A turn for the worse: The impact of deteriorating healthcare spending in South Africa.

The November 2023/2024 national health budget cuts represent the largest reduction in public health expenditure in South Africa's post-apartheid history. This article critically examines the implications of these budget cuts within the context of the country's post-apartheid health system reforms and macroeconomic strategies. Specifically, it documents the historical evolution of fiscal policies and health expenditure across three distinct periods: the 2008 - 2013 period, which witnessed a counter-cyclical fiscal strategy that improved healthcare access and began addressing apartheid-era dispa

'Foreign body': a social history of Implanon in South Africa's Eastern Cape.

This article explores the reception of the contraceptive implant, Implanon, by healthcare workers and patients in family planning units in South Africa's public health sector. Based on observations conducted at public health facilities in the Eastern Cape Province, and on interviews with nurses and patients in the same province, the study explored real-world experiences of the implant. This article examines the strategies used by nurses to promote use of the device, and explores how patients themselves responded to a widescale, national rollout of the implant within government family planning

Conceptions of Contraceptive Use in Rural KwaZulu-Natal, South Africa: Lessons for Programming.

Community family planning programmes in South Africa arose from the controversial apartheid history of controlling the African population while encouraging the growth of European migrant population. Post-apartheid population policies shifted away from population control to aligning policies to the global agenda that placed emphasis on the link between population and development. The focus on population and development polices in post-apartheid South Africa is on social equality, justice and peace rather than controlling sections of the population. Given the shift, this paper interrogates the c

Suicidal behavior in black South Africans.

In the past suicidal behavior among Black South Africans has been largely underresearched. Earlier studies among the other main ethnic groups in the country showed suicidal behavior in those groups to be a serious problem. This article briefly reviews some of the more recent research on suicidal behavior in Black South Africans. The results indicate an apparent increase in suicidal behavior in this group. Several explanations are offered for the change in suicidal behavior in the reported clinical populations. This includes past difficulties for all South Africans to access health care facilit

Moving across boundaries: migration in South Africa, 1950-2000.

Existing knowledge about historical patterns of black internal migration in South Africa is incomplete, primarily because of the lack of good life course studies as well as the apartheid government's suppression and censoring of data. This article provides a comprehensive picture of historical internal migration patterns with an analysis of a unique individual retrospective life history data set. This sample of the black population, collected in 2000, is the only known nationally representative life history data for South Africa; it includes all residential moves for each individual during his

Gender norms in South Africa: implications for HIV and pregnancy prevention among African and Indian women students at a South African tertiary institution.

In post-Apartheid South Africa, women are constitutionally guaranteed protections and freedoms that were previously unknown to them. These freedoms may have positive implications for women's ability to negotiate sexual protection with partners and hence prevent unintended pregnancy and decrease their risk of HIV. Among tertiary institution students, who are a relatively 'privileged' group, there is little information on gender norms that might shape responses to HIV-prevention programmes. To elicit gender norms regarding women's and men's roles, condom and contraceptive use, sexual communicati

Effects of early adversity and social discrimination on empathy for complex mental states: An fMRI investigation.

There is extensive evidence of an association between early adversity and enduring neural changes that impact socioemotional processing throughout life. Yet little is known about the effects of on-going social discrimination on socioemotional functioning. Here we examined how cumulative experiences of social discrimination impact brain response during empathic responding-a crucial issue in South Africa, given its historical apartheid context and continuing legacies. White and Black South Africans completed measures of social adversity (early adversity and social discrimination), and underwent

The state of psychiatry in South Africa today.

The very idea/philosophy behind the South African government's policy of Apartheid (separation), is insulting and humiliating to the black people and tends to incite arrogance and a sense of superiority, even omnipotence, in the whites. Personality development is skewed along racial lines. The mental effects of malnutrition and certain physical diseases, both of which are widespread in South African blacks, especially among their children, are well known. The torture to which security-police detainees have been subjected has resulted in thousands of cases of mental ill-health, as well as physi

South Africa: a legacy of family disruption.

This article draws together unusual characteristics of the legacy of apartheid in South Africa: the state-orchestrated destruction of family life, high rates of unemployment and a high prevalence of HIV/AIDS. The disruption of family life has resulted in a situation in which many women have to fulfil the role of both breadwinner and care giver in a context of high unemployment and very limited economic opportunities. The question that follows is: given this crisis of care, to what extent can or will social protection and employment-related social policies provide the support women and children

A psychosocial study of albinism in a predominantly Mulatto Caribbean community.

Psychosocial data collected from 12 albinos in rural and isolated parts of the Dominican Republic suggest a stigma is associated with the absence of skin color. The general population living in the area has a skin pigmentation of dark brown. The purpose of the investigation was to assess whether there were psychosocial problems for albinos with white skin and hair. Prior studies imply albinism is a negative social characteristic associated with prejudice and apartheid in a predominantly mulatto community. This researcher recommends that psychologists and health-care providers be familiar with

Living alone and depression in a developing country context: Longitudinal evidence from South Africa.

In South Africa, single-person households were a household type enforced by apartheid legislation, designed to restrict the "influx" of Africans into "white" and typically urban areas of the country. Yet the increase in living alone has been one of the most marked demographic trends of the post-apartheid period. The trend, which has occurred alongside the persistence in individual labor migration patterns and a decline in rates of union formation, has been driven by changes among working-age adults, rather than among older adults. This is the first empirical study to investigate the mental hea

Dementia Diagnosis in Postapartheid South Africa: Providers' Perspectives in Ethnographic Context.

This article situates perspectives of South African social service and health care providers on older adults who live with dementia and Alzheimer's disease in ethnographic context. A review of findings from multi-year field research on long-term care and service provision for older adults in peri-urban Mpumalanga shows: racial disparities in accessing formal dementia diagnoses; aggression, forgetfulness, and wandering as the most reported symptoms of presumed dementia; and provider-reported ethno-racial differences in families' diagnostic- and care-seeking practices. Findings corroborate evalu

'I can do things that others can't': Civic policing as weaponized volunteering in eThekwini, South Africa.

In this article, we analyse civic policing in post-apartheid South Africa as a form of 'weaponized volunteering'. We use 'weaponized volunteerism' as a conceptual lens to refer to practices that rest on the potentiality and/or willingness to use physical violence or to harness the physical violence of others under the guise of 'volunteer work'. By drawing from ethnographic fieldwork conducted by both authors in eThekwini, South Africa, we show that by framing civic policing as weaponized volunteerism, we are able to analyse the violence at the core of policing and underline the varied ways tha

Interventional cardiology in South Africa.

South Africa is a country in political and cultural flux. Because of the iniquitous system of apartheid, inequalities have been perpetuated at all levels including health care. Due to the high prevalence of ischemic heart disease and rheumatic heart disease, coronary angiography, PTCA, intracoronary stenting, and balloon valvuloplasty have become established cardiological procedures in South Africa. However, a new political dispensation promises changes to health care, which will impact on interventional cardiology. With a proposed emphasis on primary health care, resources for tertiary health

Modelling the economics of gaming in South Africa.

This paper considers the application of two models for determining the optimal location and characteristics of a casino in a post-apartheid South Africa. The intention in developing the models was to allow provinces a facility for considering how to maximize the return to the stakeholders in the license award process, namely society at large, as represented by the provincial government, and the casino operator. The Allocation Model works on an estimate of total potential gambling spend and how that may be best distributed amongst a number of casinos. The Profitability model takes the estimated

Caregivers' and parents' explanatory models of intellectual disability in Khayelitsha, Cape Town, South Africa.

BACKGROUND: Post-apartheid, understanding and management of intellectual disability remain poor in South Africa, complicated by various contextual and cultural explanations used to describe and conceptualize this condition. METHOD: We conducted 20 semi-structured interviews with primary caregivers and parents of children with intellectual disability residing in Khayelitsha, a low-income setting in Cape Town, South Africa. We used Kleinman's Explanatory Models (EMs) of illness to explore terms used to describe and conceptualize this condition. RESULTS: Carers' explanatory models included biom

South Africa's protracted struggle for equal distribution and equitable access - still not there.

The purpose of this contribution is to analyse and explain the South African HRH case, its historical evolution, and post-apartheid reform initiatives aimed at addressing deficiencies and shortfalls. HRH in South Africa not only mirrors the nature and diversity of challenges globally, but also the strategies pursued by countries to address these challenges. Although South Africa has strongly developed health professions, large numbers of professional and mid-level workers, and also well-established training institutions, it is experiencing serious workforce shortages and access constraints. "Injuries are beyond love": physical violence in young South Africans' sexual relationships.

South Africa's complex social and political history has produced conditions for interpersonal violence of multiple kinds to flourish. Violence experienced by girls and young women, including within their sexual relationships, has become an area of intense research and policy interest since the end of apartheid. Drawing on a long-term ethnographic study of young people in an urban township, this article explores how violent practices are variously construed, differentiated, and legitimated, in particular through the assignment of blame and the significance accorded to bodily marking. Art-based reflections from 12 years of adolescent health and development-related research in South Africa.

This paper presents empirical and methodological findings from an art-based, participatory process with a group (n = 16) of adolescent and young advisors in the Western Cape Province of South Africa. In a weekend workshop, participants reflected on their participation in 12 years of health and development-related research through theatre, song, visual methodologies and semi-structured interviews. Empirical findings suggest that participants interpreted the group research encounter as a site of empowerment, social support and as a socio-political endeavour. Through song, theatre and a mural ill

Heroin hustles: Drugs and the laboring poor in South Africa.

This article sets out a political economic framework to understand South Africa's dramatic upsurge in heroin use in the 2000s. Drawing on interviews with users and their families, it shows how the opioid gained influence among men in their twenties living in apartheid-engineered townships marked by chronic unemployment. Giving particular attention to histories of work, it documents the ways that men hustle to generate an income to buy heroin, showing their relationship to families who support them and community members who may employ them. The article challenges the view that heroin users' inc
