The adult acquired flatfoot is a challenging clinical entity with numerous procedures available to the surgeon for correction of the deformity. In a supple stage II flatfoot, naviculocuneiform arthrodesis with appropriate ancillary soft tissue augmentation and other adjunctive osseous procedures has proven effective with reproducible success. It may be used to balance a symptomatic flatfoot, re-establishing medial column stability and alignment while maintaining essential tri-tarsal joint motion. In addition to a review of the evidence-based medicine regarding this procedure, a detailed surgic…

Although RSL fusion is a viable option for isolated radiocarpal arthritis, the enthusiasm for this procedure should be tempered with the reality that kinematics of the wrist is not entirely suited for independent midcarpal flexion and extension [10]. Limited wrist flexion and extension is expected following a successful RSL arthrodesis. The effects of imposed abnormal kinematics are further shown by the high incidence of RSL nonunions, occurrence of scaphoid fractures, and postoperative deterioration of the midcarpal joint [15,22]. In a young patient with posttraumatic arthritis or rheumatoid …

Isolated talonavicular fusions have most commonly been performed for pes valgus deformity, rheumatoid arthritis, and post-traumatic arthritis. Recent reports have advocated its use for posterior tibial tendon dysfunction. The isolated fusion of the talonavicular joint is an effective procedure to reduce rearfoot biomechanical faults. This allows the procedure to not only address local pathology but general rearfoot deformities as well. Complications with the procedure are rare.

It has been generally accepted that residual cartilage and subchondral bone has to be removed in order to get bony fusion in arthrodeses. In … eemed to be the mechanism behind cartilage disappearance. The stability of the fixation achieved at arthrodesis surgery is an important factor in determining success or failure. Dowel arthrodesis without additional fixation proved to be deleterious. A good fit of the bone surfaces appears necessary. In the ankle joint, it would be technically demanding to ret …

Subtalar arthrodesis is an accepted surgical procedure for hindfoot disorders including rheumatoid arthritis, postinfectious arthritis, degenerative joint disease, trauma, neuromuscular disorders, and residual of congenital foot deformities. This procedure has the advantage of joint specific stabilization without restricting motion in the uninvolved transtarsal joints. Several surgical techniques have been described with good long term results. The purpose of this report is to present the indications, describe a surgical technique, and discuss expected results of the procedure currently used. …

This retrospective review of midcarpal arthrodesis was undertaken between July 1985 and March 1990. Fourteen patients were identified and 11 were available for follow-up evaluation. Mean follow-up time was 66 months (range, 48-102 months). Four patients eventually underwent total wrist arthrodesis due to continued pain. Seven patients improved and each returned to work between 3 and 4 months after surgery. Wrist motion was 50% and grip strength 65% of the normal side. Midcarpal arthrodesis has a predictable fusion rate; however, it does not reliably relieve pain.

A review of eight patients who underwent nine tibiocalcaneal arthrodeses was performed. The surgical indications were failed previous surger … in two patients, wound slough in one patient, and a prominent fibula in one patient. Tibiocalcaneal arthrodesis should be reserved as a salvage procedure because it is technically difficult and has a significant risk of complications.

This article reviews the surgical treatment of talar avascular necrosis. Specifically, arthrodesis for this complex entity and potential treatment of nonunions are discussed. The hallmarks of treatment are evolving and can range from nonoperative measures to amputations. Nonoperative treatment and the results of current arthrodesis techniques for late-stage avascular necrosis are reviewed. Surgical correction requires an understanding of the condition's natural history, utilization of structural and nonstructural bone grafting techniques, and stable fixation. Although the methods described fol…

OBJECTIVES: To compare the clinical outcomes and quality of life of patients surgically treated for lumbar spinal stenosis with decompression and posterolateral fusion, and decompression with interbody fusion.
METHODS: The study included 88 patients with lumbar canal stenosis who underwent surgery treatment (decompression and interbody fusion in 36 patients and decompression and posterolateral fusion [PL] in 52 patients). The clinical outcomes were assessed using the Oswestry Disability Index (ODI), Roland-Morris (RM) functional disability scale, and visual analog scale (VAS) for pain. These

Arthrodesis of the wrist may be indicated for a variety of conditions and can be achieved by many techniques. We have had experience with radiocarpal arthrodesis by fusion of the capitate to the radius after a modified proximal row carpectomy . The common feature in our 10 patients was a flexion deformity of the wrist. In five of the patients it was the result of spastic posturing. Five of the patients had a variety of other conditions. Primary arthrodesis occurred in all patients and the cosmetic improvement was appreciated by all patients. Functional improvement seemed to be most related to …

Cavus foot is a complex podiatric deformity that requires precise and in-depth work-up through an objective, physical, and radiographic examination. The goal of surgical treatment is to eliminate pain while establishing a plantigrade foot structure. Triple arthrodesis has proven to be an effective surgical procedure for treatment of moderate to severe rearfoot deformity with or without the presence of rearfoot arthritic changes. The foot and ankle surgeon must always be aware that no two cavus deformity cases are alike, therefore one may require additional surgical procedures including soft ti…

BACKGROUND: Gait asymmetries have been reported following ankle arthrodesis. However, similar reports do not exist for tibiotalocalaneal arthrodesis (TTCA), which involves further articular fusion. This study aimed to assess the extent of gait asymmetry following TTCA when compared to ankle arthrodesis.
METHOD: Gait assessment was performed on 36 participants, including 12 ankle arthrodesis, 12 TTCA and 12 controls - using 3-D inertial sensors and pressure insoles. 48 gait parameters were monitored on both operated and non-operated sides. Questionnaires including AOFAS, FAAM, EQ-5D were used …

About 90 % of all cases of painful posttraumatic ankle arthritis can be very successfully treated with a minor invasive ankle arthrodesis technique by using a small anterior approach and a fixation with four 6. 5 mm screws of which the posteromedial and transfibular one are inserted percutaneously. The results with this standardized procedure have been reported previously as excellent and good in a mid-term run of 6 years (34). This technique leads to a high union rate of 99% (92 of 93) with rapid bone healing within 8 ± 2 weeks, it causes a low minor complication rate of 8 % and enables a sig…

Shoulder arthrodesis is an end-stage salvage option for the failing, painful joint that cannot undergo or has failed reconstruction. It is indicated for irreversible and nonreconstructible massive rotator cuff tears and deltoid muscle denervation as well as for detachment of the deltoid from its origin. Rarely, arthrodesis is done to stabilize the glenohumeral joint after many failed attempts at shoulder reconstruction. Arthrodesis for failed prosthetic arthroplasty or tumor resection presents additional challenges because of the associated bone loss on the humeral and/or glenoid side of the j…

The author has analysed the results in 14 patients who had a hip arthrodesis carried out 18 years ago. It may be concluded that the vast majority of these patients have generally been satisfied with their condition during this period and that they are capable of performing their physical activities. Most of these patients showed certain changes in the surrounding bonejoint structures, while no lesions of the opposite hip were found. Therefore, the hip arthrodesis merits to be a choice for a correct treatment as long as the other hip is healthy, and the lumbar spine and knee, of the same side, …

BACKGROUND: Subtalar arthrodesis is the surgical procedure commonly performed to treat subtalar arthritis. Subtalar arthrodesis may have a higher nonunion rate if there is a preexisting adjacent joint arthrodesis. The aim of this retrospective cohort study was to compare the subtalar arthrodesis union rate of patients with native tibiotalar joints to that of patients with prior tibiotalar arthrodesis. The secondary aim was to assess risk factors for nonunion.
METHODS: A retrospective cohort study of consecutive patients that underwent a subtalar arthrodesis in a single center between 2010 and…

Four patients who had had grade-III or grade-IV symptomatic spondylolisthesis and a failed posterior or posterolateral arthrodesis were treated with transperitoneal anterior lumbosacral interbody arthrodesis and fixation with a fibular graft; the procedure was sometimes followed by posterolateral arthrodesis of the spine. The patients were examined ten to twenty-two years after the salvage operation. At follow-up, all had a solid fusion of the fifth lumbar to the first sacral vertebra. No neurological deficits had worsened and no iatrogenic neurological deficits had developed. The slip angle h…
