Assessment of marginal adaptation after apicoectomy and apical sealing with Nd:YAG laser.

OBJECTIVE: Recurrent periapical disease poses a substantial challenge to the dental practitioner. However, the use of novel methods and materials in periapical surgery has increased the success rates of treatment. The objective of this study was to assess the effects of mineral trioxide aggregate (MTA) retrofilling and apical sealing with Nd:YAG laser (at 10 Hz, 150 mJ, 1.5 W, in 60 sec pulses) as adjuncts to apicoectomy. METHODS: Perpendicular root end resection was performed with diamond-coated burs on 40 canine teeth previously subjected to endodontic treatment. Teeth were randomly allocat

Vital apicoectomy of the teeth: a 1-4 week histopathological study in Macaca mulatta.

Several studies have reported devitalization of teeth following surgical procedures in the area of the root apices. The purpose of this investigation was to observe the 1-4-week wound healing pattern in monkey dental tissues following intentional vital apicoectomy (IVA). Two adult Macaca mulatta were used in this study. All of the teeth were surgically transected within the apical one-third with a bur. The soft tissues were then closed. At death, a maxillary and a mandibular quadrant for 1, 2, 3 and 4 weeks following surgery were obtained after vascular perfusion. Each quadrant was demineraliz

Assessing the efficacy of apicoectomy without retrograde filling in treating periapical inflammatory cysts.

OBJECTIVES: The necessity of retrograde filling after apicoectomy is controversial in cases of non-inflammatory cysts as opposed to bacteria-related periapical abscesses. This study aims to investigate whether the presence or absence of retrograde filling during apicoectomy has differential long-term prognostic implications between inflammatory and non-inflammatory cysts. MATERIALS AND METHODS: This retrospective study included patients who underwent tooth apicoectomy during jaw cyst enucleation between 2013 and 2022, and who underwent follow-up cone-beam computed tomography for at least 6 mo

Healing process of dogs' pulpless teeth after apicoectomy and root canal filling at different levels.

Dogs' teeth with infected root canals, were submitted to apicoectomy and the root canals were filled flush or 2 mm short. In a third group the canals were left unfilled and only the access openings were closed. One hundred and eighty days after the treatment, the animals were killed and the specimens prepared for histological analysis. Repair was not observed in the teeth with unfilled root canals. The healing observed in the teeth with root canals filled flush was less complete than the healing observed when the root canals were filled 2 mm short.

VOLUMETRIC AND RADIOGRAPHIC OUTCOMES OF BONE REGENERATION AFTER APICOECTOMY WITH PRF-BASED THERAPIES - A PRELIMINARY STUDY.

BACKGROUND: Successful apicoectomy relies on effective bone healing at the resected root apex. Platelet-rich fibrin (PRF) and bone graft materials have been proposed to enhance regeneration, yet comparative evidence using both two-dimensional (2D) and three-dimensional (3D) imaging remains limited. OBJECTIVE: To compare bone regeneration following apicoectomy using PRF, PRF combined with Bio-Oss, and conventional technique, and to evaluate postoperative pain and edema. MATERIAL AND METHODS: Thirty patients undergoing apicoectomy were randomly assigned to three groups (n=10 each): PRF, PRF +

Resolution of a Cystic Endodontic-Periodontal Lesion Utilizing Combined Apicoectomy and Guided Tissue Regeneration: A Case Report.

INTRODUCTION: Endodontic-periodontal combined lesions pose a treatment challenge for clinicians, especially when the lesion is persistent and does not show signs of healing after initial endodontic treatment. This case report describes interdisciplinary management of a non-healing combined endodontic-periodontal lesion related to a peri-apical cyst, through a combined approach of apicoectomy and guided tissue regeneration (GTR) with enamel matrix derivative (EMD). Most documented cases have been treated in a sequential manner, with minimal literature present on the effects of a combined approa

Ventral Transorbital Apicoectomy of the Maxillary Second and Third Molar Teeth in Guinea Pigs (Cavia porcellus): 26 Cases.

A common cause of teeth malocclusion and feeding disorders in guinea pigs is macrodontia and odontogenic abscesses. If the maxillary second or third molar teeth are affected, surgical access to them has so far been achieved through enucleation or orbital evisceration due to their location at the base of the orbit. The study aims to demonstrate a transorbital surgical approach to the apices of the maxillary molar teeth (M2 and M3) in guinea pigs, allowing preservation of the eye. Twenty six apicoectomies of maxillary M2 and M3 were performed. The surgical approach involved a skin incision above

Development and treatment of retrograde peri-implantitis involving a site with a history of failed endodontic and apicoectomy procedures: a series of reports.

Osseointegrated implants provide predictable restorative support for crowns, restorations, prosthesis abutments, and removable dentures. Their widespread use in recent years has produced different types of complications. Retrograde peri-implantitis, a lesion occurring at the periapical area of an osseointegrated implant, has recently been described. This paper presents a series of reports describing the occurrence and management of retrograde peri-implantitis involving implants replacing teeth with histories of failed endodontic and apicoectomy procedures.

Healing of Extensive Through-and-Through Periradicular Lesion Using Apicoectomy Without Bone Grafting: A Case Report With 3-Year Recall.

The pathogenic process of through-and-through lesions leads to erosion and loss of both cortical plates. Endodontic microsurgery (MS)with bone graft and membrane placement (guided tissue regeneration (GTR)) is the proposed treatment for such cases. This article is aimed at discussing an unusual treatment protocol for a large through-and-through lesion without the use of bone grafting or membrane. A 26-year-old Air Force pilot traveling at high altitudes presented to the Endodontic division for management of pain and swelling related to the upper left anterior lesion concerning Teeth #21, #22,

All-in-one digital template-guided apicoectomy and modified socket shield technique for apical-infected aesthetic sites: A technical note.

INTRODUCTION: The socket-shield technique (SST) is one of the effective methods for maintaining hard and soft tissue contours during immediate implant placement. For non-restorable teeth complicated with chronic apical inflammation, conventional SST alone often fails to ensure complete elimination of apical infection. TECHNICAL NOTE: For aesthetic zone teeth with chronic apical inflammation that are planned for SST, fabrication of an apicoectomy guide using digital technologies not only enables thorough removal of apical lesions and guarantees the required shield length but also provides an o

Implant Apicoectomy as a Minimally Invasive Alternative to Explantation in Retrograde Peri-Implantitis: A Case Report with a 12-Month Clinical and Radiological Follow-Up.

Background/Objectives: Implant explantation is often considered the standard treatment for severe peri-implant inflammatory lesions, including cases with apical involvement. However, this approach is relatively invasive and aggressive, which can lead to significant loss of bone and soft tissues, potentially compromising future prosthetic rehabilitation, particularly in the esthetic zone. This study aimed to present implant apicoectomy as a conservative alternative that would allow preservation of implant stability while effectively treating localized inflammation. Methods: This case report pre

Subepithelial connective tissue graft associated with apicoectomy and root-end fillings in the treatment of deep localized gingival recession with apex root exposure: case report.

Periodontal reconstructive surgery procedures seek to correct mucogingival defects, including gingival recession. This case report describes the use of a subepithelial connective tissue graft (SCTG) associated with root-end fillings using mineral trioxide aggregate (MTA) for the treatment of Miller Class II recession with root apex exposure. A partial-thickness double pedicle flap was made, followed by root preparation with curette and bur finishing. The exposed root apex was removed and the canal was filled with MTA. An SCTG taken from the palate was placed over the root surface and covered w
