[Cutaneous necrosis at apomorphine injection points].

BACKGROUND: Apomorphine is a specific dopaminergic agonist used in the treatment of severe fluctuations of Parkinson's disease, particularly in patients on L-dopa. The drug is usually given subcutaneously, either as several daily injections or via a continuous subcutaneous delivery system. We describe two cases of localized cutaneous necrosis at the points of subcutaneous apomorphine injection. OBSERVATIONS: Two male patients presenting Parkinson's disease were treated by subcutaneous injection of apomorphine. One month later, asymptomatic necrotic lesions measuring from 2 to 5 mm appeared at

Apomorphine (Apokyn) for advanced Parkinson's Disease.

Apomorphine (Apokyn) is now available for rescue treatment of "off" episodes in patients with advanced Parkinson's disease. It must be injected subcutaneously. Apomorphine in a variety of formulations has been used in Europe for many years. Like other dopamine agonists it can cause nausea and vomiting, orthostatic hypotension, drowsiness, "sleep attacks" and dyskinesias. It should be taken initially with an antiemetic to avoid nausea and vomiting.

Apomorphine anorexia: a further pharmacological characterization.

Low doses of apomorphine reduce food intake, primarily by decreasing the rate of eating and also by reducing eating time. We have previously reported that the effect on eating time is mediated by dopamine cell body autoreceptors in the ventral tegmental area. The present experiments were designed to elucidate the pharmacological basis of the effect of apomorphine on eating rate. In the first experiment dopamine was also found to reduce food intake, but mainly by an effect on eating time. The peripheral DA antagonist domperidone abolished the effects of DA, but enhanced the effects of apomorphi

Practical considerations in the use of apomorphine injectable.

This manuscript provides a practical summary of guidelines for institution of apomorphine subcutaneous injectable therapy, including patient education, pre-treatment issues, dosage titration and side-effect care. The timing of each injection is crucial if an impending "off" period is to be averted. Patients need to be aware of symptoms of an approaching "off" period, and the injection should be administered at the onset or ideally, in anticipation of an "off" episode. Patients being considered for apomorphine treatment should undergo pre-treatment assessment and optimization of ongoing oral th

[Apomorphine in treatment of Parkinson's disease with fluctuations].

Apomorphine is a non-specific dopamine agonist, most similar to it, with a strong action on D2, D3, D4 receptors and weaker action on D1 and D5 receptors. It has been known since 100 years, and in Parkinson's disease it was used first in 1970 by Schwab and Cotzias. Apomorphine is used in Parkinson's disease with high-grade fluctuations of symptoms which cannot be controlled by oral drugs, especially in off" periods resistant to levodopa. After subcutaneous administration it changes the "off" to "on" period within 5-10 minutes. Unfortunately, its effect is short-lasting and wears off after 40-9

Vasodilatation produced by apomorphine in the hindleg of the dog.

Apomorphine, injected into the dog femoral artery in doses of 0.6 microng or higher, produced a marked increase in femoral blood flow, without a change in blood pressure. No increase in flow was produced by comparable doses of dopamine or morphine. After sympathetic denervation or administration of phenoxybenzamine, the apomorphine vasodilatation was either abolished or greatly reduced. When the vascular tone in the denervated hindleg was restored by sympathetic nerve stimulation, the vasodilatory effect of apomorphine often re-established itself; this was not the cases when the vascular tone

Sublingual apomorphine in the treatment of Parkinson's disease.

Advanced Parkinson´s disease (PD) is often complicated by fluctuations of disability depending on plasma levels of levodopa. For most patients OFF phases with worsening of tremor and immobility, but also pain, depression, autonomic symptoms are troublesome. While adjustments of levodopa administrations can relief such fluctuations for some time, "on demand" therapies become more and more important. These "on demand" therapies should provide fast and efficacious relief. During the past years, new options for on demand therapies in PD-associated OFF episodes have been developed, including new fo

[Apomorphine: a powerful ally in Parkinson's disease].

INTRODUCTION: Apomorphine, a D1-D2 dopamine agonist, is the oldest drug with proven efficacy in the treatment of Parkinson's disease (PD), and the only with similar symptomatic power to levodopa. Its usefulness in the control of motor fluctuations, both as intermittent injections and in continuous subcutaneous infusion, has been demonstrated in open label and placebo controlled trials. AIM: To analyse the role of apomorphine in the varied clinical symptoms and different clinical stages of PD through a narrative review of scientific literature (1951-2020). DEVELOPMENT: Beyond on-time increase

The efficacy of apomorphine - A non-motor perspective.

Non-motor features have a great impact on progression and quality of life in individuals with Parkinson's disease. Current treatments for PD are limited and apomorphine is one of the advanced therapies available with advantageous effects on motor complications. Several studies have suggested that apomorphine has potential benefits in PD patients beyond its established role in the treatment of motor fluctuations and levodopa-induced dyskinesia. This review examines the efficacy of apomorphine in the treatment of non-motor symptoms (NMS), describing recent studies that highlight its possible eff

Apomorphine therapy in Parkinson's and future directions.

Apomorphine infusion or injection is an important dopamine agonist non-oral therapy usually used in advanced Parkinson's disease (PD) with refractory motor fluctuations. The drug also has appreciable efficacy for nonmotor fluctuations and is the quickest to reverse predictable "off" periods. Current subcutaneous administration, however, is complicated by problems associated with needle-based therapies, such as skin nodule formation, skin irritation, and avoidance of this treatment option by needle-phobic subjects. In this review we focus on what the future might hold for apomorphine injection/

The pharmacological properties and therapeutic use of apomorphine.

Apomorphine (APO) is an aporphine derivative used in human and veterinary medicine. APO activates D₁, D(2S), D(2L), D₃, D₄, and D₅ receptors (and is thus classified as a non-selective dopamine agonist), serotonin receptors (5HT(1A), 5HT(2A), 5HT(2B), and 5HT(2C)), and α-adrenergic receptors (α(1B), α(1D), α(2A), α(2B), and α(2C)). In veterinary medicine, APO is used to induce vomiting in dogs, an important early treatment for some common orally ingested poisons (e.g., anti-freeze or insecticides). In human medicine, it has been used in a variety of treatments ranging from the treatment of addi

[Use of apomorphine in Parkinson s disease].

INTRODUCTION. Apomorphine is a D2 dopamine agonist, possibly underused despite its efficacy in situations where other medications have failed or lack of indication for stereotactic surgery. Rapidly effective (between 7 and 10 min) and potency approaching that of levodopa allows apomorphine to reduce reliably severe fluctuations, off period dystonia, biphasic dyskinesias or such symptoms as dysphagia, anismus of micturition difficulties. Unlike other agonists, apomorphine has a modest capacity to induce neuropsychiatric complications. Brief duration of response (<60 min) and the need of subcuta

Electrophysiologic and hemodynamic effects of apomorphine in dogs.

Apomorphine is a dopamine receptor agonist used as an emetic, for Parkinson's disease, and for treating erectile dysfunction. This study was conducted to monitor cardiovascular function in dogs given the standard emetic dose (0.05 mg/kg) or 10 times that. Measurements were made during baseline and at 1, 5, 15, 30, 45, and 60 min after iv administration. There were no changes produced by the 0.05 mg/kg dose of apomorphine except for a decrease in mean systemic arterial pressure (AoPm) at the 1 through 15 min recordings. For the 0.5 mg/kg dose, there were reductions in systemic vascular resistan

Intermittency in mastication and apomorphine-induced gnawing.

Rhythmic behaviors like mastication, gnawing, and locomotion, are characterized by temporal segmentation or intermittency. That is, they frequently occur as a series of short bursts interrupted by pauses rather than as one long uninterrupted burst. The function of intermittency as well as the mechanisms that produce it are unknown. Biogenic amine systems may play a role in producing intermittency; however, experimental work to confirm this is only in its infancy. The current study evaluates the structure of intermittency associated with mastication and apomorphine-induced gnawing in the guinea

Apomorphine anorexia: a behavioural and neuropharmacological analysis.

Anorectic effects of apomorphine were studied in a microstructural analysis paradigm. Low doses of apomorphine (less than 0.1 mg/kg SC) reduced food intake, by reducing both the rate of eating and eating time. The neuroleptics haloperidol and thioridazine blocked the effect of apomorphine on eating time, but not on eating rate. Anorectic effects elicited by apomorphine administration to the ventral tegmental area and, to a lesser extent, the substantia nigra were mediated by a selective reduction of eating time. Effects of apomorphine on eating time appear to result from an action at presynapt

Antagonism of apomorphine-induced pecking in pigeons.

Central nervous system stimulants, tranquillizers and other central nervous system depressants, antiemetics, antihistamine drugs and autonomic blocking agents were examined for their ability to prevent the pecking response in pigeons induced by apomorphine (250 mug/kg intramuscularly). Reduction in the proportion of positive responses or significant increase in the latent period of pecking were taken as the criterion of effectiveness. Protection was afforded by caffeine, lysergic acid diethylamide, morphine, rauwolscine, triflupromazine and yohimbine. In addition, a significant increase in lat

Apomorphine enhances harmaline-induced tremor in rats.

BACKGROUND: Harmaline-induced tremor is a well-known model of essential tremor in humans. The aim of the present study was to examine the influence of apomorphine, a non-selective dopamine receptor agonist, on the tremor induced by harmaline in rats. Propranolol (a first-line drug in essential tremor) was used as a reference compound. METHODS: Tremor, locomotor activity and focused stereotypy were measured objectively using force plate actimeters. Tremor was analyzed using a Fourier transform to generate power spectra for rhythmic behavior. RESULTS: The tremor induced by harmaline administer
