Ileocecal Intussusception Caused by an Appendiceal Neoplasm.

Mucinous appendiceal neoplasm occurs in less than 1% of appendicectomies. Majority of what is known in the literature is about pseudomyxoma peritonei, which exists as its disseminated form. Pictorial imagery of its pre-disseminated form is rarely observed. We present in a case report form a case of low-grade mucinous neoplasm of the appendix resulting in focal intussusception including images captured from this unique case that will become a learning case for readers of the journal.

Low-grade appendiceal mucinous neoplasms: a case series.

Appendiceal mucinous neoplasms, including low-grade appendiceal mucinous neoplasms (LAMNs), are infrequent but clinically relevant due to their potential progression to pseudomyxoma peritonei. Timely diagnosis and surgical intervention are critical to preventing complications. We present a case series of three middle-aged men with LAMNs, each of whom exhibited right lower quadrant abdominal pain as their primary symptom. Diagnostic imaging in each case revealed features consistent with appendiceal mucoceles. Pathological findings confirmed LAMN, with varying extents of disease. One patient und

Immunohistochemistry features and molecular pathology of appendiceal neoplasms.

Primary appendiceal neoplasms (ANs) comprise a heterogeneous group of tumors. The pathology and classification of ANs have been controversial, and thus, a new classification of these neoplasms was published in the World Health Organization (WHO) classification of tumors (5th edition, 2019). However, immunohistochemistry (IHC) features of epithelial ANs are not explained in this edition and the limited data on the molecular pathology of these tumors shows inconsistent findings in various studies. It would be useful to identify biomarkers appropriate for each subtype to better aid in treatment s

Low-grade appendiceal mucinous neoplasms with bowel obstruction.

BACKGROUND: Perforation of low-grade appendiceal mucinous neoplasms (LAMN) is characterized by its potential to spread mucin into peritoneal cavity, giving rise to pseudomyxoma peritonei, commonly treated with cytoreductive surgery and hyperthermic intraperitoneal chemotherapy.Symptoms of intestinal obstruction and appendiceal infection are rare at early stages of the disease, while abdominal distension are observed in the later stages due to mucin spread. METHODS: We report herein a case of LAMN with atypical symptoms in a 35-year-old woman with abdominal symptoms evoking an intestinal obstr

Pathogenesis of histologic variations of appendiceal mucinous neoplasms.

BACKGROUND: In order for peritoneal metastases from a primary appendiceal mucinous neoplasm to occur, the wall of the appendix must perforate to allow mucus with tumor cells access to the peritoneal spaces. With progression the peritoneal metastases show a broad spectrum of tumor biology varying from indolent to aggressive activity. METHODS: The histopathology of peritoneal tumor masses was determined from the clinical material resected at the time of cytoreductive surgery (CRS). All groups of patients were treated by a uniform strategy that involved complete CRS and perioperative intraperito

Mucinous appendiceal neoplasms: Incidence, diagnosis and surgical treatment.

INTRODUCTION: Mucinous tumors of the appendix are a rare pathology, with a prevalence below 0.5%. Clinical presentation usually occurs during the sixth decade of life, and mucinous tumors can clinically mimic acute appendicitis. The aim of this study is to describe the clinical and demographic variables, therapeutic procedure and diagnosis of these tumors. We analyze the association between mucinous tumors and pseudomyxoma peritonei (PP), as well as the association with colorectal and ovarian tumors. METHODS: A retrospective study was performed including patients who underwent an appendectomy

Appendiceal neoplasms derived from appendiceal tip remnants following appendectomy: a report of two cases.

BACKGROUND: Neoplasms derived from remnant appendix are rarely described, with most cases arising from the appendiceal "stump". Here, we present two surgical cases of appendiceal neoplasms derived from appendiceal "tip" remnants. CASE PRESENTATION: The first patient was a 71-year-old man who had undergone laparoscopic appendectomy for acute appendicitis 12 years prior. During appendectomy, the appendiceal root was ligated, but the appendix was not completely removed due to severe inflammation. At the most recent presentation, computed tomography (CT) was performed to examine choledocholithias

Appendiceal or cecal endometriosis with intestinal metaplasia: a potential mimic of appendiceal mucinous neoplasms.

Appendiceal or cecal endometriosis uncommonly presents diagnostic challenges, but rare cases with intestinal metaplasia can mimic mucinous tumors of the appendix. We describe 6 cases of appendiceal or cecal endometriosis with intestinal metaplasia that grossly and microscopically mimicked appendiceal mucinous tumors. The cases were characterized by marked mural hypertrophy and elastosis, with distortion of the appendix, and virtual obliteration of the lumen of the appendix. All cases had conventional endometriosis, together with varying amounts of endometriosis with intestinal-type epithelium.

Low-Grade Appendiceal Mucinous Neoplasm Presenting as an Appendiceal Mucocele: A Case Report.

Appendiceal mucocele is an uncommon clinical and radiological finding characterized by cystic dilatation of the appendix due to intraluminal mucin accumulation and may be associated with underlying epithelial mucinous neoplasms. We present the case of a 45-year-old man with obesity who presented with a four-month history of intermittent right-lower-quadrant abdominal pain, occasional abdominal distension, and nonspecific changes in bowel habits. Physical examination revealed localized tenderness without signs of peritoneal irritation, and laboratory findings were within normal limits. Abdomina

[A Case of Early Appendiceal Adenocarcinoma Coexisting with High-Grade Appendiceal Mucinous Neoplasm].

We report a case of a female in her fifties with early appendiceal adenocarcinoma coexisting with high-grade appendiceal mucinous neoplasm(HAMN)with a review of the literature. The patient presented to our hospital because of an enlarged appendix noted by contrast-enhanced CT performed for hematuria. Contrast-enhanced CT showed that the appendix had swollen to 10 mm and mucus had accumulated inside, which had no evidence of obvious malignancy. She was followed up on CT once a year. Four years after her first visit, she underwent laparoscopic appendectomy for a definitive diagnosis. There were

Intestinal metaplasia of appendiceal endometriosis is not uncommon and may mimic appendiceal mucinous neoplasm.

Endometriosis of the appendix can be an incidental finding or a cause of appendicitis, intussusception, perforation or retention mucocele. Intestinal metaplasia of appendiceal endometriosis may occur, which can lead to a misdiagnosis of low-grade appendiceal mucinous neoplasm. On a retrospective search of the pathology database from 2001 to 2015, we identified 78 appendiceal endometriosis cases and intestinal metaplasia was present in 10/78 (13%) cases. In most of the cases (90%), the foci of intestinal metaplasia were mainly localized close to the mucosa. Intestinal and endometrial hybrid gla

Risk of appendiceal neoplasm in patients with appendix disorders.

The most common appendicular disease is acute appendicitis, with a lifetime risk of 7%-8%. Complicated cases, which can occur in 2%-7% of patients, can significantly impact the severity of the condition and may require different management approaches. Nonoperative management with possible delayed appendectomy has been suggested for selected patients, however, there is a non-negligible risk of missing an underlying malignancy, which is reported to be as high as 11%. Diagnostic work-up is paramount to achieve optimal treatment with good results.

[A Case of High-Grade Appendiceal Mucinous Neoplasm].

A 70-year-old woman underwent conservative treatment for abscess-forming appendicitis. A contrast-enhanced abdominal computed tomography(CT)revealed a cystic lesion at the appendiceal base while the abscess had disappeared posttreatment. With the diagnosis of a low-grade appendiceal mucinous neoplasm(LAMN), a laparoscopic-assisted ileocolic resection was performed. The appendix was distended with mucus in the lumen. Histopathological examination showed that the tumor cells were more atypical than that of low-grade appendiceal mucinous neoplasm. There were no findings of adenocarcinoma, such as

Appendiceal pathology at the time of oophorectomy for ovarian neoplasms.

OBJECTIVE: To investigate the prevalence of appendiceal pathology in women undergoing surgery for a suspected ovarian neoplasm and the predictive value of intraoperative findings to determine the need for appendectomy at the time of surgery. METHODS: Retrospective analysis of patients who underwent oophorectomy and appendectomy during the same surgical procedures at the University of Virginia Health System from 1992 to 2007. Observations were stratified based on the nature (benign, borderline, or malignant) and histology (serous compared with mucinous) of the ovarian neoplasm, frozen compared

Post-inflammatory mucosal hyperplasia and appendiceal diverticula simulate features of low-grade appendiceal mucinous neoplasms.

Post-inflammatory mucosal hyperplasia and appendiceal diverticulosis simulate mucinous neoplasms, causing diagnostic confusion. Distinction between neoplasia and its mimics is particularly important since many authorities now consider all appendiceal mucinous neoplasms to be potentially malignant. The purpose of this study was to identify clinicopathologic and molecular features that may distinguish appendiceal mucinous neoplasms from non-neoplastic mimics. We retrospectively identified 92 mucinous lesions confined to the right lower quadrant, including 55 non-neoplastic examples of mucosal hy

Incidental High-Grade Appendiceal Mucinous Neoplasm.

BACKGROUND: High-grade appendiceal mucinous neoplasm (HAMN) is an uncommon epithelial neoplasm of the appendix, and it is a median risk lesion that can be placed between benign low-grade appendiceal mucinous neoplasm (LAMN) and malignant adenocarcinoma. The diagnosis can be both incidental during surveillance imaging or clinically similar to acute appendicitis, which presents as pain in the right lower quadrant, or local pressure on adjacent organs. It carries the potential to progress to cancer if left untreated. Our aim is to review the literature and to report this rare tumor with outlining

Appendiceal Mucinous Neoplasm: A Case of an Incidental Finding.

Appendiceal mucinous neoplasms (AMNs) are uncommon gastrointestinal tumors characterized by mucus accumulation in the appendix. Patients may complain of acute appendicitis-like symptoms with other alarming features, but approximately half of the cases of AMNs are found incidentally on imaging. Early diagnosis and management of these neoplasms are important to prevent malignant progression and complications such as bowel obstruction and pseudomyxoma peritonei. We report a case of a 28-year-old female who initially presented with vomiting and acute left lower abdominal pain radiating to the left

Appendiceal neuroendocrine neoplasms in the era of laparoscopic appendicectomy.

BACKGROUND: Incidental appendiceal neuroendocrine neoplasms are identified in 1% of surgical resections for suspected appendicitis. A proportion of these patients will require further surgery because of high risk features, which include mesoappendiceal involvement or an involved margin. While an open appendicectomy technique usually involves en bloc mesoappendiceal resection, the increasingly common laparoscopic method often skeletonizes the appendix, leaving the mesoappendix in situ. This retrospective observational study investigates whether routine resection of the mesoappendix rather than
