Prenatal diagnosis of arachnoid cysts.

Arachnoid cysts are a rare central nervous system malformation, representing only 1% of all intracranial masses in newborns. Primary (congenital) arachnoid cysts are benign accumulation of clear fluid between the dura and the brain substance throughout the cerebrospinal axis in relation to the arachnoid membrane and do not communicate with the subarachnoid space. Secondary (acquired) arachnoid cysts result from hemorrhage, trauma, and infection and usually communicate with the subarachnoid space. The common locations of arachnoid cysts are the surface of the brain at the level of main brain fi

Neuroendoscopic approach to arachnoid cysts.

A prospective study of seven consecutive patients with congenital arachnoid cysts treated endoscopically is reported. The ages of the patients at the time of diagnosis ranged from 6 to 47 years with three patients under 15 years. Two cysts were located in the posterior cranial fossa, four in the middle cranial fossa, and one in the suprasellar-prepontine area. The patients' symptoms included headache, seizures, vomiting, nausea, dizziness, balance problems, and precocious puberty. The authors performed cystocisternostomies and ventriculocystostomies via burr holes with the aid of a universal n

Arachnoid cyst slit valves: the mechanism for arachnoid cyst enlargement.

Arachnoid cysts are common, accounting for approximately 1% of intracranial mass lesions. Most are congenital, clinically silent, and remain static in size. Occasionally, they increase in size and produce symptoms due to mass effect or obstruction. The mechanism of enlargement of arachnoid cysts is controversial. One-way slit valves are often hypothesized as the mechanism for enlargement. The authors present 4 cases of suprasellar prepontine arachnoid cysts in which a slit valve was identified. The patients presented with hydrocephalus due to enlargement of the cyst. The valve was located in t

A Comprehensive Review of Arachnoid Cysts.

Arachnoid cysts are cerebrospinal fluid (CSF)-filled sacs that develop within the arachnoid membrane surrounding the brain or spinal cord, often remaining asymptomatic but occasionally causing neurological symptoms due to mass effect or CSF flow obstruction. The optimal management of these cysts remains debated, balancing conservative observation with surgical intervention. This review analyzes the epidemiology, pathophysiology, clinical presentation, diagnostic strategies, treatment options, and future management directions for arachnoid cysts. With a prevalence of approximately 1-2% and a ma

Surgical indications in pediatric arachnoid cysts.

INTRODUCTION: Arachnoid cysts are developmental lesions consisting of CSF collections within the subarachnoid space. There are many theories and hypotheses about their pathogenesis and histopathology and this may also explain the diversities seen in clinical behaviors of these cysts, their natural history and consequently their management where there is a great controversy about selecting patients for surgical intervention. The most common location in pediatrics is the Sylvian or middle cranial fossa and this made it gain more concern and greater debate about its management where its diagnosis

Temporal arachnoid cysts: are they congenital?

CASE REPORT: The authors report two cases of arachnoid cysts (ACs) neither detected during pregnancy nor shortly after birth when newborns underwent CT scan evaluation after birth head trauma. ACs were diagnosed at 10 months and 6 years, respectively. The first one becomes symptomatic, and the other one was incidentally found during a head trauma investigation. DISCUSSION: These cases give support for the postnatal pathogenesis for some of the assumed congenital ACs. We collected data from the literature that supports the acquired hypothesis for ACs.

Arachnoid cysts in tuberous sclerosis complex.

OBJECTIVE: Some clinical findings in tuberous sclerosis complex (TSC), such as hypomelanotic macules or angiofibromas are related to problems in development of the neural crest, which is also the origin of cranial leptomeninges. Arachnoid cysts have been reported in two TSC patients to date. The purpose of this study was to assess the prevalence and characteristics of arachnoid cysts in a large cohort of TSC. MATERIALS AND METHOD: We performed a review of brain MRIs of 220 TSC patients searching for arachnoid cysts. RESULTS: Arachnoid cysts were found in 12 (5.5%) (general population: 0.5%),

[Arachnoid cysts and epilepsy in children].

BACKGROUND: At neurodevelopment age arachnoid cysts (ac) are commonly found by accident in neuroimaging examinations performed in the process of differential diagnostics of different diseases, including epilepsy. They are neurodevelopmental disorders which usually develop in the fetal life. Ac are usually supratentorial, less often subtentorial and sporadically interstellar. They are often the only structural lesions of the brain found in the child with epilepsy. There is still lack of univocal statement whether presence of ac determines type of seizures and focal paroxysmal changes in EEG rec

Cystoventricular shunting of intracranial arachnoid cysts.

Ten patients with intracranial arachnoid cysts were treated with direct shunting of the cyst to a lateral ventricle. The strategic goal of cystoventricular shunting is to establish physiologically normal intracranial pressure relationships, rather than cyst obliteration. Cystoventricular shunts were successful in treating single and multiple intracranial cysts in supratentorial and infratentorial locations and in patients with normal and enlarged lateral ventricles. Cystoventricular shunting is conceptually simple as well as effective and reliable.

Intradural spinal arachnoid cysts in adults.

BACKGROUND: Idiopathic arachnoid cysts are rare lesions not associated with trauma or other inflammatory insults. To date, there have been few large series describing the presentation and management of these lesions. METHODS: Twenty-one cases of intradural spinal arachnoid cysts were identified (1994-2001). Pediatric patients and cases with antecedent trauma were excluded. There were eight women and 13 men with an average age of 52 years. Follow-up averaged 17 months. RESULTS: Cysts were most commonly found in the thoracic spine (81%). Fifteen cysts were dorsal to the spinal cord and six wer

Arachnoid cysts of the posterior fossa.

BACKGROUND: The surgical indications and management of posterior fossa arachnoid cysts (AC) are still controversial. Different surgical techniques and management have already been suggested for arachnoid cysts of the posterior fossa. AC involving the posterior fossa and especially the cerebellopontine angle may carry a high surgical morbidity because of the involvement of important neurovascular structures (e.g., brain stem and cranial nerves). Only long-term follow-up will determine the best surgical technique for such lesions. METHODS: Between 1990 and 1996 a total of 12 patients underwent

[Arachnoid cysts of the cerebellopontine angle].

The authors report two cases of arachnoid cysts of the cerebello-pontine angle. The otologic symptoms were unsteadiness, hearing fall and tinnitus. In the first case, the patient who presented a cerebellar syndrome was operated. Afterwards the hearing felt and he developed a transient hydrocephalus. The symptoms disappeared in 9 months. In the second cas, the patient was not operated. She was treated medically and supervised. Then the symptoms disappeared too. The authors review the paraclinic exams especially MR, relevant to the diagnosis and discuss the opportunity of a surgical operation.

[Supratentorial extra-axial arachnoid brain cysts].

Supratentorial extra-axial arachnoidal cysts are congenital benign formations of intracranial location filled with cerebrospinal fluid. The clinical symptomatology of arachnoidal cysts depended on the age of the patient and presence of complications. The pathogenetically justified surgical method of treatment of arachnoidal cysts in children is excision of cyst wall, emptying of its cavity, creation of shunt with the subarachnoidal spaces and basal cisternae in association with cystoperitoneostomy. In the adult patients additional cystoperitoneotomy is not performed.

Protruding arachnoid granulations mimicking dermoid cysts.

This is the report of two children who had prominent arachnoid granulations that were mistaken clinically for dermoid cysts. These lesions appeared in the midline, primarily posteriorly. During surgery, the lesions extended through the dura connected by a stalk. A pathologic examination confirmed the lesions to be arachnoid granulations.

MR imaging of intracranial arachnoid cysts.

The magnetic resonance characteristics of 16 intracranial arachnoid cysts were evaluated and several features identified that allowed the differentiation from other cystic lesions. Uncomplicated arachnoid cyst contents respond like CSF to both T1 and T2 weighted sequences. Signal from the contiguous brain is of normal intensity. The absence of signal from cortical bone and the ease of obtaining multiplanar views enable the margins of the cyst, its contents, and the full extent of the lesion to be easily defined.

Suprasellar arachnoid cysts: 1. CT recognition.

Suprasellar arachnoid cysts are basal midline masses that represent a rare but surgically remediable cause of hydrocephalus and neurologic deficits. These cysts represent a diagnostic challenge and often go unrecognized for many years. The authors review the computed tomographic (CT) findings in seven patients with documented suprasellar arachnoid cysts and define previously undescribed diagnostic criteria. These cysts usually can be differentiated from cystic midline neoplasms by their CT density, homogeneity, and location as well as by their lack of fat, lack of calcification, and absence of

Purely Endoscopic Treatment for Arachnoid Cysts.

Arachnoid cysts are benign, mostly congenital lesions that are asymptomatic in most patients. In some cases, due to their location or sheer size, they produce a mass effect or hydrocephalic obstruction of the cerebrospinal fluid (CSF) flow and thus might warrant surgical treatment. The goal of the surgery is usually to reduce pressure inside the cysts, to reduce the mass effect, or to restore the CSF pathway. Surgical treatment options are resection, fenestration, or shunting of the cyst. Over the past decades, treatment under sheer endoscopic control either through a tube or via craniotomy of

Surgical treatment of interhemispheric arachnoid cysts.

OBJECTIVE: In children, interhemispheric arachnoid cysts (IHACs) are rare lesions often associated with corpus callosum dysgenesis. It is still controversial about surgical treatments for IHACs. We aim to report our experience with pediatric IHAC patients and evaluate surgical courses and neurological developments. METHODS: Pediatric IHACs treated between 2001 and 2021 were reviewed retrospectively. IHAC was observed until they represented rapid cyst enlargement or neurological symptoms. Cyst fenestration was done by microscope or endoscope, depending on the IHAC's location. Cyst size and cor

Natural History of Intracranial Arachnoid Cysts.

BACKGROUND: Intracranial arachnoid cysts are a common incidental finding. Most of these remain asymptomatic with surgical intervention being reserved for those cysts causing symptoms from mass effect or hydrocephalus. It is not properly understood why cysts enlarge nor is it possible to predict in which cysts this will occur. The aim of this study is to review the natural history of incidental arachnoid cysts. METHODS: A retrospective case review was conducted of all patients diagnosed with an arachnoid cyst between 2007 and 2016. Case notes and radiology reports were reviewed for demographic
