Office Blood Pressure and Arteriolosclerosis in Biopsy-Proven Glomerulonephritis: Association With Renal Function Decline and Clinical Outcomes.

Hypertension is a major determinant of chronic kidney disease progression, yet the relationship between standardized office blood pressure measurements (OBPM), chronic vascular lesions, and prognosis in biopsy-proven glomerulonephritis (GN) remains poorly defined. We investigated the association between OBPM and renal histopathological lesions, focusing on arteriolosclerosis, and assessed their prognostic significance. This retrospective longitudinal study included 66 adults with biopsy-proven GN. All patients underwent standardized OBPM, laboratory evaluation, and renal biopsy. Histopathologi

The impact of arteriolosclerosis on cognitive impairment in decedents without severe dementia from the National Alzheimer's Coordinating Center.

INTRODUCTION: Alzheimer's disease neuropathologic change (ADNC), Lewy body disease (LBD), and vascular neuropathologies occur together. Previous studies have been limited by a large majority of participants with severe dementia or advanced stages of pathologies, which limits the detectability of cognitive effects from vascular neuropathologies. METHODS: Using neuropathology data from the National Alzheimer's Coordinating Center, we examined the association of vascular neuropathologies with cognitive scores in participants without severe dementia (N = 1526) using multivariable linear regressio

Differential correlations of changes in in vivo neuroimaging markers of hippocampal volume and arteriolosclerosis with declining financial and health literacy in old age.

Financial and health literacy is essential for older adults to navigate complex decision processes in late life. However, the neurobiological basis of age-related decline in financial and health literacy is poorly understood. This study aimed to characterize progression of neurodegenerative and vascular conditions over time, and to assess how these changes coincide with declining financial and health literacy in old age. Data came from 319 community-living older adults who were free of dementia at baseline, and underwent annual literacy assessments, as well as biennial 3-Tesla neuroimaging sca

Incorporation of Retinal Arteriolosclerosis into Risk Stratification of Blood Pressure Category According to the 2017 ACC/AHA Blood Pressure Guideline.

AIM: We investigated whether retinal arteriolosclerosis (RA) could be used for cardiovascular disease (CVD) risk stratification of individuals categorized according to the 2017 American College of Cardiology (ACC)/American Heart Association (AHA) Blood Pressure (BP) guideline. METHODS: We studied 291,522 participants without a history of CVD and not taking any BP-lowering medications from the JMDC Claims Database. RA was defined as Keith-Wagener-Barker system grade ≥ 1. Each participant was classified into one of the six groups: (1) normal or elevated BP without RA, (2) normal or elevated BP

VCING Scoring and White Matter Arteriolosclerosis Show Relationships to Dementia Status and White Matter Damage in an Unselected Ageing Brain Cohort.

INTRODUCTION: Vascular disease is associated with late-life dementia and other morbidities during life and commonly coexists with neurodegenerative pathologies. A complex variety of lesions can be documented, which has made development of standardised operationalised criteria difficult. A semiquantitative approach, vascular cognitive impairment neuropathology guidelines (VCING), has been developed that provides a score based on assessment of occipital white matter arteriolosclerosis, occipital leptomeningeal cerebral amyloid angiopathy (CAA) and the presence of any macroscopic infarct. Reconsideration of the DTI-ALPS index as a glymphatic surrogate: disease-specific findings in small vessel disease due to cerebral amyloid angiopathy or arteriolosclerosis.

OBJECTIVES: To explore the disease-specific performance of diffusion tensor imaging analysis along the perivascular space (DTI-ALPS) index in cerebral amyloid angiopathy (CAA) and arteriolosclerosis CSVD (aCSVD). METHODS: We retrospectively enrolled probable CAA or aCSVD patients who underwent 5.0T MRI. Disease-specific DTI-ALPS characteristics and the correlations with clinical, imaging markers, and cognition were evaluated. Regression models were applied according to outcome distributions. Multiple comparisons were controlled using the Benjamini-Hochberg false discovery rate (FDR) procedure

Association of small vessel disease with tau pathology.

Emerging evidence suggests that small vessel disease (SVD) is a risk factor for clinical dementia and may contribute to AD neuropathological changes. Watershed brain regions are located at the most distal areas between arterial territories, making them vulnerable to SVD-related changes. We examined the association of pathologic markers of SVD, specifically arteriolosclerosis in watershed brain regions, with AD pathologic changes. Participants (N = 982; mean age-at-death = 90; 69% women) were enrolled as part of one of two cohort studies of aging and dementia. At autopsy, neuropathological eval

Pathological Changes of Small Vessel Disease in Intracerebral Hemorrhage: a Systematic Review and Meta-analysis.

In intracerebral hemorrhage (ICH) with pathology-proven etiology, we performed a systematic review and meta-analysis to elucidate the association between cerebral amyloid angiopathy (CAA) and arteriolosclerosis, and directly compared MRI and pathological changes of markers of cerebral small vessel disease (CSVD). Studies enrolling primary ICH who had received an etiological diagnosis through biopsy or autopsy were searched using Ovid MEDLINE, PubMed, and Web of Science from inception to June 8, 2022. We extracted pathological changes of CSVD for each patient whenever available. Patients were g

Limbic-predominant age-related TDP-43 encephalopathy neuropathologic change and microvascular pathologies in community-dwelling older persons.

Limbic-predominant age-related transactive response DNA-binding protein 43 (TDP-43) encephalopathy neuropathologic change (LATE-NC) and microvascular pathologies, including microinfarcts, cerebral amyloid angiopathy (CAA), and arteriolosclerosis are common in old age. A relationship between LATE-NC and arteriolosclerosis has been reported in some but not all studies. The objectives of this study were to investigate the frequency of co-occurring LATE-NC and microvascular pathologies and test the hypothesis that arteriolosclerosis, specifically, is related to LATE-NC in brains from community-dwe

Cardiac Structure Relates to Hemorrhagic Cerebral Small Vessel Disease Phenotype.

BACKGROUND: Most intracerebral hemorrhages (ICH) are caused by 1 of 2 cerebral small vessel diseases (cSVDs): arteriolosclerosis and cerebral amyloid angiopathy (CAA). Hypertension is a major risk factor for ICH, but its contribution to the hemorrhagic manifestations of these arteriopathies remains uncertain. We investigated associations between a cardiac structural biomarker of systemic hypertension (left ventricular mass [LVM]) and cSVD neuroimaging phenotype in patients with ICH. METHODS: We assessed brain magnetic resonance imaging and echocardiography cross-sectional data from patients w

Relationship between cerebral small vessel disease and proteinopathies in the medial temporal lobe.

The medial temporal lobe (MTL) is strategically important for cognition and the pathogenesis of Alzheimer's Disease (AD). Cerebral small vessel disease (CSVD) independently contributes to cognitive impairment and is believed to play a role in AD. CSVD and proteinopathies related to AD often coexist in the MTL but neither the severity of CSVD, nor the associations between these pathologies have been quantitatively addressed in this brain region. We hypothesized that the severity of CSVD in the MTL is associated with the local burden of proteinopathies implicated in neurodegeneration (tau-tangle

Microvascular brain pathology and late-life motor impairment.

OBJECTIVE: To test the hypothesis that microvascular brain pathology is associated with late-life motor impairment. METHODS: More than 2,500 persons participating in the Religious Orders Study or the Memory and Aging Project agreed to annual motor assessment and autopsy. Brains from 850 deceased participants were assessed for microvascular pathology including microinfarcts, cerebral amyloid angiopathy, and arteriolosclerosis, and we examined their association with global motor scores proximate to death. RESULTS: Mean age at death was 88.5 years. More than 60% of cases had evidence of 1 or mo

Hypertensive encephalopathy in cats with reduced renal function.

The clinical, hemodynamic, and pathologic features of hypertensive encephalopathy in two cats with reduced renal mass are described. The cats developed a progressive syndrome of lethargy, ataxia, blindness, stupor, and seizures following an abrupt increase in blood pressure associated with a surgical reduction in renal mass. The cats had severe gross brain edema, evidenced by cerebellar changes of caudal coning and cranial displacement over the corpora quadrigemina and cerebral changes of widening and flattening of the gyri. Histologically, interstitial edema was most pronounced in the cerebra

Association of the Presence and Pattern of MRI Markers of Cerebral Small Vessel Disease With Recurrent Intracerebral Hemorrhage.

BACKGROUND AND OBJECTIVES: Assessing the risk of recurrent intracerebral hemorrhage (ICH) is of high clinical importance. MRI-based cerebral small vessel disease (SVD) markers may help establish ICH etiologic subtypes (including cryptogenic ICH) relevant for recurrence risk. METHODS: We investigated the risk of recurrent ICH in a large cohort of consecutive ICH survivors with available MRI at baseline. Patients with macrovascular, structural, or other identified secondary causes (other than SVD) were excluded. Based on MRI findings, ICH etiology was defined as probable cerebral amyloid angiop
