In a series of 300 total hip replacements, nine (3 per cent) dislocated. Precise measurements of the orientation of the acetabular cup were made and it was found that anterior dislocations were associated with increased acetabular-component anteversion. There was no significant correlation between cup-orientation angle and posterior dislocation. The dislocation rate for cup orientation with anteversion of 15 +/- 10 degrees and lateral opening of 40 +/- 10 degrees was 1.5 per cent, while outside this "safe" range the dislocation rate was 6.1 per cent. Other factors that were documented include

OBJECTIVE: To evaluate the influence of patellar replacement on total knee arthroplasty by comparing with non patellar replacement.
METHODS: Between September 2010 and November 2010, 63 patients (63 knees) with osteoarthritis who met the selection criteria and underwent total knee arthroplasty, were randomly divided into 2 groups: patellar replacement in 32 cases (replacement group), non patellar replacement in 31 cases (non patellar replacement group). There was no significant difference in gender, age, disease duration, osteoarthritis grading, the clinical and functional scores of American

UNLABELLED: Conversion of a failed surface hip replacement to a conventional total hip arthroplasty is reportedly a straightforward procedure with excellent results. We compared perioperative parameters, complications, and clinical as well as radiographic outcomes of 39 hemi and total hip resurfacing conversions with conventional THAs. The hips were matched by diagnosis, gender, age, body mass index, preoperative Harris hip score, and followup time to a cohort of primary conventional THAs performed during the same time period by the same surgeon. The mean operative time was longer (by 19 minut

PURPOSE: Frequency of hip impairment, with sub-luxation or dislocation, during evolution of neuromuscular diseases depends on intensity and spreading of palsy. At the end of growth or at the beginning of adult life, secondary arthritis can induce pain and lack of mobility. The importance of the chondropathy and irreducible lack of congruence may doom to failure a conservative articular or periarticular surgery. Total hip arthroplasty is an alternative, but the risks of dislocation, ectopic ossifications and infection make often refute this indication. We report our experience of total arthropl

OBJECTIVE: To introduce the technique of total hip surface replacement, evaluate the early results and review the factors which affect the results.
METHODS: From October 2000 to January 2005, 31 patients (37 hips) with osteonecrosis, osteoarthritis, hip dysplasia, ankylosing spondylitis were treated with the total hip surface replacement. Among them, 15 were male, and 16 were female, with an average age of 42 years (range from 23 - 65 years). All the 31 patients had the indications for hip surface replacement. Standard operation technique which was brought forth by Amstutz and Nelson was empl

Radial head arthroplasty is indicated for displaced comminuted radial head fractures that cannot be managed reliably with open reduction and internal fixation and that have an associated elbow dislocation. Replacement also is indicated in patients with comminuted radial head fractures that have or are likely to have a disruption of the medial col-lateral, lateral collateral, or interosseous ligaments. Biomechanical studies have demonstrated that metallic implants restore elbow stability similar to the native radial head. The early and midterm clinical experience with metallic radial head arthr

Dogs, goats, and sheep are the most commonly used animals in studies of total hip replacement (THR) arthroplasty. However, the development of a small, laboratory animal model could have significant economic and housing advantages over these models when studying the long-term systemic effects of chronic mechanically functional implants. A THR was designed and fabricated with a cobalt-alloy (F-75) femoral and UHMWPE acetabular component for PMMA-cemented implantation in the rat. Mature Sprague-Dawley rats (n = 18) were maintained at a constant weight (450 g) and were trained to walk on a low-spe

BACKGROUND: Surface replacement arthroplasty is a reconstructive alternative for the treatment of pain and deformity due to osteoarthritis and rheumatoid arthritis of the proximal interphalangeal joint of the finger. This retrospective study was performed to examine long-term outcomes of proximal interphalangeal joint prosthetic surface replacement with a proximal cobalt-chromium (CoCr) and distal ultra-high molecular-weight polyethylene component over thirty years at a single institution.
METHODS: Sixty-seven prostheses were implanted in forty-seven patients between 1974 and 2007. The mean d

Transfusion of homologous blood components is associated with immunological (incompatibility, alloimmunization, immunosuppression) and infectious risks (hepatitis, cytomegalovirus, HIV and other agents). Endoprosthetic surgery of the hip and knee frequently requires transfusion. Preoperative deposit of autologous blood can reduce homologous transfusion requirements. The simplest method is liquid storage of whole blood. In order to re-examine the efficiency of our present scheme of preoperative deposit, we studied patients scheduled for endoprosthetic surgery with respect to the amount of blood

BACKGROUND: Hormone replacement therapy (HRT) is commonly prescribed to alleviate symptoms of menopause. Incidence of women undergoing total shoulder arthroplasty (TSA) continues to rise, yet the impact of HRT on primary TSA outcomes remains unclear. The purpose of this study is to examine the effects of HRT on postoperative complications following primary TSA.
METHODS: The PearlDiver Mariner database was queried for menopausal aged females undergoing primary TSA between 2010 and 2023. Patients with prior history of breast cancer, deep vein thrombosis (DVT), pulmonary embolism, or stroke were

INTRODUCTION: Distal femoral replacement (DFR) revision surgery represents significant cost to the healthcare systems as well as significant patient morbidity/mortality. A retrospective review of revision knee surgery with DFR would elucidate a treatment algorithm for patients with complex reconstructive issues. In this study, we review patients' health metrics to better understand indications and potential complications for DFR revision. The data can then provide insight to specific patient factors that may put patients at higher risk for revision surgery.
METHODS: Seventy-one patients who u

BACKGROUND: Distraction arthroplasty aims to treat ankle osteoarthritis while preserving the native ankle joint, often to prevent or delay ankle arthrodesis or total ankle replacement (TAR). No study to date has explored TAR outcomes in patients who have had prior distraction arthroplasty. Thus, this study described the clinical, radiographic, and patient-reported outcomes for TAR at minimum 2-year follow-up in patients who had undergone prior ankle distraction arthroplasty.
METHODS: This retrospective review included 19 ankles in 17 patients who underwent TAR at a single institution subseque

INTRODUCTION: Total knee arthroplasty is a common orthopaedic procedure in the UK; consequently, revision surgery and periprosthetic fractures are increasing in incidence. Strategies for management of these cases include non-operative strategies, internal plate fixation and revision of the distal femoral component. One under-reported practice is to perform distal femoral replacement in cases with poor distal femoral bone stock.
MATERIALS AND METHODS: The department's electronic database was searched for all patients undergoing revision of total knee arthroplasty. From these, all patients havi

Total knee arthroplasty, or replacement (TKR), is now the most commonly performed surgical procedure performed in adults with haemophilia. It is indicated when end-stage haemophilic arthropathy results in intractable pain and reduced function. In patients with haemophilia, however, there has always been a concern about the high risk of infection, which carries with it potentially catastrophic consequences. The aims of this study were to review the case series of TKR for haemophilic arthropathy published in the medical literature, comparing the published infection rates and the differing clotti

BACKGROUND: Surface replacement arthroplasty aims to re-create normal hip biomechanics; however the pathoanathomy of the hip, prosthetic component design, surgical technique and other factors may have a significant impact on the range of motion restoration attained following surface replacement arthroplasty. However, there is paucity of information on the effect of such factors.
METHODS: A computerized three-dimensional hip model was created from preoperative computerized tomography images of a patient who was scheduled for a surface replacement arthroplasty. The effects of the femoral compon

The functional outcome of hip replacement following a Girdlestone arthroplasty may be difficult to predict. We reviewed 39 hips in 39 patients with a minimum follow-up of 12 (range 12-208) months from re-implantation total hip arthroplasty. The patients were treated in one institution between 1983 and 2000, and their mean age at conversion was 65 (32-85) years. The main indication for the Girdlestone arthroplasty was peri-prosthetic infection. A post-operative complication occurred in 26 cases and in 17, surgical revisions were performed. The average Harris hip score (HHS) following conversion
