INTRODUCTION: Endometriosis is a benign chronic inflammatory disease responsible for debilitating pain and decreased quality of life. The traditional treatment is based on estroprogestins, progestins, GnRH analogs, and surgery. Recently, aromatase inhibitors (AIs) demonstrated good efficacy in controlling symptoms and size of endometriotic implants, mostly because they suppress extraovarian estrogen synthesis, which can enhance the hypoestrogenic state ameliorating symptoms of endometriosis.

PURPOSE OF REVIEW: The aromatase inhibitors are increasingly used as adjuvant therapy in postmenopausal women with hormone receptor positive breast cancer. With additional experience using these agents, unanticipated side effects have become apparent. Women who experience side effects from adjuvant endocrine therapy are the individuals who derive the greatest benefits. Aromatase inhibitors (AI) increase the disease-free survival of patients with receptor-positive breast cancer. They inhibit the endogenous production of estrogen by 50-90% and, in contrast to tamoxifen, reduce bone mineral density. Placebo-controlled studies have lacked statistical power to detect changes in fracture incidence; however, AI increases the incidence of fractures in comparison with tamoxifen. Therefore, we suggest that post-menopausal women starting on AI should be offered a DXA scan and that antiresorptive therapy should be considered for those with osteoporosis.

Tamoxifen and GnRH analogues (GnRHa) represent the mainstay of endocrine manipulations in premenopausal women. In recent years the emerging importance of estrogen signaling in males in addition to its major role in the female reproductive system became highlighted. Aromatase is the key enzyme for synthesis of estrogens from androgens and is responsible for controlling the androgen/estrogen ratio. Inhibition of aromatase gene expression can be achieved in different ways and is important for the treatment of several estrogen-dependent diseases, such as breast cancer in females or gynecomastia/breast cancer in males, or for non-tumorigenic conditions like precocious puberty and the induction of ovulation.

PURPOSE OF REVIEW: Approximately 15% of the infertile population faces an ovulatory disorder; most of them classified as WHO class two anovulation [polycystic ovarian syndrome (PCOS) prototype]. BACKGROUND: Aromatase inhibitors have recently been proposed for the treatment of endometriosis; however, no previous study examined the effects of these agents on pain and urinary symptoms of premenopausal women with bladder endometriosis.
CASE: Two premenopausal patients with bladder endometriosis were treated with letrozole (2.5 mg/day), norethisterone acetate (2.5 mg/day), elemental calcium and vitamin D3 for 6 months. The double-drug regimen quickly improved pain and urinary symptoms in both patients. One patient had no significant adverse effect and continued the therapy for 14 months.

Aromatase inhibitors (AIs) were originally developed for the treatment of advanced breast cancer in postmenopausal women. Their use in reproductive medicine has been extensively studied in the past decade. We reviewed the current strategies for ovulation induction for anovulatory women, mostly women with polycystic ovarian syndrome (PCOS), and the scientific basis for use of AIs in reproductive medicine. The AI, letrozole, is effective in ovulation induction in women with PCOS resistant to clomifene citrate and ovarian stimulation for intrauterine insemination and in vitro fertilization (IVF).
