigine in their food daily. Treatment was given for 7 weeks starting 1 month prior to weaning. Behavior was recorded twice weekly for 8 weeks, and was rated for anxiety and depression. Cerebrospinal fluid was collected at the beginning and end of the trial and ana

Separation anxiety disorder, panic disorder, and school refusal.

Studies attest to the chronic nature of anxiety in youth. Consequently, there is a growing literature examining causative pathways to these disorders and efficacious treatments for them. This review examines separation anxiety disorder, a commonly occurring childhood anxiety disorder, and panic disorder, a more rare anxiety disorder in youth. Finally, school refusal, which may be tied to an internalizing or externalizing disorder or no disorder at all, is examined.

Separation anxiety disorder in children: disorder-specific responses to experimental separation from the mother.

BACKGROUND: Separation anxiety disorder (SAD) is one of the most common anxiety disorders in childhood and is predictive of adult anxiety disorders, especially panic disorder. However, the disorder has seldom been studied and the attempt to distinguish SAD from other anxiety disorders with regard to psychophysiology has not been made. We expected exaggerated anxiety as well as sympathetic and respiratory reactivity in SAD during separation from the mother. METHOD: Participants were 49 children with a principal diagnosis of SAD, 21 clinical controls (CC) with a principal diagnosis of anxiety d

Do Youth with Separation Anxiety Disorder Differ in Anxiety Sensitivity From Youth with Other Anxiety Disorders?

Studies on the relationship between separation anxiety disorder (SAD) in childhood and panic disorder (PD) in adolescence and adulthood have yielded results which suggest a common underlying vulnerability for both disorders. In this study, we examined whether one such possible vulnerability-anxiety sensitivity-differed for youth diagnosed with SAD versus other anxiety disorders. Anxiety sensitivity was assessed using the Childhood Anxiety Sensitivity Index (CASI) in 315 clinic-referred youth (ages 6-17, 113 girls). 145 children (46%) were diagnosed with one or more primary anxiety disorder, in

Reported early separation anxiety symptoms in patients with panic and generalised anxiety disorders.

Attachment theory has proposed that early separation anxiety is a risk factor for adult anxiety disorder, with the recent focus being particularly on panic disorder. The results of empirical studies examining this link are, however, contradictory, possibly because of inconsistencies across studies in measuring memories of early separation anxiety. In the present study, a psychometrically sound measure, the Separation Anxiety Symptom Inventory (SASI) was used to compare memories of such early symptoms in panic disorder (including those with mild phobic-avoidance), generalised anxiety disorder a

Adult separation anxiety and insomnia symptoms in individuals with mood and anxiety disorders.

The Diagnostic and Statistical Manual for Mental disorder (DSM-5) identifies severe sleep disruption as one of the clinical features and diagnostic criteria for Separation Anxiety Disorder (SAD), with one diagnostic criterion being a persistent reluctance or refusal to sleep away from home or to go to sleep without being near a central attachment figure. While SAD is commonly associated with childhood, it can also present in adulthood (A-SAD), especially in individuals with mood and anxiety disorders. During this phase of an individual's life, A-SAD may prominently interfere with daily activit

Childhood separation anxiety and separation events in women with agoraphobia with or without panic disorder.

OBJECTIVE: This study aimed to investigate the association between separation anxiety and actual separation events during childhood in adult patients with agoraphobia with or without panic disorder (PD). METHOD: Forty-two women with agoraphobia with or without PD participated in long-term follow-ups after exposure-in-vivo treatment. We assessed separation anxiety and separation events from age 0 to 18 years, as well as adult separation from a spouse. RESULTS: Childhood separation experiences (55%) and separation anxiety were significantly higher in patients than in healthy subjects, but both

The effect of mothers' perception of birth on separation anxiety.

This study was conducted to examine the effect of mothers' perception of birth on separation anxiety, a rarely studied but clinically important topic in perinatal mental health. A descriptive and correlational design was used to identify relationships without intervention. The sample consisted of 305 postpartum women between the seventh day and sixth week, recruited from a private hospital. Data were collected using the Introductory Information Form, the Perception of Birth Scale, and the Adult Separation Anxiety Questionnaire. Statistical analyses included independent t-tests, ANOVA, Pearson

Separation anxiety: at the neurobiological crossroads of adaptation and illness.

Physiological and adaptive separation anxiety (SA) is intimately connected with the evolutionary emergence of new brain structures specific of paleomammalians, the growth of neomammalian--and later hominid--brain and skull size, and the appearance of bipedalism. All these evolutionary milestones have contributed to expanding the behavioral repertoire and plasticity of prehuman and human beings, at the cost of more prolonged dependency of the infant and of the child on parental care. Separation anxiety disorder (SAD) can be seen as an exaggerated/inappropriate manifestation of SA that constitut

A neurobiological framework of separation anxiety and related phenotypes.

In the DSM-5, separation anxiety disorder (SAD) is newly classified in the chapter on anxiety, renewing research efforts into its etiology. In this narrative review, we summarize the current literature on the genetic, endocrine, physiological, neural and neuropsychological underpinnings of SAD per se, SAD in the context of panic disorder, separation anxiety symptoms, and related intermediate phenotypes. SAD aggregates in families and has a heritability of ~43%. Variants in the oxytocin receptor, serotonin transporter, opioid receptor µ1, dopamine D4 receptor and translocator protein genes have

Separation anxiety in children and adolescents treated with risperidone.

Risperidone is an atypical antipsychotic that has been investigated as a treatment for several severe psychiatric disorders in children and adults. A low dose of risperidone was added to other medications to treat two adolescent boys with obsessive compulsive disorder (OCD) and a childhood history of separation anxiety disorder, and one prepubertal boy with a history of attention deficit hyperactivity disorder and escalating behavior suggestive of mania. Each patient developed severe separation anxiety that resolved when risperidone was discontinued. The response was similar to that described

Can drug effects on anxiety and convulsions be separated?

The effects of benzodiazepines, barbiturates, a series of novel putative anxiolytic compounds and anxiogenic compounds are reviewed in animal tests of anxiety and on experimentally-induced seizures. It is clear from the data that drug effects on anxiety and convulsions are not always in the same direction; certain compounds are apparently both anxiolytic and proconvulsant, others are anxiogenic and anticonvulsant, others have varied effects depending on the test situation. It is suggested that this work necessitates considerable revision of our traditional concepts of an "anticonvulsant." The

Separation anxiety: Extreme dehiscence of a mitral annuloplasty ring.

We report a patient admitted with acute pulmonary edema 3 months after mitral valve repair, with no history of inter-current febrile illness. Transesophageal echocardiography (TEE) demonstrated severe mitral regurgitation (MR) and an abnormally positioned annuloplasty ring, suggestive of dehiscence. The extreme extent of ring dehiscence was visualized on 3-dimensional TEE (3D), with near-complete separation of the ring. Strept.Mitis and Cristatus were isolated from the ring following redo mitral valve surgery, confirming endocarditis as the mechanism for dehiscence. This report highlights the

Separation anxiety and panic disorder in clinically referred youth.

This study examined whether youngsters with separation anxiety disorder (SAD) and panic disorder (PD) had experienced more separation-related events than youngsters with SAD (without comorbid PD). We also examined whether age of onset of SAD and comorbidity with other psychological disorders was related to the occurrence of PD. We compared youngsters who were diagnosed with SAD and PD (N=31) with youngsters who were diagnosed with SAD without comorbid PD (N=63) for the number of separation-related events, severity of psychopathology, and parent and child CBCL ratings, age of onset of SAD, and

Panic, suffocation false alarms, separation anxiety and endogenous opioids.

This review paper presents an amplification of the suffocation false alarm theory (SFA) of spontaneous panic [Klein DF (1993). False suffocation alarms, spontaneous panics, and related conditions. An integrative hypothesis. Arch Gen Psychiatry; 50:306-17.]. SFA postulates the existence of an evolved physiologic suffocation alarm system that monitors information about potential suffocation. Panic attacks maladaptively occur when the alarm is erroneously triggered. That panic is distinct from Cannon's emergency fear response and Selye's General Alarm Syndrome is shown by the prominence of intens
