Arcanobacterium hemolyticum infection: confused with scarlet fever and diphtheria.

Arcanobacterium hemolyticum infections are a common cause of pharyngitis and rash in the 10- to 30-year-old age group. Despite its prevalence, many emergency and primary care physicians may not be aware of the pathogenic potential of this organism. We present a case that illustrates the distinctive clinical spectrum of A. hemolyticum infections that may be confused with drug allergy, group A streptococcal scarlet fever, diphtheria, and even toxic shock syndrome. Recognition of this syndrome will reduce misdiagnoses and facilitate appropriate treatment.

Properties of an Arcanobacterium haemolyticum strain isolated from a donkey.

The present study was designed to characterize phenotypically and genotypically an Arcanobacterium haemolyticum strain (A. haemolyticum P646) isolated from a purulent nasal discharge of a donkey. A. haemolyticum P646 showed, compared to sheep blood, an enhanced hemolytic reaction on rabbit blood agar, a synergistic CAMP-like reaction with Streptococcus agalactiae and Rhodococcus equi as indicator strains, a reverse CAMP reaction in the zone of Staphylococcus aureus beta-hemolysin and the typical biochemical properties of this species. The species identity could be confirmed by MALDI-TOF MS ana

Necrotising fasciitis in a bull due to infection with Arcanobacterium haemolyticum.

CASE HISTORY: A 2-year-old Hereford bull was lame for one week before becoming recumbent. CLINICAL AND PATHOLOGICAL FINDINGS: The scrotum and ventral perineal region were cold and blackened caudally. The semimembranosus and semitendinosus muscles were firm on palpation. The bull was subject to euthanasia, and necropsy revealed that the skin and S/C tissues of the caudal half of the scrotum were grey and necrotic. The caudal and distal aspects of the semimembranosus and semitendinosus muscles were grey and necrotic to a depth of approximately 15 cm, and these changes appeared to track along fa

Antimicrobial Resistance in Corynebacterium spp., Arcanobacterium spp., and Trueperella pyogenes.

There is currently only limited information on the antimicrobial susceptibility and resistance of Corynebacterium spp., Arcanobacterium spp., and Trueperella pyogenes from animals. The comparability of the data is hampered by the use of different antimicrobial susceptibility testing methods and interpretive criteria. To date, standard broth microdilution methods and clinical breakpoints that are approved by the Clinical and Laboratory Standards Institute and are applicable to Corynebacterium spp., Arcanobacterium spp., and T. pyogenes are available. The lack of species-specific clinical breakp

Soft tissue infections with Arcanobacterium haemolyticum: report of three cases.

We report here three polymicrobial wound infections associated with Arcanobacterium haemolyticum in rural patients aged between 60-65 years. In two patients, one with cellulitis and the other with postoperative wound infection following amputation of the limb, Arcanobacterium haemolyticum was isolated repeatedly along with beta haemolytic streptococci (BHS). The BHS belonged to Lancefield's group G and group C respectively. In another patient, who was a diabetic with chronic osteomyelitis, Arcanobacterium haemolyticum was isolated along with Proteus vulgaris . All the three isolates of Arcanob

Antimicrobial susceptibility of Arcanobacterium pyogenes isolated from cattle and pigs.

The minimum inhibitory concentrations (MICs) of 18 antimicrobial agents were determined for 49 Arcanobacterium pyogenes isolates (42 bovine isolates and 7 porcine isolates). Benzylpenicillin and ampicillin were the most active antibiotics, with MIC ranges of < or = 0.0125-0.05 microgram/ml for both bovine and porcine isolates. All isolates were susceptible to penicillins and cephems. MICs for 90% of the isolates of dihydrostreptomycin, gentamicin and oxytetracycline for bovine isolates were > 100 micrograms/ml, 1.56 micrograms/ml and 25 micrograms/ml, respectively. More resistance to dihydrost

[A frequently overlooked bacteria in clinical microbiology laboratories: Arcanobacterium haemolyticum].

Arcanobacterium haemolyticum, previously known as Corynebacterium haemolyticum, is a facultative anaerobic, gram-positive bacillus with negative catalase and positive CAMP inhibition test results. It may be the causative agent of about 0.5-3% of acute bacterial pharyngitis especially in children and young adults. Since growth of A.haemolyticum is usually inhibited by flora members and since it slowly develops hemolysis in sheep blood agar and its colony morphology resembles beta-hemolytic streptococci, it is frequently overlooked in the evaluation of throat cultures. The aims of this study wer

[Multiple peritonsillar abscesses caused by Arcanobacterium haemolyticum in a young female].

HISTORY: A 20-year-old previously healthy woman presented with a four-week history of sore throat and inability to swallow. INVESTIGATION AND DIAGNOSIS: Initial examination revealed tender swelling and exudation of both palatine tonsils, trismus and bilateral cervical lymphadenopathy. Laboratory tests showed leukocytosis, elevated C-reactive protein, but unremarkable Epstein-Barr virus titers. COURSE AND TREATMENT: The patient was unsuccessfully treated for three days with topical antiseptics and systemic cefuroxime. Cervical magnetic resonance tomography (MRT) disclosed multiple peritonsill

Evaluation of a commercial kit in the identification of Arcanobacterium haemolyticum and Actinomyces pyogenes.

Listeria-Zym is a commercial kit designed to identify Listeria spp. within 4 h. Its ability to identify two clinically important aerobic catalase-negative, gram-positive rods, Arcanobacterium haemolyticum and Actinomyces pyogenes, was evaluated. Xylose fermentation and alpha-mannosidase tests differentiated Arcanobacterium haemolyticum (n = 49) from Actinomyces pyogenes (n = 24) strains.

Arcanobacterium haemolyticum in children with presumed streptococcal pharyngotonsillitis or scarlet fever.

Arcanobacterium haemolyticum was cultured from pharyngeal specimens obtained from 12 of 129 children with pharyngotonsillitis, some of whom had a scarlatiniform rash. This organism should be considered to be a cause of infections that are clinically similar to those caused by beta-hemolytic streptococci.

Insights into Arcanobacterium haemolyticum: A Narrative Review of an Emerging Pathogen Revisited.

Arcanobacterium haemolyticum is a facultative anaerobic, Gram-positive bacillus that has garnered attention due to its role in human infections, particularly among adolescents and young adults. Traditionally associated with pharyngitis, this organism is increasingly recognized for its involvement in systemic infections, including bacteremia, central nervous system abscesses, and Lemierre's syndrome. The pathogenicity of A. haemolyticum is attributed to its production of hemolysins and neuraminidase, facilitating tissue invasion and immune evasion. Clinically, infections often present with sore

The burden of Arcanobacterium haemolyticum pharyngitis: A systematic review and management algorithm.

INTRODUCTION: Arcanobacterium haemolyticum causes pharyngotonsillitis in children and young adults. It is rarely isolated in pharyngeal swabs as testing for it is not routine. Data on complications, management, and antibiotic susceptibility testing is limited. We sought to review the available literature on the presentation and management of A. haemolyticum pharyngotonsillitis in this age group. METHODS: A systematic review of eligible studies reporting pharyngotonsillitis and related complications in children and young adults caused by A. haemolyticum was conducted. Literature from case repo

Hemorrhagic necrotizing splenitis in a slaughter pig infected with Arcanobacterium species.

A 6-month-old barrow presented with lethargy, inappetence and dysstasia. At necropsy, multiple coalescing hemorrhagic foci were detected in the margins of the spleen. Gram-positive bacilli were isolated from the spleen, kidney, muscle and liver. Comparative 16S rDNA gene sequencing analysis of the isolates (TO16177) revealed that they would be the same species of unpublished Arcanobacterium species strain HJ57-14E (accession no. gi 18873551) (99.7% similarity based on a comparison of 675 bp). Histologic examination of the splenic tissue sections revealed extensive necrosis and inflammation, an

Arcanobacterium haemolyticum pharyngitis and exanthem. Three case reports and literature review.

BACKGROUND: Arcanobacterium haemolyticum is a bacterial pharyngeal pathogen that infects adolescents and young adults, frequently causing an exanthem that may mimic a viral exanthem, toxic erythema, or drug eruption. To our knowledge, the cutaneous manifestations of A haemolyticum infection have not previously been reported in the dermatologic literature. OBSERVATIONS: The severity of this infection can range from mild pharyngitis to a diphtheria-like illness and even septicemia. We report three cases that demonstrate the spectrum of manifestations of this disease, including an unusual case w
