Arthroscopy of the hip has become a well-established procedure for minimally invasive therapy of hip disorders. The surgical technique is demanding. A fracture table for distraction of the joint is required, as well as an exact positioning technique is most important. Because of eh great distance between skin and joint only restrictive intrarticular maneuvers are possible. Arthroscopically the hip can be divided into a central and peripheral compartment. For arthroscopy of the central compartment distraction of the joint is necessary. This part comprises the loaded surface of the femoral head

Arthroscopy of the knee was first performed early this century. However, it became generally accepted and grew to be a major contributor to orthopaedic surgery only during the last two decades. It has improved our understanding of numerous knee pathologies and allowed us to treat many of these disorders with considerably less morbidity. But there remain areas that warrant attention. Training in arthroscopic technique needs to be intensified and our knowledge of knee conditions should be improved if we are to avoid certain pitfalls and complications during arthroscopic surgery. Another issue th

We present experience from 37 arthroscopies of the elbow. 15 surgical procedures were performed, nine of which were extractions of free bodies. One deep infection occurred (in the first patient, on whom arthrotomy was performed as well), but no neurovascular complications occurred. Three preoperative diagnoses were changed, and a diagnosis could be made in 14 cases of unknown etiology. Exploratory arthrotomy was avoided in 13 patients. In our view the method is indicated in cases of pain of unknown cause, free intraarticular bodies and/or osteochondritis, and locking episodes where no reason c

Elbow arthroscopy is a valuable adjunctive means to diagnose difficult elbow problems and is a relatively atraumatic means to remove loose bodies. It is also helpful with the patient in whom the objective findings do not coincide with persistent and significant subjective complaints. However, the use of elbow arthroscopy in a routine fashion is to be avoided, and the procedure should have limited indications at this time. Ideally it should be done primarily only by those experienced in the technique of arthroscopy who possess a significant knowledge of elbow anatomy and pathology.

Arthroscopic surgery is an established technique and is frequently done as an out-patient procedure. The technique requires two or three portals of entry into the joint with visual triangulation of operating instruments towards the area of pathology. Increased diagnostic accuracy, definitive endoscopic treatment, and minimal morbidity are major advances derived from arthroscopy.

The operative technique and indications for shoulder arthroscopy have been reviewed, with a description of proper technique and instrumentation. Visualization within the joint is usually superior to visualization with arthrotomy. With the refinement of techniques and equipment, it is expected that the frequency of transarthroscopic procedures will increase in the near future.

UNLABELLED: Obesity is a major public health issue, as incidence is rising in all developed countries, although the proportion is lower in Europe than in the U.S. Over and above the metabolic consequences and increased risk of diabetes, cardiovascular pathology and certain forms of cancer, the present study focuses on osteoarticular risk, and in particular on pathologies manageable by arthroscopy. It also analyzes results and complications specific to arthroscopy in these indications. Meniscal and ligamentous pathologies of the knee, rotator-cuff pathology in the shoulder and tendon pathology

The past decade has brought a number of advances in the use of hip arthroscopy to diagnose and treat intra-articular pathology of the hip joint. As such, hip arthroscopy has been used to treat patients who otherwise may have needed a complicated open procedure or have gone untreated. The history and physical examination of the patient as well as imaging modalities, such as magnetic resonance imaging, play a vital role in determining a successful outcome. Patient selection also is an important component of success. As the technique and instrumentation of hip arthroscopy improves, this procedure

As our understanding of the anatomy and function of the elbow joint continues to grow and technology continues to advance, our ability to correct disorders of the elbow with arthroscopic techniques will expand. Today, we are at the brink of major advances in the arthroscopic evaluation and treatment of elbow ailments. Many open surgical procedures currently being performed will undoubtedly be adapted for an arthroscopic approach, as we are already seeing in the treatment of radiocapitellar arthrosis, tennis elbow, arthrofibrosis, and ulnohumeral arthroplasty. Elbow procedures, such as ligament

This article describes the surgical technique of arthroscopy of the ankle and the indications for its use. In addition to describing the common anterolateral and anteromedial arthroscopic portals, a discussion is also given regarding portal anatomy of ankle arthroscopy and the various neurovascular structures that lie in close proximity to the arthroscopic portals. Arthroscopy of the ankle has a definite role in the management of certain lesions of the ankle, such as loose bodies, or osteochondral or chondral defects of the talus. Ankle arthroscopy is a technically demanding surgical procedure

The procedure for diagnostic shoulder arthroscopy and proper positioning of operative portals are discussed. The superior medial portal approach to the shoulder is described. Conditions visualized by arthroscopy include the following: glenoid labrum tears, synovitis, loose bodies, instabilities, adhesive capsulitis, rotator cuff lesions, and extra-articular decompressions.

For arthroscopy of the elbow to be safe and effective, the arthroscopist must develop a systematic and reproducible arthroscopic examination technique. We believe that the technique and the identification of anatomic structures described in this article provide a safe and reliable means of performing this procedure. Attention to detail, especially when establishing the portals, is essential in performing a safe examination and in avoiding damage to the neurovascular structures. We prefer the 4-mm, 30-degree-angled arthroscope. It provides the wide field of view necessary for both diagnostic an

Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association1985eng
A technique of diagnostic and surgical arthroscopy of the elbow is presented, and the normal intraarticular anatomy as viewed from the anterolateral, anteromedial, and posterolateral portals is described. A preliminary study of 12 patients who underwent surgical arthroscopy of the elbow demonstrated that removal of loose bodies produced the best objective and subjective results. Less satisfactory results were obtained when procedures such as capitellum and radial head chondroplasties were performed. Using a preoperative and postoperative point accumulation rating system for four objective and

Between December 1979 and June 1982, 74 arthroscopic examinations of the shoulder were done at our institution. Arthroscopy was done in 28 shoulders to evaluate glenohumeral instability, in 23 to assess the joint and tendons before acromioplasty for chronic supraspinatus tendinitis, in 8 to diagnose or stage arthritic diseases, in 7 to define rotator cuff tears, in 3 to assess long-standing periarthritis, in 2 to visualize intra-articular fractures, in 2 to treat infections, and in 1 to evaluate the extent of pigmented villonodular synovitis. Examination of the shoulder with the patient under

Fiberoptics permit a minimally invasive direct inspection of the knee joint. Arthroscopy is indicated when no definitive clinical diagnosis can be made. An organized approach includes inspection of the suprapatellar pouch, the articular surfaces of the patella, the medial and lateral compartments of the knee and the intercondylar area. Common afflictions diagnosed by arthroscopy include chondromalacia, meniscal lesions, osteoarthritis, cruciate ligament injuries, chronic synovitis and loose bodies. Complications and morbidity are minimal.

A diagnosis by clinical examination and arthrography was not possible in 160 out of 500 arthroscopically examined patients, most of them with post-traumatic knee disorders. In 157 cases the clinical diagnosis was wrong, and in another 58 cases incomplete. Only in 89 patients (18%) arthroscopy proved the clinical diagnosis to be correct. At arthroscopy, a meniscal injury was found in 156 patients (medial meniscus 57, lateral meniscus 64, both menisci 8). With regard to the menisci a previous arthrography was found correct only in 103 out of 213 cases, i.e. in 48%. Lesions of the articular carti
