Mesenteric artery occlusive disease.

The age and advanced stage of atherosclerosis in this patient population require careful preoperative evaluation and attention to detail in the perioperative period in an effort to avoid complications in other organ systems resulting from diffuse occlusive disease. The keys to accurate diagnosis and successful management of patients with acute or chronic mesenteric ischemia include a detailed history, focusing on the quality and temporal relation of the symptoms; an accurate vascular assessment on physical examination, with attention directed to ruling out nonvascular causes of the symptoms; a

Carotid artery occlusive disease.

Carotid disease is a major contributor to stroke, one of the leading causes of death and disability in the United States. Clinically significant stenosis can be detected by duplex ultrasound using well-established criteria. In addition to optimal medical management, surgical and endovascular revascularizations of carotid disease have been demonstrated to be effective interventions that reduce the risk of stroke in properly selected patients.

Basal arterial occlusive disease.

Seventeen patients with basal occlusive disease have been seen over the past 4 years. Nine of these had a classical moyamoya appearance on angiography. Of these nine, seven were adults and two were children. Eight had ischemic episodes, and one had a hemorrhage. One had neurofibromatosis with a chiasmal glioma and had received radiotherapy. Eight patients underwent superficial temporal-middle cerebral artery (STA-MCA) bypass, and one refused operation. Six patients improved, one had a stroke on the opposite side, and one died. Eight patients had unilateral basal arterial occlusive disease. Of

Anterior spinal artery as a collateral channel in cases of bilateral vertebral arterial steno-occlusive diseases.

BACKGROUND AND PURPOSE: We report 6 cases of retrograde flow through the anterior spinal artery (ASA) from cervical vertebral artery (VA) to intracranial distal VA because the perfusion from bilateral vertebral arteries was tenuous. Its hemodynamic and clinical implications are discussed. METHODS: In association with bilateral steno-occlusive disease of vertebral arteries, 6 cases of retrograde flow through ASA were reviewed in terms of clinical and angiographic characteristics. All 6 patients presented with stroke in the posterior fossa and underwent conventional angiography as part of diagn

[Whole blood viscosity in patients with acral arterial occlusive diseases and their modification by rheologically active drugs].

Measurements of the whole-blood viscosity in patients with organic and functional disturbances of the acral blood supply were performed. In comparison with the whole-blood viscosity in normal persons patients with acral occlusive diseases showed a significantly increased viscosity. On the other hand, in patients with functional disturbances of the acral blood supply no differences could be established. In a therapy study the good efficacy of rheologically effective medicaments could be demonstrated particularly as combination therapy of pentoxifylline, acetylsalicylic acid and tolazoline in pa

Occlusive arterial diseases of the upper and lower extremities found in workers occupationally exposed to vibrating tools.

Hand-arm vibration syndrome (HAVS) is primarily a disorder of the fingers and hands. However, in some cases, vibration-exposed workers are observed to have also episodic blanching of the hands and feet. In latter cases, arteriographies of both the upper and lower extremities are necessary to diagnose the background arterial disorders. In this study, eight HAVS subjects with such disorders were examined by arteriography for differential diagnosis in cases of workers' accident compensation. In three HAVS cases with thromboangiitis obliterans, the arteriographic examination revealed obstructive c

[Results of in situ arterial thrombolysis by the combination of urokinase and lysyl plasminogen in acute arterial occlusive diseases of the lower limbs].

35 patients with acute arterial occlusions [27] and graft thromboses [8], responsible for severe and recent ischemia, were treated by fibrinolytic therapy (Urokinase: 1 000 units/kg/hour, and Lys Plasminogen). These drugs were delivered at the site of occlusions using a 5 French catheter. Angiographically, initial success was obtained in 30 patients (85%) and a significant clinical benefit persisted 5 months later, in 20 patients (57%). 4 distal embolisms during the treatment were noted, and one woman died a few hours after the withdrawal of an axillary catheter of a cerebellar infarction. Arterial reconstructions for pediatric splanchnic artery occlusive disease.

OBJECTIVE: Pediatric splanchnic arterial occlusive disease is uncommon and a rare cause of clinically relevant intestinal ischemia. This study was undertaken to better define the clinical manifestations and appropriate treatment of celiac artery (CA) and superior mesenteric artery (SMA) occlusive disease in children. METHODS: Clinical courses of 30 consecutive children undergoing operations for splanchnic arterial occlusive disease at the University of Michigan from 1992 to 2017 were retrospectively analyzed. RESULTS: Vascular reconstructions were performed for splanchnic arterial disease in

OCULAR ARTERIAL OCCLUSIVE DISORDERS AND CAROTID ARTERY DISEASE.

OBJECTIVE: To compare prevalence of carotid artery disease and its various types of lesions in different types of ocular arterial occlusive disorders. DESIGN: Cohort study. SUBJECTS: 614 consecutive patients (728 eyes) with ocular arterial occlusive disorders. METHODS: At first visit, all patients had a detailed ophthalmic and medical history, comprehensive ophthalmic evaluation, and carotid artery evaluation (by Doppler/angiography) on the side of ocular arterial occlusion, and echocardiography. The same ophthalmic evaluation was performed at each follow-up visit. Ocular arterial occlusive

Carotid artery disease in patients with retinal artery occlusion.

Embolization from the carotid bifurcation has been proposed as the most common cause of central retinal artery occlusion (CRAO) and branch retinal artery occlusion (BRAO). The purpose of this study was to evaluate carotid artery disease in patients with CRAO and BRAO. Using carotid ultrasonography, 17 patients (13 males, 4 females, mean age 68.7 +/- 7.1 years) with CRAO and BRAO were examined for stenotic findings and plaque morphology (homogeneous or heterogeneous) within 7 days after onset. The internal carotid artery (ICA) ipsilateral to the affected side showed a significantly higher incid

Large Artery Atherosclerotic Occlusive Disease.

PURPOSE OF REVIEW: Extracranial or intracranial large artery atherosclerosis is often identified as a potential etiologic cause for ischemic stroke and transient ischemic attack. Given the high prevalence of large artery atherosclerosis in the general population, determining whether an identified atherosclerotic lesion is truly the cause of a patient's symptomatology can be difficult. In all cases, optimally treating each patient to minimize future stroke risk is paramount. Extracranial or intracranial large artery atherosclerosis can be broadly compartmentalized into four distinct clinical sc

[Acute peripheral arterial occlusive disease].

Acute lower limb ischemia is not infrequently associated with limb loss 10-30%) or death of the affected patient (15-30%). Thus, acute lower limb ischemia is an emergency. There are two categories of acute arterial occlusion: embolism (70-80%) and thrombosis (20-30%). Early diagnosis is a prerequisite for optimal management. Initial clinical examination is the crucial point: it makes the diagnosis of acute ischemia, localizes the level of occlusion, collects etiologic information and allows for assessing the local and general impact of ischemia on the patient. If sensation and motor function a

[Rheology and occlusive arterial disease of the legs].

Peripheral occlusive arterial disease (P.O.A.D.) is one of the situations in which hemorheological abnormalities are usually described. However the clinical relevance of hemorheological measurements in angiologic practice remains to be defined. The aim of the study was to investigate whether hemorheological disturbances are associated with alterations in oxygen diffusion and prognosis of the arterial disease. Three groups of patients were included in this work. First, a study was realized on 160 nondiabetic P.O.A.D patients (suffering from intermittent claudication to critical limb ischemia) i

Arterial occlusive disease of the upper extremity.

Twenty-five upper extremities were operated upon in 24 patients because of arterial occlusive disease. Transthoracic endarterectomy was performed in 4 patients with short central occlusions of the subclavian artery. In the remaining patients, various bypass procedures were performed using reversed autogenous veins or prosthetic grafts. There were 2 early and 6 late deaths. One prosthetic graft had to be removed 2 months after the operation because of infection. At follow-up, 9 months to 17 years after surgery, 11 bypass grafts were patent. One vein graft and one prosthetic graft had occluded.
